Monday 3rd September 2014 Dear Parents & Caregivers, Thank you for returning the previous form advising us of the likelihood of your child attending our Year 5/6 Camp to Lakewood Lodge (www.lakewoodlodge.co.nz) Our school camp will run in Week 3 of Term 4, from Tuesday 28th October to Friday 31st October 2014 and will cost $255.00 per student. The total cost per student includes accommodation, transport, food, activities and specialist instructors. Fee payment for school camp is due before Friday 24th October 2014, although installments and other arrangements can be made via the school office. Prior to signing the consent form below, please read and complete the attached Medical Form. It is important you understand and complete both forms in full, so as to provide our school and Lakewood Lodge staff with all the necessary information.
