Sizewise Rentals is committed to working with our employees to provide a safe work place.
It is our policy that employees should report unsafe conditions and will not perform work tasks without using the appropriate safety equipment or Personal Protective Equipment.
We all play an equal part. We have established this initiative to
achieve our goal of minimizing work place injuries and incidents.
Each site will track Accident free days using a poster.
This poster is to be posted in plain view of all employees.
The Site Manager will update the poster daily to reflect how many consecutive accident free days the site has accomplished.
On the lower portion of the poster, contact information has been provided for each employee to report any safety issues or concerns to the Sizewise Safety Officer.These calls will remain confidential whenever possible.
Any workplace injury or accident requires the Incident Report Form to be completed and submitted to Human Resources within 23 hours.
Only an accident resulting in a visit to a health care provider will reset your days to “0”.
What is a reportable incident or accident?
Any vehicle accident that results in damage to any vehicle or property, injury to any person requires the Vehicle Accident Report Form to be completed and submitted to Human Resources.
Only reports that result in a claim or payout against the Company will reset your days to “0”.
Any equipment malfunction at a facility that results in patient or staff injury is to be documented on the Company Incident Report Form.
Only malfunctions due to improper maintenance procedures or lack of in
service will reset your days to “0”.
An incentive program has been developed to encourage and reward employees and sites for achieving the Company’s goal of operating accident and incident free.
These incentives are intended to reward safety-minded employees and sites, as well as to provide recognition for accomplishing these goals.

The focus for each month will be identified on the monthly poster.

Reporting a Worker Compensation Claim:
It is Company Policy that all incidents and accidents are documented and reported to Human Resources within 23 hours of the incident.
What paperwork is required within
23 hours?
The latest revision to the Incident Report Form is located in the Hazardous Communication/Infection Control Policy and Procedures Manual maintained within the MSDS Right To Know Station.
Replacement forms can be downloaded and printed from the company server.
If I complete this form and submit this,
will I loose my incentives and be considered and
unsafe employee?
No, the Worker’s Compensation process begins
only if you require a visit to an
occupational clinic or physician for
medical treatment!
What is the first step if I need to report an
injury?
The first step in the process is to report
the incident or injury to your immediate
supervisor.Your supervisor will complete the Injury Kit paperwork and begin the process.
The Injury Kit is designed to assist both the injured employee and the Company in providing a positive outcome and returning the injured employee to work as quickly as possible.

Reporting a Motor Vehicle Accident:
What about
motor vehicle
accidents?
Motor Vehicle Accident Report Forms and
Procedures are located in the Vehicle
Inspection Log Book within the vehicles.
Complete this form
immediately and notify your supervisor.
Take pictures of the accident with a disposable camera.
Pictures should include all vehicles or property involved in the accident.
Submit all information to Human Resource within 23 hours of the accident.
If you are injured, seek medical
attention and notify your supervisor or Injury Counselor as soon as possible.
Product Incident Reporting:
Complete this form if there are any unusual, harmful, or potentially harmful occurrences involving clients, patients or equipment.
Submit this form to Human Resources within 23 hours of the reported occurrence.
Near misses can be defined as: An employee narrowly escaping an injury or accident.
These can be reported by either the employee or another employee who witnessed the event.
