Two months back ,child was apparently normal. Child has fever and respiratory distress since two days back. Child was diagnosed as case of diaphragmatic hernia at GMCH-32, from where he was referred to PGI.History of Past illnessNo significant history of past illness.

Family HistoryNo of family members 5Type of family Nuclear No H/O Congenital problems in the family.Health facility near home.Child lives in a village. There is a dispensary at a distance of 1 km .HousingPatient lives in her own pucca house. There are total 3 rooms. Indian toilet facility is there. Source of water is from tap and hand pump.Personal HistoryChild is on normal diet . The child used to have16 to 18 hrs of sleep at home.Birth History child is born by FTNVD . There is no history of any birth injury or birth asphyxia during birth. Child cried at birth. Birth weight- 2.5 kg .IMMUNIZATION HISTORYImmunization complete till date.NUTRITIONAL STATUSChild is on breast feeding.
