The concrete contractor must provide a concrete permit prior to each project. Contractors will have three permit payment options: 1) Obtain a $40.00 concrete construction permit for work done within the right-of-way. 2) Obtain a $40.00 concrete construction permit for work done on private property. 3) Obtain an $80.00 concrete construction permit for work done both on private property and within the right-of-way. Please obtain a permit from the City Engineer's Office (2nd floor of City Hall) prior to every project. 2. The staking of line and grade for sidewalk sections will be done by the Engineering Department for each permit requiring such work. Permit must be obtained at least five (5) working days prior to commencement of work to allow time for staking. 3. Please provide the schedule, including approximate beginning and ending dates for the concrete work, prior to the approval of the concrete permit. 4. Permit holders will contact the City Engineering Department at 715-421-8205 at least 48-hours prior to pouring any concrete to allow time to inspect forms for line and grade. 5. Contractors are responsible for providing traffic control to protect the public during asphalt, concrete or excavation process. Traffic control must be done in accordance with the US Department of Transportation’s Manual on Uniform Traffic Control Devices. The contractor can request traffic control to be done by the City at the cost of the contractor. 6. The permit holder is responsible to contact Diggers Hotline (1-800-242-8511) for location of underground utilities prior to any construction of sidewalks. 7. Section 6.02(11) of the Wisconsin Rapids Municipal Code requires that concrete construction on City right-of-way must be cured by covering with suitable material or sprayed with water retaining compound. 8. Section 11.06.65(e) of the Code states the maximum width of a residential driveway is 30-feet not including approach tapers. All other zoning areas have a maximum width of 36 feet not including approach tapers. 9. Prior to any concrete construction for utilities, the Engineering Department must be contacted to inspect the work prior to construction. Concrete Requirements
