LockOut TagOut (LOTO) and its impact on hospitals

• Hospitals are accredited by The Joint Commission (TJC). TJC is the regulatory body having primary jurisdiction over hospitals:

• Joint Commission’s Environment of Care accreditation quality audit section specifies conformance to Federal Safety Regulations.

• OSHA Lockout / Tagout regulations apply.

• TJC concurs that the procedures must be readily accessible 24/7.

• TJC identifies nonconformance as a serious ‘Leadership Finding,’ which must be corrected within 45 days.

• Failure to implement jeopardizes accreditation and the facility’s Medicare/Medicaid funding.

OSHA Requires Machine-Specific LOTO Procedures:

1) OSHA requires machine-specific LOTO procedures for equipment with more than one energy source.

2) These procedures must be documented and identify the equipment covered.

3) A separate procedure must be created for each distinct piece of equipment to be locked out.

4) The procedure must include specific steps for shutting down, isolating, blocking and securing equipment to control hazardous energy.

5) The procedure includes specific steps for the placement, removal and transfer of LO/TO devices.

6) The employer shall conduct and certify periodic inspections of the lockout procedures at least yearly.

OSHA requires machine-specific procedures and is stepping up enforcement.
