Executive Summary

In April 2008, Aged Care Services Australia WA (the St Ives Group) commissioned Verso Consulting to undertake a Community Needs Study in support of their proposal to develop a Retirement Village and Residential Aged Care Facility in Carine, a suburb in the city of Sterling in North Perth.

This report focuses on a coastal area which can be considered the focal catchment area for the proposed retirement living development. This report analyses an area bordered by Cambridge Street and The Boulevard to the south, Whitfords Avenue to the north and Mitchell Freeway to the east. This area includes the proposed development site area and is referred to as the ‘Carine area’ or ‘study area’ in this report. This area falls within the Metropolitan North planning region of the Commonwealth Department of Health and Ageing (DoHA).

The objectives of this report are to:

• present a demographic analysis which outlines the current and projected aged care needs

• examine the current residential aged care services and their distribution across the study area and the wider Metropolitan North region, including an evaluation regarding the adequacy of the current allocation

• provide a commentary on the types of aged care services which may best meet the identified needs

A range of primary sources have been analysed: Australian Bureau of Statistics (ABS) Census of Population and Housing, 2001 and 2006; ABS Socio-Economic Indexes for Areas (SEIFA) data, 2008; ABS Survey of Disability, Ageing and Carers, 1998; Access Economics Report on Dementia Prevalence for Alzheimer’s Australia WA 2005; Carers WA Report “A Profile of Caring in WA”; Department of Veterans’ Affairs (DVA) Beneficiaries by Local Government Area (LGA); Department of Health and Ageing (DoHA) Service Lists 2007; WA Tomorrow Population Projections.

Outcomes of Demographic Analysis

• Metropolitan North is experiencing strong growth of the ageing population, with the current 55+ population of 115,776 forecast to rise to 175,060 by the year 2011 which is a 51.2% increase, which will result in increasing needs for housing developments suitable for older persons wishing to remain living within their local community

• In 2006, Stirling LGA (26.1%) had a greater proportion of persons aged 55+ than the Metropolitan North region as a whole (22.8%) indicating a high need for aged care housing in this part of the region

• Cambridge (2,857), Joondalup (9,128) and Stirling (17,489) LGAs are currently home to 68% of the population aged 70+ in Metropolitan North (42,953), confirming high needs for residential aged care in the local areas around the proposed development site

• Joondalup LGA is predicted to experience one of the steepest growth rates in WA for the 70+ population over the next decade – increasing by 73.1% to 15,800 persons by 2021. In Metropolitan North, this growth rate is eclipsed only by Wanneroo which is predicted to have a 151% increase to 16,300 persons. This

confirms projected increasing demands for aged care in areas adjacent to the catchment for this proposed development

• Population projections indicate that the 85+ population will increase by 53.4% in Metropolitan North by 2021. Current research into Alzheimer’s disease and other forms of dementia estimate that 25% of persons aged 85+ will experience moderate to severe dementia. These research findings confirm there will be ongoing needs for dementia care in this region and a requirement for retirement living and aged care developments to include provision for dementia-specific programs

• In the Study Area, 9.8% (1,741) of persons aged over 65 provide unpaid care, help or assistance to family members or others because of a disability, a long-term illness or issues related to old age. Anecdotal evidence suggests that the majority of these carers would be caring for their ageing spouse/partner. This proportion is higher than for the Metropolitan North region as a whole (9.2%). This suggests that a potential demand for local aged persons accommodation options suitable for older couples with healthy ageing and carer support programs incorporated into the development

Unmet Demands for Residential Aged Care

There are 68 residential aged care facilities in Metropolitan North offering approximately 1,545 high care and 2,017 low care places. Of these, 18 are located within the Study Area and offer 400 high care and 639 low care places.

The current DoHA allocation ratio for residential aged care places is 44:1000 (high care) and 44:1000 (low care) in reference to the population aged 70+. The provision ratios for Metropolitan North and the Study Area are:

This analysis confirms that the study area is under-resourced when compared to the DoHA planning ratio and the Metropolitan North level of provision as a whole. A local area analysis has revealed that Joondalup (28.2:1000 high care, 34.8:1000 low care) and Stirling (17.6:1000 high care, 45.8:1000 low care) will be significantly ‘under-bedded’ in future years given they are currently below the allocation ratio and are projected to experience high increases in the older population segment.

The future allocation of residential aged care places for Metropolitan North foreshadowed by the DoHA in 2007 (400 places in 2008-09 and 205 places in 2009-10) will not be adequate to keep pace with the rapidly increasing 70+ population. This will result in growing needs for community and home care programs linked to age-appropriate housing developments in this region.

Key Findings from Community Consultations

In 2008 a Community Needs survey was conducted with agencies in the region with key findings as follows:

• There are insufficient residential respite options (both low care and high care) within the Metropolitan North region which suggests that future residential care developments should include provision for respite beds

• “Many older persons are living in homes that are unsuitable or unsafe for older people to live in” (direct quote from survey participant)

• There are few senior-friendly housing developments in the established areas of the Metropolitan North region, requiring older persons to move away from ‘their area’ to find more appropriate housing

• There are insufficient day activity options for older persons, including recreation programs and dementia-specific day care, which suggests that retirement village developments in the region should include provision of facilities suitable for community programs

1.2 Defining the ‘Carine Area’

The St Ives Group has defined their area of interest as a coastal strip bordered by Cambridge Street and The Boulevard to the south, Whitfords Avenue to the north and Mitchells Freeway to the east. The proposed development site sits within this area and is located at 29 Silica Road, Carine, Western Australia, 6020.

It should be noted, however, that the catchment area for a service based in Carine may be wider than the area described above and depicted below.

The 2006 ABS Census indicates that 22.8% of the Metropolitan North population is currently aged 55+, which is slightly lower than that of the Study Area at 24.9%.

2.1.2 Projected 55+ Population in 2011

Projections prepared by the Western Australian Planning Commission, based on 2001 Census data, predict a substantial increase in the size of the population aged 55+, as detailed below. It is particularly worth noting the sizeable 44% increase projected for Wanneroo, which is well above that of the region (22.1%) and the state (18.9%).

The 2006 ABS Census indicates that 8.5% of the population of the Metropolitan North is currently aged 70+, which is slightly higher than the state figure of 8.4%. However, the proportion of the population aged 70+ in the Study Area is slightly higher at 9.5%.

2.3 85+ Population

The significance of reporting the 85+ population is that current research indicates that the average rate of moderate to severe dementia amongst Australians is about one in fifteen aged 65+; among people aged 80-84 the rate is one in nine, and among those aged 85+ it is one in four.1

Internationally, the incidence of dementia is reportedly higher than in Australia: 30% of Canadians over the age of 85 have Alzheimer's or a related disease2 and 30-40% of persons over 85 in the United States have Alzheimer’s disease”3.

Incidence of dementia potentially creates a range of special care needs which are compounded by more complex care requirements of the 85+ population and a higher likelihood of living alone. The 2006 ABS Census indicates that 1.5% of the Metropolitan North population is currently aged 85+, slightly higher than the state figure of 1.4%.

2.4 Unpaid Carers

The 2006 Census was the first to report on the number of people who spent time providing unpaid care, help or assistance to family members or others because of a disability, a long-term illness or issues related to old age.

The table below examines the numbers of persons aged 55+ and 65+ who provided unpaid assistance, and identifies potential demand for ageing carers who may seek a suitable, local residential aged care facility for their family member as their care needs increase. According to the 1998 ABS ‘Survey of Disability, Ageing and Carers’ 45% of male carers and 55% of female carers are caring for their spouse/partner.

It is significant that the proportion of unpaid carers in the Carine Study Area is higher than that of the Metropolitan North Region.

This data also highlights the need for carer support groups/activities in the Carine area and it is recommended that any new aged care facility specifically addresses this identified area of need.

2.5 Economic Profile

The analysis of Income, Tenure and Advantage/Disadvantage below indicates that the Study Area is relatively advantaged in relation to the wider Metropolitan North region and WA as a whole. This indicates strong demand potential for a residential aged care service which is prepared to offer Extra Services to residents.

2.5.1 Income

Weekly income figures reported by the 2006 Census indicate that there is a significantly higher proportion of persons aged over 55 years in the Study Area with a weekly income in excess of $800 as compared to Metropolitan North and to the state:

• Seven of the ten most advantaged LGAs in WA are located in the Metropolitan North planning region

• The Study Area is relatively advantaged in relation to the wider Metropolitan North region and WA as a whole. This indicates strong demand potential for a residential aged care service which is prepared to offer Extra Services to residents. 25.3% of persons aged 55+ and 9% of persons aged 70+ in the Study Area reported a weekly income in excess of $800 as compared to Metropolitan North (17.1% and 4.8%)

• The age 70+ and 85+ populations in Cambridge (69.1% and 50.7%), Joondalup (75.2% and 55.7%) and Stirling (68.6% and 51.9%) LGAs have are more likely to own/be purchasing their home than comparable cohorts in the wider Metropolitan North region (67.8% and 49.8%)

• The majority of Metropolitan North residents aged 55+, 70+ and 85+ are likely to be in a position where they can choose the type of care and accommodation

they take up as their needs and circumstances change. However, community consultations confirm older persons generally prefer to seek alternate accommodation arrangements within their own local neighbourhood

2.5.2 Tenure

The ABS Census 2006 collated Tenure data under six classifications:

• Fully owned

• Being purchased

• Being occupied under a lifetime tenure scheme

• Rented

• Being occupied rent-free

The table above indicates higher representation of secure tenure in Metropolitan North as compared to WA as a whole across all the age cohorts examined. This indicates that the majority of Metropolitan North residents aged 55+ (76.2%), 70+ (67.8%) and 85+ (49.8%) are likely to be in a position where they can choose the type of care and accommodation they take up as their needs and circumstances change.

2.5.3 Socio-Economic Indices for Areas (SEIFA)

The Australian Bureau of Statistics (ABS) has produced a number of indices based on 2006 Census data, which measures the level of advantage in any given area. These are known as the 'SEIFA' indexes (Socio-Economic Indexes for Areas).

The SEIFA Index provides an objective measure to determine relative advantage of a particular area against the Australian average, as well as other areas. Factors such as household income, education and profession, household and dwelling size are used to determine an area’s Index value.

Areas are given an index 'score'. The Australia-wide average has been fixed at 1,000 so that, generally speaking, an area with a score above 1,000 can be considered relatively advantaged, and an area with a score below 1,000 can be considered relatively disadvantaged. The further away from 1,000 the scores are, the more or less advantaged a given area is.

2.6 Aboriginal and Torres Strait Islander Persons (ATSI)

The Metropolitan North population includes a small but significant ATSI population, with just over 1,000 persons over the age of 45, according to Census figures. The reported figures for the Study Area are generally consistent those of the Region for persons aged 45+ and 55+, with a higher proportion of 65+ persons.

While the population is recorded at this level, the actual numbers may fluctuate due to the inclination of Aboriginal and Torres Strait Islanders to regularly move within wider family groups, in conjunction with a frequent unwillingness to report their ATSI status to government agencies. This impacts on the statistical accuracy of this information and may mask the propensity of ATSI residents to access health or aged care services across different regions as their health care needs and family commitments change.

The target age group for aged care is lower for ATSI persons due to a reported tendency to experience a range of age-related health issues at an earlier age (45+).

2.7 Veterans and War Widows/Widowers

There is a higher proportion of Veterans and War Widows/Widowers in the Metropolitan North Region than across WA as a whole (22.04% of the 70+ population) as compared to the state average of 19.84%, as reported by the Department of Veterans’ Affairs.

Likewise, each of the target LGAs is reported to have higher proportions of Veterans and War Widows/Widowers than the State: Cambridge (29.59%), Joondalup (21.14%) and Stirling (20.59%).

2.8 CALD Communities

2.8.1 Non English Speaking Countries of Birth

The countries of birth of residents of the Metropolitan North Region represent a wide variety of nations. Of particular interest are countries of birth which are predominantly non-English speaking (NESB).

The table below indicates the size of the CALD communities aged 70+ in Metropolitan North. The largest group, clearly, are those who were born in Italy (2,152 across the region) and it is critical that aged care services respond to their. There are, however small groups such as the Guyanese (3 persons living in Stirling and 3 in Nedlands).

The diversity of NESB require that aged care services develop comprehensive ‘multi-cultural’ responses to meet the needs of persons from any NESB who may be eligible and choose to access their services.

4 Consultation findings

4.1 Methodology

Telephone surveys were conducted in early 2008 with key agencies in the Region including:

• Local Councils: Cambridge, Stirling, Subiaco and Wanneroo

• ACATs: Osborne Park and Sir Charles Gairdner Hospital

• Referral/Advisory Services: Independent Living Centre Commonwealth Carelink Centre and Carer Resource Centre, The Aged Persons’ Support Service (TAPSS)

4.2 Identified regional needs

The following community needs have been identified by Verso Consulting through community consultations conducted annually since 2005. This body of work has enabled us to track the following care issues and service gaps in relation to community aged care.
