The Charlottesville Free Clinic’s mission includes providing a high-quality volunteer community health support system for low-income uninsured adults and practical experience for current and future health care professionals. There is no better example of this principle in practice than the CFC’s approach to diabetes care. Diabetes affects 25.8 million people, or approximately 8.3% of the population in the United States, according to the Centers for Disease Control and Prevention. About 18.8 million people are diagnosed with the disease while 7 million are undiagnosed. It is the seventh leading cause of death in the United States.

Closer to home, approximately 8.7% of Virginia’s population, or 531,366 people, have been diagnosed with diabetes, according to statistics from the Virginia Department of Health. We see these statistics reflected in our patient population here at the CFC. In fiscal year 2012, diabetes was the third most frequent diagnosis for our patients, behind high cholesterol and hypertension. About 11% of our patient population has a diabetic diagnosis, and approximately 15% of the prescriptions filled in the CFC pharmacy are for the direct treatment of diabetes. This translates to 400 thirty-day prescriptions each month. CFC Pharmacy Coordinator Chris Hicks says that when one considers the host of health problems diabetes causes or exacerbates it is logical to assume that the number is much higher, but it is difficult to quantify. Our medical providers do help diabetic patients treat the disease with medication, but the CFC also offers other programs and support to promote healthy lifestyle changes for diabetics.

Insulin Titration ProgramOne of the programs we are excited about is our innovative Telemedicine Insulin Titration program for patients taking long-acting insulin for their Type 2 diabetes. The CFC’s program is one of the first of its kind in the country, and other clinics have begun to adopt this model for diabetes management. The CFC’s Insulin Titration team consists of University of Virginia Medical School students from all four years of the program working under the guidance of Molly McElwee, RN, CDE, a volunteer diabetes educator affiliated with the UVA School of Medicine, and Dr. Helmy Siragy, one of our volunteer endocrinologists. The students are divided into teams, and each team is responsible for about 4 patients with 12 patients total in the program. The students call each patient every week to ask about their glucose levels, their diet and their insulin regimen to provide closer follow-up, guidance and encouragement for their diabetes management. Calls can range from five minutes to more than 45 minutes. By using a matrix and consulting with our diabetes educators and endocrinologists, the students make small adjustments to patients’ insulin dosages with the ultimate goal of lowering their HA1C levels, which provide a picture of longer-term blood sugar levels. The students also work with patients to promote lifestyle changes and to provide support as well as a measure of accountability with their treatments. The program is constantly looking to recruit more CFC patients who would benefit from these services.

Dear Friend of the Free Clinic,Springtime at the Charlottesville Free Clinic means many things. It’s when we identify our strategic priorities for the following year and set the budget to support our plans. It’s when we honor our volunteers in a special way during and around National Volunteer Week and plan for the challenge of staffing clinics during the summer, and it’s when we are in high gear with events like the annual golf tournament sponsored by NAIFA (June 10) and our annual benefit concert. Look for a concert announcement coming soon to Facebook and like us at facebook.com/cvillefreeclinic to be one of the first to know. This year we are adding to the momentum that spring already brings. First, we will be hosting another in our series of “community conversations,” this time featuring local physician Pamela Ross and retired CIGNA executive Wendell Potter, both of whom appeared in the award-winning documentary Escape Fire. Please see the enclosed invitation and share it with others who may be interested in attending. Also, thanks to a grant from the CACF Future Fund, we will be hosting a Mission of Mercy Dental Day on April 20 at Covenant Church. Through the generosity of volunteers from around the state,

we hope to treat over 400 patients. This is one way that we are achieving one of our strategic priority areas (described in our last House Call available on our website) to “Meet the Dental Demand.” Another of our priority areas is to “Conduct Patient Outreach” to increase awareness of the CFC and ensure that prospective patients and our partner agencies are clear about our services and eligibility criteria. In this newsletter, you will read about our new partnership with CVHAM to reach out to local uninsured musicians. Additionally, we will be participating in a series of “health care road shows” to visit local neighborhoods to help residents understand what health care reform might mean to them and to provide on-site health screenings. Other partners in this project include the UVA School of Nursing, the UVA Batten School of Leadership & Public Policy, the Alzheimer’s Association and JABA. While things remain busy and eventful here at the CFC, there is never a day that passes that we aren’t reminded of the generosity of the many volunteers, donors and community partners that keep the engines running at the CFC. CFC Executive Director Erika Viccellio honored by United Way On December 5, 2012, the United Way-Thomas Jefferson Area presented CFC Executive Director Erika Viccellio with its 2012 Excellence in Nonprofit Leadership Award for outstanding community leadership exemplified by her work with the Charlottesville Free Clinic and the Community Mental Health & Wellness Coalition. Upon receiving the award and the accompanying $5,000 grant, Erika said, “This recognition belongs to many more people than me: the board, staff, volunteers and donors who all make wonderful things happen at the CFC. There are so many people that are behind the CFC’s successes -- I just feel lucky to be a part of the team.” This is not the first recognition of the CFC’s excellence by the United Way. The CFC board received the inaugural Excellence in Nonprofit Board Governance Award in 2007.

CFC will host Mission of Mercy Dental Day April 20th

Did you know that the Charlottesville Free Clinic has increased its available dental appointments by 800% over the past six years and yet still has more than 400 people on its acute and restorative dental care waiting lists? Have you ever lived with dental pain? Hundreds of adults in our community live with debilitating dental pain affecting their sleep, diet and work. Nationwide, one-fourth of all emergency room visits are directly related to oral disease. Poor oral health not only generates pain but also can lead to a septic infection, heart disease, diabetes and even death. The Charlottesville Free Clinic is the only provider of free dental care for adults in our community. Despite day and evening clinics with paid and volunteer

dental professionals and monthly dental clinics with VCU dental students, the wait for an appointment can be as long as 18 months. Thanks to a $23,000 grant from the CACF Future Fund, support from the Virginia Dental Association Foundation and help from more than 250 volunteers, this spring the CFC will host a Mission of Mercy Dental Day at Covenant Church on Rio Road. During the one-day clinic, we hope to provide free extractions, fillings and cleanings for over 400 uninsured or underserved adults. The Virginia Dental Association Foundation (VDAF) launched the Mission of Mercy (MOM) Project in 2000 to help meet the overwhelming dental needs of uninsured and underserved residents of Virginia. Since then it has supported several MOM dental days throughout the state each year. MOM Projects are usually 1 to 2 day dental events drawing on volunteer support from the community and across the state. The VDAF provides event-planning guidance, coordinates student and faculty volunteers from the VCU School of Dentistry and supplies dental chairs and equipment. For a typical MOM Dental Day, more than 250 dental professionals—from the hosting community and throughout Virginia—as well as 200 medical professionals, college students, pharmacy professionals and community members volunteer their time and expertise.

The CFC is also one of the first to introduce a Spanish-speaking Telemedicine Insulin Titration Program. This past fall, the CFC added a team from UVA’s Latino Medical Student Association to reach out to Spanish-speaking patients who have to overcome cultural and language barriers in addition to the already difficult task of managing diabetes. This unique team is one of the first groups in the country to use telemedicine on a weekly basis with this subset of patients and to tackle the barriers they face. Mary Whittemore, one of the students involved in the program, completed an evaluation of the Insulin Titration program in 2012 and found that patient satisfaction is high, the medical students enjoy the experience and most importantly, many patients have experienced a drop in their HA1C levels. About 80 students expressed interest in participating in the program, and she says it’s a great experience for medical students who are interested in primary care because they can establish a long-term relationship with patients compared to their rotations in school, which happen over a month, and they can apply the medical clinical judgment learned in class to real-life experiences. Another benefit to the program is the emotional support that patients feel by connecting with a volunteer on a weekly basis, as many only meet with their physician every three to six months. “Talking every week helps people take ownership of the disease,” says Pranay Sinha, a third-year medical student. He also says the program is a validation of the need for human interaction in medicine, as the volunteers can notice issues that technology may not detect, including any need for mental health support.

Diabetes Support GroupDiabetic patients make up a significant portion of the patient population that CFC Nurse Practitioner Barrie Carveth treats during our daytime clinics. She provides ongoing care for much of our chronic patient population – those men and women in our community with medical issues that require more frequent and consistent treatment. As part of her care, Barrie facilitates a Diabetes Support Group, which meets on the first and third Wednesdays each month for two hours. The group is open to all CFC patients with Type 2 diabetes that is not well controlled and helps address lifestyle changes that will help patients better manage their diabetes. Much like the CFC’s smoking cessation program, Quit Smoking Charlottesville, each patient sets his or her own goals for change and can attend as many or as few sessions as they please. Diabetes specialists and educators from the UVA Health System and Martha Jefferson Hospital help lead the sessions, and each month focuses on a different topic, including stress management, physical activity and nutrition. Barrie says the greatest advantage of these group sessions is the support the participants get from each other, in addition to the knowledge they gain about controlling their diabetes through lifestyle changes.

Diabetes Education“Diabetes is a concept that takes many visits for patients to grasp and accept” says Fannie Rankin-Vinci, one of the CFC’s volunteer diabetes educators. Fannie meets with CFC patients to help them manage their diabetes, monitor their medication and teach them ways to address their disease through diet and exercise. She also performs chart reviews to make sure our diabetic patients are up to date on their annual physical exams, follow up appointments and blood work. She schedules appointments with volunteer endocrinologists and arranges appointments for specialized medical services like eye exams and podiatry with providers in our Specialty Medical Care Network if needed. In addition to her work with the Insulin Titration program, Molly McElwee RN, CDE sees patients on her own as a diabetic educator. She is diabetic herself and says she wants to deliver the same quality of care that she receives from her own educator through the CFC’s program. “We cover the gamut in our diabetes care, covering labs, diet, lifestyle changes,” she says. “Everything is covered, including emotional support. Patients are scared, and we provide a support system for them so they know they can face and handle their disease.”

The start of a new year means changes to the CFC’s board. Congratulations to Todd Cabell, who was named board chair in November 2012. Todd has been involved with CFC for nine years as a volunteer and board member -- most recently as Development Committee chair and board secretary. He is currently the director of eCommerce at Crutchfield Corporation. Todd follows Carolyn Engelhard at the helm of our board. Carolyn has been involved with the CFC for over nine years and is currently director of the Health Policy Program at the UVA School of Medicine. We thank Carolyn and our outgoing treasurer Ray Murphy for their expertise and leadership. We also congratulate our new secretary Janet Silvester, director of pharmacy and director of emergency services at Martha Jefferson Hospital, and our new treasurer Steve Burns, senior partner at Quad-C Management. We also wish to thank Dr. Joseph Kayser, Elliot Kuida and Dr. David Swett for their years of service on our board and for the many ways that they continue to support the CFC. They retired from the board at the end of 2012. We were thrilled to welcome five new members to the CFC’s board in January 2013. CFC BOARD PROVIDES Leadership & Governance

Elizabeth Alcorn, DDS Elizabeth has volunteered at the CFC for 12 years. Prior to that time, she was a volunteer at the free clinic in Winchester, Va. She currently runs her own practice, Charlottesville Dental Care. She received her DDS from The Medical College of Virginia and her undergraduate degree from the University of Virginia. Elizabeth is involved in the Highland Support Project in Guatemala and has experience in national and international dental volunteerism. She chairs the CFC Dental Advisory Committee.

Corey Feist, JDCorey has recently become involved with the CFC. He is currently the chief of staff and general counsel for the University of Virginia Physicians Group. Corey earned his Bachelor of Arts from Hamilton College and his Juris Doctor in Law from Dickinson Law School of Penn State University.

Will Knight, MDWill has volunteered at the CFC for more than 4 years. He has been a hospitalist physician at Martha Jefferson Hospital for the past two years. Will received his MD from the University of Florida and his undergraduate degree from Princeton University. He currently holds the position of Medical Director at the CFC.

Chris Powell Chris has been involved with the CFC for the past year serving on the Development Committee. He is currently the vice president of BB&T Insurance where he has worked for

the past 21 years in a variety of capacities. Chris earned his undergraduate degree from Hampden Sydney College. He has served on the board at the Virginia Institute of Autism for the past 8 years, including a term as board president.

Bebe Heiner, LPCBebe is the board chair and founder of The Women’s Initiative, whose mission is to provide effective counseling services, social support and education for women in our community. She is a licensed professional counselor with 11 years of experience providing psychotherapy to individuals, couples and families. Her community service includes board and committee membership at Mary Baldwin College, the Charlottesville Area Community Foundation, the Virginia Institute of Autism, the Virginia Center for the Creative Arts at Sweetbriar College and Planned Parenthood.

dental care clinics. One student has gone on to become X-ray certified and another has obtained a position working at another local dental clinic. We are also happy to have a new pair of first-year UVA dental residents volunteering at the dental clinic. Drs. Graham Hearn and Ross Oates joined second-year residents Drs. Charlie Beavers and Oscar Vega who have been coming in one to two evenings each month to help with extractions at our acute clinics.Thanks also to the dental residents at the VA Hospital in Richmond who have also volunteered in the dental clinic about once each month since the fall.

Meet Mr. G. He’s 50 years old and has been seen at the CFC for about a year. A man of many talents, he currently works in manufacturing and is a talented chef. Prior to becoming a CFC patient, he was uninsured and paid out-of-pocket costs to see a private doctor, using up much of his savings to pay for his health care. Eventually, the cost of seeing his doctor became so great that he could not afford care or his prescription medication. A friend referred him to the CFC as he suffered from anxiety and hypertension, and his health was deteriorating. After just one visit to the CFC, he saw a hypertension specialist who put him on an appropriate blood pressure medication regimen, and his health began to improve. His CFC doctor told him he was lucky he hadn’t suffered a stroke. Mr. G. also worked with the CFC’s Integrated Care team to learn how to better manage his stress and anxiety. He successfully negotiated a pay raise at work after role-playing in his counseling sessions with our Integrated Care Coordinator. Mr. G. says he’s getting excellent medical care here at the CFC, and he feels relaxed coming to the clinic. He knows that the doctors, nurses, mental health providers and community members at the CFC volunteer because they want to help the community, and it shows through the care patients receive. To show his gratitude, Mr. G. cooked up a delicious Italian-themed dinner for our volunteers in January, complete with pasta, homemade sauce, garlic bread, salad, mushrooms in white wine sauce and roasted peppers. Our volunteers were very grateful for his efforts, and Mr. G. is grateful for the CFC. “I’m lucky,” he said. The CFC “saved my life.”

It’s been a busy few months in the CFC Dental Clinic! During the fall of 2012 and the beginning of 2013, our volunteer and staff dentists treated 1,034 patients in 1,883 visits. Drs. Richard Heath and Diana Marchibroda went above and beyond to treat dental patients at our on-site clinic while our full-time staff dentist was on leave. Dr. Daniel Vacendak, our part-time per-diem dentist, also worked additional evening hours to see patients on our restorative care list and has expanded his daytime hours to provide continuity of care for Dr. Conrad Jenkins’ patients now that Dr. Jenkins has retired. We are so grateful for Dr. Jenkins’ service to the CFC’s patients over the last four years, and we celebrate his cumulative 33 years of service to dental patients in our community. Updates FROM OUR DENTAL CLINIC

Proffitt as our dental office assistant last year. Stephanie volunteered for two and a half years doing administrative front desk work in the dental clinic before she joined the staff, so she is a familiar face to many here at the CFC. Becky Jones, a registered dental hygienist, started volunteering with us one morning each week last fall to provide limited on-site dental hygiene care and cleanings for our patients. She has also served as a resource for oral hygiene education. This past fall, Judy Eppard, our dental office coordinator, taught a dental assisting class through CATEC which was held here at the CFC. Six students received certificates through the program and learned basic dental assisting terminology, protocol, procedures and techniques. Every fall, employees of state agencies, public universities and community colleges have an opportunity to donate to more than 1,300 nonprofit organizations throughout the state through the Commonwealth of Virginia Campaign (CVC). The University of Virginia runs the largest CVC in the commonwealth. UVA employees have generously supported the CFC through the CVC every year since 1997. UVA employees pledged $80,700 to the CFC through the 2011 CVC. UVA CVC Chair Jim Fitzgerald has shared two exciting pieces of news with the CFC. One, the totals are in and through the 2012 UVA CVC, employees generously donated $1,072,000, crossing the $1,000,000 mark for the first time. Two, through the 2012 campaign, UVA employees have designated $84,041 to the Charlottesville Free Clinic. Hats off to the many UVA employees who led efforts around Grounds for the 2012 CVC - job well done! Thank you to the more than 400 employees who generously supported the CFC through this year’s campaign. We’ll be acknowledging individual donors when we receive the report from the 2012 CVC campaign this spring. Thank you for your continued support; we are truly grateful.

The Charlottesville Area Quilters Guild (CAQG) provides an opportunity for local quilters to share their love of quilts with each other and the community as well as work together to support local nonprofit organizations. There are 6 local chapters and about 120 members in our area. Every other year, CAQG hosts a quilt show and quilt raffle to showcase members’ talents and raise funds for the Charlottesville Free Clinic and other community service projects. 2013 CAQG Quilt Raffle: Summer’s SongEvery other year, the CAQG raffles off a quilt made by members to raise funds for the Charlottesville Free Clinic. The 2013 raffle quilt, “Summer’s Song,” features vertical panels of hand-appliquéd and embroidered flowers, birds, small animals and insects separated by panels of blue batik fabric braids. Del Horan and Mona Houle coordinated the making of the quilt with contributions from many CAQG members. The quilt measures 95 inches wide by 100 inches long. You can look at a color photo of this beautiful quilt on the CFC’s website at cvillefreeclinic.org. The “Summer’s Song” quilt will be displayed at the CAQG Quilt Show XIII in April and will be raffled at the guild’s fall meeting on November 2, 2013. You do not need to be present to win. LOCAL QUILTERS continue support for CFC

PATIENT Story Charlottesville has a thriving music community. On any day of the week, you will find talented musicians performing at restaurants and music venues throughout the community. It’s also a fact that many of these musicians cannot make a living as performers and are working additional jobs to make ends meet. One thing that often falls to the bottom of the priority list is health insurance. According to a 2002 report from the Future of Music Coalition, approximately 44% of musicians in the U.S. do not have health insurance. There are two main reasons for this. Because musicians are most often freelancers, they do not have access to employer-sponsored health insurance and are forced to purchase individual policies that are substantially more expensive. In addition, because their bookings may vary widely from month to month, they can have a hard time keeping up with premium payments. Inspired by the highly successful Health Alliance for Austin Musicians (HAAM) in Austin, TX, a dedicated group of community members has worked together to ensure that Charlottesville musicians have access to affordable health care services even if they are uninsured. The Central Virginia Health Alliance for Musicians (CVHAM) was formed in 2012 and has just received its 501(c)(3) tax-exempt status. CVHAM’s mission is to reach out to low-income uninsured working musicians to provide information and guidance on accessing free and reduced-cost health care services and to raise funds to provide grants for medical or dental testing, treatment and procedures not offered for free. The Charlottesville Free Clinic will be an appropriate referral for many musicians and the CFC is pleased to partner with CVHAM to help get the word out to the music community. CFC Executive Director Erika Viccellio states that “the CFC already provides free health care for a number of local musicians, but we know that there are many more who don’t know about us. Thank You CFC Supporters!With our recent dental clinic staff expansion and integrated care initiative, the CFC’s operating expenses for fiscal year 2013 (July 1, 2012-June 30, 2013) are projected to be $1,498,700. The majority of the CFC’s annual funding comes from donations from individuals, foundations, churches, businesses and civic groups through our fall annual fund letter mailing, newsletters, annual report mailing and our website.
