Extending care and compassion to thecommunity has been a goal of SuburbanHospital for nearly 75 years. On a dailybasis, Suburban Hospital employeesdedicate their time and expertise toaddress the community’s most pressinghealth issues through various communityimprovement initiatives.
Below are several examples of hospitalcolleagues who have made an impact inour community beyond the hospital walls.
Dr. Dominique Foulkes, medical directorand chair of the Suburban HospitalPediatrics Department, volunteers hertime to inspire our community’s youngestresidents. Dr. Foulkes authors a blog onMontgomery County’s Girls On The Run(GOTR) website. Topics include providingtips on outdoor safety, why you get a feverwhen sick and how to stay hydrated whileexercising. When leading hospital tours forlocal Girl Scout troops and studentgroups, Dr. Foulkes demonstrates how totake vital signs, explains what happenswhen a patient is admitted to the hospital,and explains which items should beincluded in a First Aid kit.
Ahmed Omar, network analyst for SuburbanHospital’s Management Information Systemsdepartment, has been an energetic andcommitted volunteer for MontgomeryCounty’s GOTR bi-annual 5K Fun Runsince 2013. Through health-focusedinteractive games and activities such as the“Jumpstart your Heart,” “Create a HealthyPlate” and “Know your Blood PressureNumbers,” Ahmed and other SuburbanHospital employees encourage girls andtheir families to maintain healthy lives bypromoting physical activity, good nutritionand overall well-being.
The Diabetes Fine Tuning program is aseries of small group sessions for recentlydischarged patients and their care partnersto discuss crucial components of diabetescare. Pharmacist Lee Ann Alexandercontributes her time and expertise byanswering pharmaceutical-related questionsfrom patients and assists with theirdiabetes management goals to improvetheir health and quality of life. Her role as apharmacist has been vital to ensuring amultidisciplinary approach to diabetesmanagement.
Another diabetes focused initiative is theDiabetes Nurse Champion program, whichaims to improve the lives of people withdiabetes who are admitted to SuburbanHospital. Champions Periwinkle “Wink”Mackay, a nurse educator, and Jun Bie, anurse practitioner, have led the DiabetesChampion program since January 2015.Wink and Jun manage all aspects of theprogram, from recruiting participants todesigning the curriculum. Their leadershipand vision have enabled an initiative to beput in place that was previouslynonexistent. Focused on the goal ofproviding better care for our patients withdiabetes, Wink and Jun are champions forpeople living with diabetes.
Once a month, respiratory therapists, AyoSeriki and Shanmugam “Panneer”Panneerselvam facilitate the American LungAssociation’s Better Breathers Club®. Ateach meeting, the therapists provide asupportive environment where individualsliving with Chronic Obstructive PulmonaryDisease (COPD) and other respiratoryconditions are able to learn techniques tohelp them cope with their condition. Ayo
and Paneer are alsostrong advocates forsmoking cessationprograms and makingresources available forpatients who arecommitted to quittingsmoking.
In FY16, SuburbanHospital conducted 2,595community healthimprovement programs,screenings, classes,seminars and activitiesthat served 71,323individuals and dedicated$21,451,227 incommunity benefitcontributions to supportthe needs of MontgomeryCounty residents.

The 2016 Community Health NeedsAssessment (CHNA) processidentified—through primary andsecondary data, community surveysand input from public health expertsand stakeholders—five healthpriorities: cancer, obesity,cardiovascular health, diabetes andbehavioral health. As a result of thesefindings, strategies have beendeveloped to address and evaluateeach health need. They also overlapwith local, state and national prioritiesas well as Suburban’s 2013 CHNAfindings. This relationship affordsSuburban the ability to align itscommunity health improvementefforts in order to decrease healthinequities, lack of access andunhealthy behaviors. During the 2016 CHNA priority-setting process, behavioral health wasidentified as the top health priorityamong stakeholders, and for thosereasons, we are highlighting this health issue.
The statistics in our community areharrowing. Since the last CHNA in2013, there has been a 12 percentincrease in people visiting a hospitalemergency department for abehavioral health condition in

p Support groups to help communitymembers manage emotional stressassociated with chronic and acutehealth conditions; and
p Community health improvementprograms to foster social support,particularly among the seniorpopulation.
“There is such a significant need forthese services in our community and Iam proud of the approach SuburbanHospital takes in providing behavioralhealth care,” notes Suburban Hospitalpresident Jacky Schultz. “Our programsare recognized across the regionbecause we emphasize respect for thepatient and family while also providingthe least restrictive level of care bothwithin the hospital and in thecommunity. In fact, we are in theprocess of expanding our crisis unit tomeet the needs of our patients.Behavioral health services are a priorityat Suburban Hospital.”

Dr. Foulkes authors a blog onMontgomery County’s Girls On The Run(GOTR) website. Topics include providingtips on outdoor safety, why you get a feverwhen sick and how to stay hydrated whileexercising. When leading hospital tours forlocal Girl Scout troops and studentgroups, Dr. Foulkes demonstrates how totake vital signs, explains what happenswhen a patient is admitted to the hospital,and explains which items should beincluded in a First Aid kit.
Ahmed Omar, network analyst for SuburbanHospital’s Management Information Systemsdepartment, has been an energetic andcommitted volunteer for MontgomeryCounty’s GOTR bi-annual 5K Fun Runsince 2013. Through health-focusedinteractive games and activities such as the“Jumpstart your Heart,” “Create a HealthyPlate” and “Know your Blood PressureNumbers,” Ahmed and other SuburbanHospital employees encourage girls andtheir families to maintain healthy lives bypromoting physical activity, good nutritionand overall well-being.
The Diabetes Fine Tuning program is aseries of small group sessions for recentlydischarged patients and their care partnersto discuss crucial components of diabetescare. Pharmacist Lee Ann Alexandercontributes her time and expertise byanswering pharmaceutical-related questionsfrom patients and assists with theirdiabetes management goals to improvetheir health and quality of life. Her role as apharmacist has been vital to ensuring amultidisciplinary approach to diabetesmanagement.
Another diabetes focused initiative is theDiabetes Nurse Champion program, whichaims to improve the lives of people withdiabetes who are admitted to SuburbanHospital. Champions Periwinkle “Wink”Mackay, a nurse educator, and Jun Bie, anurse practitioner, have led the DiabetesChampion program since January 2015.Wink and Jun manage all aspects of theprogram, from recruiting participants todesigning the curriculum. Their leadershipand vision have enabled an initiative to beput in place that was previouslynonexistent. Focused on the goal ofproviding better care for our patients withdiabetes, Wink and Jun are champions forpeople living with diabetes.
Once a month, respiratory therapists, AyoSeriki and Shanmugam “Panneer”Panneerselvam facilitate the American LungAssociation’s Better Breathers Club®. Ateach meeting, the therapists provide asupportive environment where individualsliving with Chronic Obstructive PulmonaryDisease (COPD) and other respiratoryconditions are able to learn techniques tohelp them cope with their condition. Ayo
and Paneer are alsostrong advocates forsmoking cessationprograms and makingresources available forpatients who arecommitted to quittingsmoking.
In FY16, SuburbanHospital conducted 2,595community healthimprovement programs,screenings, classes,seminars and activitiesthat served 71,323individuals and dedicated$21,451,227 incommunity benefitcontributions to supportthe needs of MontgomeryCounty residents.

The 2016 Community Health NeedsAssessment (CHNA) processidentified—through primary andsecondary data, community surveysand input from public health expertsand stakeholders—five healthpriorities: cancer, obesity,cardiovascular health, diabetes andbehavioral health. As a result of thesefindings, strategies have beendeveloped to address and evaluateeach health need. They also overlapwith local, state and national prioritiesas well as Suburban’s 2013 CHNAfindings. This relationship affordsSuburban the ability to align itscommunity health improvementefforts in order to decrease healthinequities, lack of access andunhealthy behaviors. During the 2016 CHNA priority-setting process, behavioral health wasidentified as the top health priorityamong stakeholders, and for thosereasons, we are highlighting this health issue.
The statistics in our community areharrowing. Since the last CHNA in2013, there has been a 12 percentincrease in people visiting a hospitalemergency department for abehavioral health condition in
help for their loved ones.We must doeverything we can to protect ourcommunity and provide treatment to allthose in need.”

It also outlines the outcomes weaim to achieve and how we apply theexpertise of our professional careproviders to address behavioral healthconcerns.
Some of our initiatives include:
p Comprehensive services forindividuals with emotional problems,mental illness and addictive diseases,as well as services designed to fostermental health;
p Support groups to help communitymembers manage emotional stressassociated with chronic and acutehealth conditions; and
p Community health improvementprograms to foster social support,particularly among the seniorpopulation.
“There is such a significant need forthese services in our community and Iam proud of the approach SuburbanHospital takes in providing behavioralhealth care,” notes Suburban Hospitalpresident Jacky Schultz. “Our programsare recognized across the regionbecause we emphasize respect for thepatient and family while also providingthe least restrictive level of care bothwithin the hospital and in thecommunity. In fact, we are in theprocess of expanding our crisis unit tomeet the needs of our patients.Behavioral health services are a priorityat Suburban Hospital.”
