    • Surface/highlight key patient information to inform the care team

    • Assist the provider and patient with planning the visit/care interaction

    • Enhance patient memory of the content of interactions with Care Team

    • Support patient engagement and self management

    • Problem Statement:

    • Patients need a more User-Friendly, easy to understand format!

  • How often is the AVS used?

    • In 82 providers in the Alliance network

    • 91% of providers printed an AVS for more than half of their patients

    • 41% of providers had an AVS printed for 90% or more of their patients.

    • Mean frequency of printing of the AVS was 79.9%

  • The After Visit Summary

  • Quotes from Health Center Patients

    • “The most important things is for me to see my numbers”

    • “I keep mine in my purse so if something happens to me, everyone knows how to care for me”

    Dr. Nita Mohanty

  • Background

    • Journey Mapping exercise at the 2017 Alliance User Conference

“We Need Clinical Content that will Improve Workflows!”

  • Pains and Gains Framework

  • Write or Draw Us Your Story!

    Your Table

    • Indicate your Name and your Role at the

    Health Center

    • Share Pains with the Clinical Content

    • Share Gains from the Clinical Content.

    • What Job do you need the Clinical Content

    to do for you?

  • An example:

    Name: Nita MohantyRole: Pediatrician

The guidelines for Peds BP management changed in August 2017 and the EMR has Percentiles reflecting older guidelines.

I need the Peds BP content to help me use the correct percentiles and create a way for me to easily average BP measurements to practice guideline based care

The current content has new features I like such automated percentiles and historical display of previous BPs. An added way to average BPs would be a huge benefit.
