    05 NEW MAIN INSTALLATION BUILDER DIG AND BACKFILL PLASTIC (OH, KY, PA, VA)

    This item is to be used in new construction and replacement situations where a third party will dig the ditch prior to construction ofthe pipeline, and then backfill the same ditch after construction of the pipeline. This item includes site restoration.

    Lining up, fusing of the plastic pipe, installation of the warning tape and locate wire, installation of any specified plastic valves upto and including 4, hauling material, including all supervision, labor, equipment and miscellaneous supplies not otherwiseprovided according to the Owners Specifications and Procedures. This item also includes an air test and interior cleaning(pigging) and purging of the pipeline.

    Installations will be in accordance with Owners Specifications and Procedures. See General Specifications for additionalrequirements.

    07 & 09 NEW / REPLACE MAIN INSTALLATION STEEL (OH, KY, PA, VA,)

    This item includes the installation of new and replacement gas distribution mains. This item includes all excavation andbackfilling, lawn restoration (seed, straw & mulch), incidental flagging, sloping, shoring up to 10 feet, supervision, labor,equipment and miscellaneous supplies not otherwise provided. This item also includes:

    These items shall include bending, laying and lowering of the pipeline including minor stream crossings (defined as crossings thatcan be made with normal trenching equipment), the installation of warning tape and wire (if applicable) with the laying of steelpipe is included. Air test and interior cleaning (pigging) and purging of the pipeline, Nitrogen will be supplied by Owner whenneeded.

    Cleaning, priming, coating and wrapping per manufacturers specifications of all bare joints, holidays and damaged coating isincluded. Jeeping to find holidays in the coating is also required. Contractor is to supply Holiday Detector. This item includesthe installation of anodes and test stations when required. Sandblasting is included.

    The unit of measure will be paid according to the installation length, and method specified by Owners Authorized Representative.

    The only extras that will be considered will be welding, special fittings, boring, rock excavation, hard surface removal and

    restoration, special backfill, extra depth, frost cutting and sod installation, where applicable, and those will be paid according toprices as established elsewhere in the Contract.

    Installations will be in accordance with Owners Specifications and Procedures. See General Specifications for additionalrequirements.

    10 BORING WITHOUT CASING GAS MAIN PLASTIC OR STEEL (OH, KY, PA, VA)

    This item includes all boring where casing is not required, including excavation and backfilling of the approaches, sloping, shoringup to 10 feet, supervision, labor, equipment and miscellaneous supplies not otherwise provided. This unit is measured from thebore hole face on the near side to the bore hole face on the far side (useable bore length) and the unit size is the nominal carrierpipe diameter. The unit of measure for this item is lineal feet.

    The insertion of the carrier pipe through the hole is included in this price. Boring can be made by auger, hole hog or otherapproved method.

    This item includes spotting and verification of all utilities as required by law. Pavement openings shall be kept to a minimum bypreferred method such as, but not limited to, vacuum excavation, hand digging, posthole digging, etc.

    Any bores made for the Contractors convenience at his discretion and not as specified by the Owners Authorized Representativeshall be included in Items 01 thru 09. Contractor shall make two (2) attempts. If unsuccessful, Contractor will make one attempt ata new location selected by the Owners Authorized Representative. If unsuccessful, Contractor will be paid for a completed boreand will complete the crossing by the open-cut method or by boring at a new location. Total payment is not to exceed the cost oftwo (2) bores or one bore and an open-cut.

    15 WELD-END INSULATOR (OH, KY, PA, VA)

    This item shall include all operations, excluding welding, necessary for the complete installation of weld-end insulators in thefacility under construction. This item also includes the installation of an anode and test station where required. Insulator will be

    installed in a manner so as to avoid external load damage. Contractor must verify that the insulator is operational, before and afterinstallation.

    The unit of measure is per insulator installed and based on the size of the insulator. Welding associated with the installation ofinsulator shall be paid according to Section 17.

    17 WELD JOINTS (OH, KY, PA, VA)

    This includes performing all tasks necessary to complete the welding of steel services, mains and inline fittings (elbows, tees,etc) including lining up, welding and cutting of pipe by any approved process, including cleaning, filing, grinding, trimming andbeveling where required. Contractor will provide welding rod as specified by Owner. Item includes welding of any end caps, purgepoints or pressure verification fittings.

    The unit of measure is per each size of weld completed and will be paid in addition to lay and bore prices for mains and services.

    20 NEW SERVICE PLASTIC OR STEEL (OH, KY, PA)

    This item includes the installation of service lines (2 and smaller) from new and existing mains. This item includes all excavationand backfilling, lawn restoration where applicable, incidental flagging, shoring up to 10 feet, sloping, testing and purging,supervision, labor, equipment, and miscellaneous supplies not otherwise provided.

    This item is for LUMP SUM unit price to install a complete service line by any means (direct bury, bore, directional bore, insert,trench, etc). When pricing this item, Contractor should consider that the method of installation selected by the Contractor andthe Company will be selected to minimize the total cost to install the service. The lump sum prices provided should include theinstallation of the service tee, excess flow valve, curb valve and curb box, where required, first-cut regulator, anode, warning tape,locate wire, riser, meter set assembly, suitable meter protection (as needed) and other fittings according to the Owners

    Specifications and Procedures.

    Service lines that are 120 in length will be paid the additionalfootage unit in combination with the 61-120 service unit for the distance greater than 120.

    This unit will also include the installation of the riser and meter set assembly, with the exception of PA West. Tie-in of company-owned service to customer-owned service (PA West Only) will be included.

    Unit prices for Service Lines W/O Mains pertain to services installed where re-mobilization occurs and is applicable to scattered

    services only and will not be paid for service replacement projects consisting of bundled services.

This item includes the installationof service lines (2 and smaller) from new and existing mains. This item includes all excavation and backfilling, lawn restorationwhere applicable, incidental flagging, shoring up to 10 feet, sloping, testing and purging, supervision, labor, equipment, andmiscellaneous supplies not otherwise provided.

    This item is for LUMP SUM unit price to install a complete service line by any means (direct bury, bore, directional bore, insert,trench, etc). When pricing this item, Contractor should consider that the method of installation selected by the Contractor andthe Company will be selected to minimize the total cost to install the service. This item alsoincludes an air test and purging of the pipeline.

    21 REPLACE SERVICE WITHOUT RELIGHT - PLASTIC OR STEEL (OH, KY, PA)

    Same as Item 20, except that retirement of the old service is included. If the old service cannot be retired in the same excavationor location, the Contractor will be paid either an isolated main bell hole, if the service is retired at the main (item 30-005) or aservice isolated bell hole (item 30-010), when the service is retired on the service line, in a location other than the main..     This item will include restoring gas service to a customer, according to applicable Columbia Policy and Procedures andrequirements of the National Fuel Gas Code (NFPA-54). This includes necessary house piping tests such as meter dial tests(where permitted), pressure-drop tests, etc., as well as visual inspection of piping, venting, and equipment installations.Appliances will be lit provided the equipment is in proper functioning correctly; this would include checking the appliance forproper combustion, ventilation, venting, and operation according to Columbia procedures and requirements of the National FuelGas Code.

    Pre-testing of house piping should be considered and is recommended in situations where house piping may be considered in poorcondition or when re-establishing service to a slab type homes (house lines are typically under the concrete slab).

    Contractor will be responsible for coordinating the necessary work and provide the customers with adequate communication.Contractor will be provided a picture ID for performing re-lights and meter move-outs. Contractor is responsible for ensuring thatnecessary paper work (Service Orders) are filled out correctly and submitted to the Owners Authorized personnel in a timelymanner. This item will be paid regardless of the appliances being re-lit, provided the contractor went through all thenecessary tasks mentioned above and provided all necessary documentation, as to why the relight was not performed.

    The only extras that will be paid will be pipe footage as described above, rock excavation, and hard surface removal / restoration,and those will be paid according to prices as established elsewhere in the Contract.

Tie-in of the service line from main to curb valve or property line to the CSL isincluded in all of these items. These items will also include the installation of the riser and meter set assembly. All service lineand Customer Service Line items include replacing / upgrading of an existing outside meter setting. This includes tying in to anexisting customer house line.

    Unit prices for Service Lines W/O Mains pertain to services installed where re-mobilization occurs and is applicable to scatteredservices only and will not be paid for service replacement projects consisting of bundled services.

    This item will include restoring gas service to a customer, according to applicable Columbia Policy and Procedures andrequirements of the National Fuel Gas Code (NFPA-54). This includes necessary house piping tests such as meter dial tests(where permitted), pressure-drop tests, etc., as well as visual inspection of piping, venting, and equipment installations.Appliances will be lit provided the equipment is in proper functioning correctly; this would include checking the appliance forproper combustion, ventilation, venting, and operation according to Columbia procedures and requirements of the National FuelGas Code.

    Pre-testing of house piping should be considered and is recommended in situations where house piping may be considered in poorcondition or when re-establishing service to a slab type homes (house lines are typically under the concrete slab).

    Contractor will be responsible for coordinating the necessary work and provide the customers with adequate communication.Contractor will be provided a picture ID for performing re-lights and meter move-outs. Contractor is responsible for ensuring thatnecessary paper work (Service Orders) are filled out correctly and submitted to the Owners Authorized personnel in a timely

    manner. This item will be paid regardless of the appliances being re-lit, provided the contractor went through all thenecessary tasks mentioned above and provided all necessary documentation, as to why the relight was not performed.

    This item includes the installation of Owners company service lines from new and existing mains.     These items shall include the necessary labor, material and equipment for furnishing, hauling, installation, maintenance,backfilling, proper placement and removal of the following various environmental control items as directed by OwnersAuthorized Representative.

    Silt Fencing (Paid by lineal foot)A temporary sediment barrier consisting of a filter fabric stretched across and attached to supporting posts and entrenched in theearth. The filter fabric may be natural (burlap) or synthetic (geo-textile). The fence is placed across or at the toe of a slope or in aminor drainage way to intercept and detain sediment and decrease flow velocities from drainage areas of limited size; applicablewhere sheet and rill erosion or small concentrated flows may be a problem.

    Hay-Straw Bales (Paid by each bale)A temporary sediment barrier composed of straw bales staked across or at the toe of a slope or around a drainage structure inlet tointercept and detain sediment and decrease flow velocities.

    Jute Netting (Paid by square yard)Large rolls of natural materials that are applied to high erosion areas such as stream banks. These fiber blankets are laid over theground and must be anchored using metal or wooden pins. They allow vegetation to grow while holding soil in place.     This item is to be used only upon authorization of Owners representative when asphalt street repairs are required and hot mixplants are not open because of weather conditions. This item is for installation of a 6" concrete base and a 2" concrete cap (cap tobe replaced with hot mix when plants reopen). This item includes all materials, labor, supervision, equipment, and disposal of spoilfrom the pavement installation. The units paid will be on a per square foot basis. Any saw cutting required will be included in thisitem.

    47-210 Remove 2 Concrete Temporary cap, install hot mixcap

    This item is to be used for the removal of temporary concrete and the installation of permanent hot mix in repairs made during coldweather. This item includes all materials, labor, supervision, equipment, and disposal of spoil from the pavement installation. Theunits paid will be on a per square foot basis.

    48-0XX Concrete:

    This item will include installing concrete to match the original conditions as specified by Owners Representative and inaccordance with the standards for road and street construction of the applicable governmental authority having jurisdiction.     49-305 thru 49-320 Asphalt Mill and Overlay

    Contractor to furnish equipment, material, labor and supervision to mill and replace asphalt pavement surfaces of depths of 1.5 or3.0 inchs. This item includes seal tacking exposed edges and sealing joints between new and old materials. Work is to beperformed as specified by authorized Owners Representative and in accordance with the applicable governmental authorityhaving jurisdiction. A lump sum charge for moving all equipment in and out of the project site in order to perform this work willbe paid under item 49-300.

    This includes performing all tasks necessary to complete the welding of a pressure control fitting to including lining up, weldingand cutting of pipe by any approved process, including cleaning, filing, grinding, trimming and beveling where required. Theprice includes all welding associated with the installation of the fitting including the fittings itself, welding of equalization fittings,and any other welding required to install fitting according to Company Operating Procedures.

    This will be paid in addition to Items 54, 55, 56 & 57 when Contractor is performing the welding.

    The unit of measure is each by type and size of pressure control fitting installed.

    74 REPLACE CUSTOMER SERVICE PLASTIC (PA West)

    This item includes the installation of customer-owned service lines (2 and smaller) from new and existing mains. The retirementof the old service is included in the price when located in the same excavation. This item includes all excavation and backfilling,saw cutting, pavement removal, backfill material, bell holes when inserting, lawn restoration where applicable, incidental flagging,shoring up to 10 feet, sloping, testing and purging, supervision, labor, equipment, material and miscellaneous supplies nototherwise provided. This item shall include the installation of the service riser and installation of the meter setting and changing ameter with the same connection size when required by Owner. This item includes adjusting an existing setting for alignment and/orpainting an existing meter setting as required by Owner. This item also includes the installation of the warning tape; locate wire,and other fittings according to the Owners Specifications and Procedures.

    Tie-in of company-owned service to customer-owned service will be included.

    The unit of measure will be paid as a lump sum for up to 50 feet of customer service line installed, length measured from the curbbox (property line if no curb box is present) to the meter set (or service entrance to building). Customer service lines in excess of50 feet will be paid according to prices as established elsewhere in the Contract.

    75 REPLACING / UPGRADING METER SETTING (ISOLATED) (OH, KY, PA, VA)

    This item includes replacing / upgrading of an existing outside meter setting. This includes tying in to an existing customer serviceand house line. This item shall include changing a meter with the same connection size when required by Owner. This item shallinclude installing a meter bar, a domestic regulator or installing an orifice into an existing domestic regulator as required by

    Owner. This item includes adjusting an existing setting for alignment and/or painting an existing meter setting as required byOwner. When applicable, this item will not be paid in conjunction with Items 74-060), 20, or 21. This unit is for residential sizedmeter settings.

    75 SETTING OF NEW RESIDENTIAL METER (OH, KY, PA, VA)This item shall include the setting of the new residential meter with at time of new service line installation. Contractor will purgegas through meter to ensure that meter dials are functioning. Contractor will be expected to touch up the setting with fresh paintfrom any wrench marks caused during installation of the meter. Contractor will not activate service to the customer.Responsibility for live gas ends at the meter shut-off valve. If customer fuel line is not installed capping the meter outlet pipingand pinning the meter valve are included.

    76 MOVING METER WITHOUT RELIGHT (INSIDE METER MOVED TO OUTSIDE) (OH, KY, PA, VA)

    This item will include the moving of the meter and/or regulator and will include drilling exterior building walls where required,installing up to 6 feet of fuel line, tie-in from outlet of meter set to existing customer fuel line (pipe size not to exceed 1-1/4),removal of old service pipe and necessary patching of building wall.

    Additional pay units exist for excess fuel line; flex tubing, and multiple meter sets (item 76-025).

    Contractor will be responsible for coordinating the necessary work and provide the customers with adequate communication.

    Fuel Gas Code, 2009 Edition, NFPA 54, ANSI Z223.1, or latest edition. All steel pipes shall be at least of standard weight(Schedule 40) and shall be black iron that complies with ASTM A53, Standard Specification for Pipe, Steel, Black and Hot-Dipped, Zinc- Coated Welded and Seamless. The contractor will also be responsible to provide pipe dope for the installation ofthis material, plastic (PVC) sleeves for masonry walls, caulking and paint for the outside houseline piping. The Company willprovide all service line materials necessary to pipe from the main line to the outlet of the meter set assembly.

    76-908 and 76-910 ADD PIPING / FT.

    Are additional pay items as defined within the unit descriptions 76-900, 76-902, 76-904, and 76-906.

    77 RELIGHT CUSTOMER GAS EQUIPMENT (OH, KY, PA, VA)

    This item will include the relighting of customer gas equipment due to the disconnection of gas service. This item will includerestoring gas service to a customer, according to applicable Columbia Policy and Procedures and requirements of the National FuelGas Code (NFPA-54). This includes necessary house piping tests such as meter dial tests (where permitted), pressure-drop tests,etc., as well as visual inspection of piping, venting, and equipment installations. Appliances will be lit provided the equipment isin proper functioning correctly; this would include checking the appliance for proper combustion, ventilation, venting, andoperation according to Columbia procedures and requirements of the National Fuel Gas Code.

    Pre-testing of house piping should be considered and is recommended in situations where house piping may be considered in poorcondition or when re-establishing service to a slab type homes (house lines are typically under the concrete slab).

    Contractor will be responsible for coordinating the necessary work and provide the customers with adequate communication.Contractor will be provided a picture ID for performing re-lights and meter move-outs. Contractor is responsible for ensuring thatnecessary paper work (Service Orders) are filled out correctly and submitted to the Owners Authorized personnel in a timelymanner. This item will be paid regardless of the appliances being re-lit, provided the contractor went through all the necessarytasks mentioned above and provided all necessary documentation, as to why the relight was not performed.

    98 TIME AND EQUIPMENT RATE ADJUSTMENTS

    Adjustments for Time and Equipment rates shall be restricted to the following:

    Hours charged for show-up time when no Work is performed - 1 1/2 hours Hours charged for Work started and discontinued prior to four hours - 2 hours or actual hours, whichever isgreater

    Hours charged for Work started and discontinued after four hours - Actual hours

    Overtime multiplier when applicable and authorized by Owner for labor elements only - 1

    Equipment Units will only be paid for the period that the equipment is utilized

    98 FLAGGING

    This item will be paid on blanket work when full time flagging requiring additional man power on site is necessary and agreedupon between Contractor and Owner or is required by the governmental agency having jurisdiction. This item does not includeincidental flagging as covered elsewhere in the Contract.

    The Contractor shall furnish all labor, supervision, equipment, tools, materials, transportation and services necessary to performand finish the work as herein specified. Equipment provided by the Contractor should include hard hats, safety vests, radiocommunications, flags, signs, and any other equipment and materials necessary to perform the Work. All Work shall beperformed in accordance with the requirements of Owner.

    98 SEWER FACILITY LOCATES AND CONFLICT IDENTIFICATION

    Prior to commencement of excavation activities, the Contractor is to perform an adequate sewer investigation relative todetermining the existence of and location of existing sewer facilities.
