Scholarships 2016 SCHOLARSHIP APPLICATION

As a result of our generous benefactors in the community, we have a small number of Eaglebot and Technobot need-based scholarships available for students. We strongly encourage those interested in the program who may have financial limitations to complete the following process for financial assistance.

The Waiver Request Form and Instructions are located to your right. Please open a copy and review the document to determine if your child/children would qualify for scholarship relief.

In order to qualify for a partial fee waiver, you must provide items 1 and 2 below:

1. Complete the Waiver Request Form which is available as a link on this page (to your right). Please list all members in the household using the space provided on the waiver request form, including parents/guardians and all other children and/or adults.

2. Demonstrate financial need and submit proof of Current Year Income Tax Return – i.e. a copy of page 1 and 2 of your 2015 federal income tax return is required.

3. Submit proof of good academic standing (3.0 or higher GPA requirement) – i.e. a copy of your child’s last report card is appreciated. Note, if you do not have the last report card, Polk State College will verify the child’s academic standing through the school district office.

Complete the request form and return it along with the required supporting documentation by email to Christy Canady ([email protected]) or fax it to her attention to 863.669.4929.

Important Note: the $35 non-refundable deposit is required of all registrants and it is highly recommended that you go ahead and complete the online registration process to ensure your child has a reserved seat.

Guidelines for Awarding TALON Robotics Partial Fee Waivers

Selection: The Review Committee will process the initial screening of applicants based on the stated qualifying criteria. Final approval and selection of each recipient will be made by the TALON Scholarship Committee. Recipients must use the partial fee waiver in the current session, or it will automatically be voided.

TALON Robotics

Partial Fee Waiver Guidelines

Through the generous contributions of Polk State TALON donors, some scholarship relief will be available for those with significant need.

This year, all participants will be required to pay a registration fee of $35.00.

Partial fee waivers will be awarded on a sliding scale based on household income and number of persons in the family. To assist us in determining eligibility, we will use the established Federal Income Eligibility Guidelines.

Anyone wishing to apply for a partial fee waiver will be required to provide proof of household income at the time of application.

In order to qualify for a partial fee waiver, you must meet all of the requirements below*:
