DoH – Behaviour Change Model – running for 7 years

Evaluation showed a 72% sustainedbehaviour change in participants

Over 8000 CWC across England

Aim

Reduce health inequities by targeting people from socially disadvantaged and other ‘hard to reach’ groups and supporting them to change their lifestyle and behaviour

The role of CWCs

To identify, or have referred to them,individuals or families from hard to reach and / or disadvantaged groups

People / families can self refer too

Work with people 1:1, or in a groups,to discuss their lifestyle and wellbeingand identify areas for improvement

Work to set goals, agree an action plan, providing support where necessary

Monitor and review progress, revising the plan where necessary

Referral Criteria

Desire to improve healthand or wellbeing

Need some form of supportor information

Need help accessing other services(health or community)

Consent provided (having readand signed the consent form)
