PARENTAL FORM TO ASSESS SYMPTOMS

According to CDC, people with COVID-19 have had a wide range of symptoms reported – ranging from mild symptoms to severe illness. Symptoms may appear 2-14 days after exposure to the virus. People with these symptoms may have COVID-19:

• New loss of taste or smell• Sore throat• Congestion or runny nose• Nausea or vomiting• Diarrhea

• Fever or chills• Cough• Shortness of breath or difficulty breathing• Fatigue• Muscle pain or body aches• Headache

PARENTAL AGREEMENT OF DAILY TEMPERATURE CHECKS

Parents will need to agree that guests will have his/her temperature checked via a non contact thermometer.

If a guest’s temperature is 100.4°F/38°C or higher, a second test to confirm a temperature of 100.4°F/38°C or higher will be taken. If both readings are 100.4*F or above, the guest will not be allowed to stay for the session and will have to leave the store.
