System Features

The modular case design offers improved aesthetics and better access to the instrumentation thus simplifying the procedure further. The inclusion of Elastosil® Handles ensures maximum surgeon grip as well as minimising heat retention following sterilisation.

Large Diameter Guide Wires

The Systems feature largerdiameter Guide Wires with taperedcore. This unique design reducespotential for a stress riser at thethread/shank junction. The larger diameter provides greaterbending stiffness to reduce deflection. This ensures the most precise of screw placement.

Operative Technique - 4.0mm

Insert Guide WireUsing the ø1.4/2.7mm Double Drill Guide,insert a ø1.4x150mm Guide Wire to the appropriate depth.

Use Image Intensification to control reduction and Guide Wire placement. Place additional Guide Wires as necessary. Remove the Double Drill Guide.

Note: In dense bone, puncturing the cortex with the ø1.4x150mm Drill Bit to initiate the Wire may reduce potential heat and/or deflection.

Alternative: Substitute the ø1.4x150mm Guide Wire with a ø1.4x150mm Drill Bit. Throughout the procedure it is possible to interchange the Guide Wire and Drill Bit.

Option: A Parallel Drill Guide is available for parallel placement of Guide Wire.

Countersink or Washer?Where soft tissue coverage is minimal, use of the Countersink to further recess the low profileScrew Head may be beneficial. Care should be taken to ensure the Cortex is not countersunk past its capacity.

In osteoporotic bone or where the Cortex is thin,Washers can be applied to spread the load of theScrew Head over a greater area.

Note: The Countersink design does not require pre-drilling.

Measure for Screw LengthSlide the Direct Measuring Gauge over the ø1.4x150mmGuide Wire. The Direct Measuring Gauge measures directto the tip of the Guide Wire. This ensures that the finalScrew position corresponds with the initial tip positionof the Guide Wire.

Select appropriate Screw length. Length adjustment isparticularly important if the tip is near an articular surface.

Note: Care should be taken to ensure the Direct Measuring Gauge tip engagesthe bone when a reading is taken.

Insert ScrewUsing the Cannulated Screwdriver with Elastosil® Handleand the Screw Holding Sleeve, insert the selected screwover the Guide Wire. Release the Screw Holding Sleeveprior to final tightening.

Remove Screwdriver and Screw Holding Sleeve.

Note: Always verify Guide Wire and Screw Position with Periodic Image Intensification.

Verify Final ReductionVerify the final position of the Screw.

Remove and discard the Guide Wire.

Repeat as necessary for additional screws.

• Medial and lateral malleolar and pilon fractures

• Proximal and distal humerus fractures

• Os calcis, talar and patellar fractures

• Ligament fixation of the proximal humerus

• Fractures of the pelvis and acetabulum

• Arthrodesis of the tarsals

Step one

Insert Guide WireUsing the ø2.0/3.5mm Double Drill Guide, insert a ø2.0x150mm Guide Wire to the appropriate depth.

Use Image Intensification to control reduction and Guide Wire placement. Place additional Guide Wires as necessary. Remove the Double Drill Guide.

Insert Guide WireGuide Wire insertion

Using an appropriate Power Tool, the 3.2x300mmGuide Wire is inserted to the appropriate depth througha tissue guide assembly. Use Image Intensification tocontrol reduction and Guide Wire placement.

Multiple Guide Wire insertion

Connect the Parallel or Multiple Drill Guide to theElastosil® Hammer Handle. Using an appropriate Power Tool the 3.2x300mm Guide Wire is inserted to the appropriate depth. Use Image Intensification to control reduction and place further parallel Guide Wires as necessary.

Note: If washers are required ensure correct screw spacing is chosen.

Technical Tip

Avoid Inferior Screw placement below the level of the Lesser Trochanter to avoid PostoperativeSubtrochanteric Fractures. In the A/P view the initialGuide Wire should be placed at the level of the LesserTrochanter and inserted along the Inferior portion ofthe Femoral Neck. The Lateral view should show theGuide Wire centrally located in the Femoral Neck and Head. Anteversion may be checked by placing a Guide Wire over the Anterior Neck.

Operative Technique - 6.5/8.0mm

Indications 6.5/8.0mm6.5/8.0mm Indicators are as follows:

• Intracapsular fractures of the femoral neck

• Intertrochanteric fractures of the femur

• Tibial plateau fractures

• Fractures of the dorsal pelvic ring

• Pelvic sacroiliac joint disruptions

• Ankle arthrodesis

Measure for Screw LengthRemove the Guide Wire Insertion Sleeve then slide theDirect Measuring Gauge over the Guide Wire. The Direct Measuring Gauge measures directly to the tipof the Guide Wire. This ensures the final Screw positioncorresponds with the initial tip position of the Guide Wire.

Select appropriate screw length, always ensuring to selectthe lesser of the two sizes, if the measurement is betweensizes. It is recommended to subtract between 5-10mmfrom the measurement on the Direct Measuring Gauge to avoid the potential penetration of the Femoral Head*.

Note: The Screw Insertion Sleeve is designed to allow the Direct Measuring Gauge to pass through it. This reduces potential softtissue damage and larger insertions.

This may not be necessary in other indications where it is desirable to insert a screw which matches exactly the position of the Guide Wire.Thus maximum thread purchase in the cortex is achieved.

Insert ScrewUsing the Cannulated Screwdriver with Elastosil®

Handle and the Screw Holding Sleeve insert theselected screw over the Guide Wire. Release the screwHolding Sleeve prior to final tightening. Each Screw isinserted through the Soft Tissue Protection Sleeve.

Note: Always verify the Guide Wire and screw position with periodic Image Intensification.

Verify Final reductionVerify the final position of the screw.

Remove and discard all Guide Wires.

Repeat as necessary for additional screws.

Options: Tap /Drill - In hard or sclerotic bone, pre-drilling and pre-tapping may be necessary.

Cannulated Screwdriver with AO fitting - This can be used with either the Elastosil® Handle with AO Coupling or a Power Tool. If a Power Tool is selected final tightening must be carried out by hand to prevent stripping.

Cleaning: Care should be taken to utilise the Cleaning Stylet for Inter and Post-Operative cleaning of cannulations. Correct inter-operative use of this instrument prevents accumulation of debris.

Removal: It is recommended that the Solid Screwdriver be used for Screwremoval. This can apply greater torque and will reduce the potential for damaging the hex tip on the Screwdriver.

Single Use Items: Discard all single-use implants and instruments utilised during the procedure.
