    +. 6he office lift is overcrowdeddaily in the peak hours %morningcheck7in time, afternoon lunchbreak and evening check7outtime'. 6his causes some staffsespecially senior and ladiesstaffs to get suffocated and feeluncomfortable. 6his also affectsthe productivity of staffs whenthey e/perience it in the morningcheck7in time.

    . 6he tube light fi/ture on the roof in the office hall area in 8 th floor is found to be hanging loose dueto improperly tightened boltswhich attach the fi/ture onto theroof. 4lso the bolts are found tobe in corroded state. 6hiscondition creates a ha-ardousenvironment and risk of droppedobject on staffs who pass byunder the fi/ture.

    0. 6he 4* vent in the store room in9th  floor cause stinky odour tospread throughout the room.6his probably due to dirty,clogged vent filters which wasnot cleaned for the last years.ven though other vent filter inthe office are subjected toroutine maintenance and

    cleaning, the vent filter in thestore room has been neglectedcausing uncomfortable smelland poses risk of suffocation for the office boys who visit thestore room for various purposes.

    3. 6he stair case way which isused as emergency escaperoute is obstructed at 8th floor of the office building with piles of old office files stacked to 37 feetheight. In addition to this, theescape route in +;th  floor of 

    +. "nly limited number of staffs shall beallowed to enter the lift at a time. 6hisshall be done under the supervisionof building watchman.

    . Discussion with building managementshould be held to set a policyregarding the usage of lift with limitednumber of people so that no one getuncomfortable. &riority to enter the liftshould be given to ladies and senior citi-ens.

    >. 6he electric wire of kettle in thekitchen in 9th floor is found to bein poorly insulated condition dueto various reasons including

    improper usage. 6his can leadto the risk of electrical shock tothe staff who accidently touchthe poorly insulated area of thewire.

    8. 6he cord cover of trailing electricfle/ passing across the office

    walkways and aisles is found tobe damaged in 8th floor. 6houghinsulated and avoid electricalha-ards, it can cause tripha-ards and even head injury or fracture.

    ++. 6he photocopier in the 9th  floor office is found to be producinge/cessive noise when operating.6his creates uncomfortableworking environment to thosewho sit nearby the photocopier and prolonged e/posure to thisnoise can lead to hearingimpairment to those staffs.

    +. 6he electric sockets in someworkstation at 8th floor are foundto be in almost broken conditionand some are corroded. Staffsare found to be still using thesepartially broken2corrodedsockets to plug7in their laptopcharger pin. 6his poses risk of electrical shock to the staffs.

    office boys is found to be not instandard or safe way of liftingweights in =th floor. 6his can leadto various health problems suchas back pain2injuries, work7related upper limb disorders,muscular problems etc.

    +>. 6he way of using laptop bysome staffs in 9th  floor is notsafe. @or e/ample, using laptopunder poorly lighted area, and

    eye to laptop monitor is not incorrect level etc. 6his can causeeye strain and eye irritation tothe staff. 4lso poorly lighted areacan cause trip ha-ards to staffswho pass by.

    6he health and safety inspection revealed that overall health and safety system in the officebuilding is good. Cood housekeeping practices were observed in most of the floors. 4lso,safety facilities such as first aid kit, welfare facilities such as drinking water facility, locker facility, sufficient lighting, eating2rest area, comfortable temperature, good ventilation, cleanand hygienic work stations etc were observed. #owever, there are some areas of concernregarding the ade)uacy of safety facility in some floors, especially 8 th and 9th floors. "ut of ;ha-ards observed during the inspection, main ha-ards, to which this report focuses, aregiven below:

    +. &oorly insulated electric wire of kettle in 9th floor posing electric risk.. 6he unavailability of drinking water facility in 8th floor posing risk of dehydration.0. "bstructed emergency escape route in +th and +;th floors posing risk of entrapment

    during emergency situation.3. ack of first aid bo/ and materials in =th and +th floors posing risk of injury escalation.. Ansafe way of using laptop posing ergonomic related risks.

    3& %AIN !ININGS

    3&5  #oorly  in(ulated electric 0ire of ettle

    6he electric wire of kettle in the kitchen in 9 th floor is found to be in poorly insulated condition

    due to various reasons including improper usage. 6he kettle is reported to be 0 years old andis a reputed brand product. #owever due to improper usage practices by some staffs, water consistently come in contact with the wire causing the wear and tear of the electric wire. 6hisposes the risk of electrical shock to the staffs who accidently touch the poorly insulated area of the wire. "nly this kettle is available in the 9th floor kitchen and ;E employees uses this kettleon average times a day. 6he increased fre)uency of usage increases the probability of risk.

    6he article7> of International abour "rgani-ation %I"' convention: *+>8 7 Safety and #ealthin *onstruction *onvention which stipulates that Fall electrical e)uipment shall be installed andmaintained by a competent person, and so used as to guard against dangerG is found to beviolated in this case.I recommend providing immediately a temporary insulation to the re)uired area by means of insulation tape to avoid any electric ha-ards. ater on, the kettle can be replaced with newone. I also recommend providing the staff awareness about the proper usage the kettle and tointroduce defect reporting system in the office so that staffs who see the defect can report it tomanagement. In addition to these, regular inspection for the condition of kettle body and itswire is re)uired.

    3&7 The una)aila+ility of drinin- 0ater facility

    6he drinking water facility is not provided in 8th floor. 6he staff re)uired to go to other floors justto drink water. 6he reason being said to be few staffs are working in the 8th floor. &rolongede/posure to this condition can leads to dehydration of staffs.

    3&3 O+(tructed e*er-ency e(cape route

    6he stair case way which is used as emergency escape route is obstructed at + th floor of theoffice building with piles of old office files stacked to 37 feet height. In addition to this, theescape route in +;th floor of office building also found to be obstructed with building materialsby the building management which can hinder easy movement of staffs in case of anemergency situation. 6hese unsafe conditions pose the risk of being trapped or get delayed toreach the assembly point during an emergency.

    6he first aid bo/ is found to be empty in = th floor. In +th floor, only paracetamols are found inthe bo/. "ther necessary first aid materials such as cotton, adhesive bandages, tapes, anti7septic papers, anti7biotic ointments etc are found to be missing. 6his can cause delay ingetting first aid to staffs in emergency in these floors.

    6he way of using laptop by some staffs in 3 th floor is not safe. @or e/ample, using laptop under poorly lighted area, and eye to laptop monitor is not in correct level etc. 6his can cause eye
