CR was established in 1975 to serve individuals with mental health diagnoses or intellectual disabilities as well as those who

were homeless. Since that time, CR’s programs have expanded while remaining focused on helping individuals experience greater

independence, dignity, fulfillment and happiness within their own communities.

Since CR’s inception, literally thousands of individuals have been assisted. Every man, woman and youth we work with receives specialized and focused attention designed to promote fulfilling, high-quality lives of maximum independence.

CR supports residents of Arlington, Fairfax, Chesterfield, Loudoun, Prince William, Richmond and Anne Arundel (Md.) and is Virginia’s largest private provider of services to individuals with intellectual disabilities. Today, CR employs over 400 professionals and supports over 600 individuals annually in nearly 40 programs.

Chair’s Letter

Since 1975, CR has strengthened the fabric of our community by providing compassionate and personalized support

to individuals with disabilities. Today, we offer independence, opportunity, quality-of-life, and community to individuals with intellectual and/or mental health disabilities, youth transitioning from foster care, those without a permanent residence, and individuals living with HIV/AIDS. The crucial work we do has been made possible by CR’s supportive network of neighbors, friends, advocates, volunteers and a dedicated staff.

Over the past decade, CR’s capacity to serve has nearly doubled as our quality of care has only increased; this has been achieved through the drive and commitment of the entire CR network. As the future offers new opportunities, we sometimes face challenging obstacles to increased growth. We will continue to humbly turn to you for support.

An example of new opportunities and challenges is last year’s US Department of Justice settlement with the Commonwealth of Virginia. With the scheduled closing of state institutions that support our neighbors with intellectual/developmental disabilities, we are provided an opportunity to offer more Virginia residents the least restrictive living environment possible; a core philosophy of CR and of the vast majority of our colleagues. Nevertheless, available funding to develop the requisite community-based supports is in dire conflict with the imminent closure schedule and the existing capacity of the private sector. Of the funds available, allocations do not adequately address the often complex medical and/or behavioral needs of those leaving institutions. As the state’s largest private provider of supports to individuals with intellectual disabilities, CR is particularly well poised to expand its residential services. Yet despite an agency-wide effort, by mid-2014 – nearly two years after the settlement was announced – we have received funding only to support six additional individuals. Other private providers are facing similar funding challenges. Still, we remain resolved and continue to work by communicating with legislators as we seek adequate funding.

Over the past year, at least one potential challenge has turned to opportunity that celebrates CR’s adaptability. Across the country, mental health services are compromised as states are forced to cut budgets, and a new report released by NAMI (National Alliance on Mental Illness) exposed that Virginia has cut funding for mental health programs by 9 percent – $38.6 million

– since 2007. While these cuts may have meant the end of Arlington Artworks, CR’s therapeutic art program, instead it led to the program’s expansion. Today, by supporting not only adults with mental illness, but also individuals with intellectual disabilities, the wider distribution of costs has ensured the program’s continuation while enhancing the lives of even more individuals. Most importantly, Artworks is still available to those who have previously thrived through the therapy.

In the dynamic and evolving environment in which we find ourselves, it is individuals like Jonathan that provide unyielding inspiration. Two years ago a middle-aged Jonathan, long believed to be completely deaf, came to live in one of CR’s community-based homes. Soon after settling in, our devoted support staff became doubtful of the degree of Jonathan’s hearing loss. A long overdue test proved his partial ability to hear, and today Jonathan is wearing hearing aids, working with a speech therapist, and finally enjoying vocal communications. His expression of joy and satisfaction encourages us to increase our efforts and accept new challenges.

In that spirit, we maintain our focus as we continue to explore new opportunities. Thank you for advocacy and support, and for being our community.

President’s Letter

It is only through the enthusiastic initiative of community leaders like Nancy Falck and the late Christine Nall that CR is here today

to serve over 600 individuals each year. Nearly four decades ago in Fairfax County, Nancy and Christine founded a program to support individuals returning to the community after psychiatric hospital discharge. Together, the women not only began a housing program, but brought to it the various mental health and community support services that would help facilitate long-term success. For the first time, county residents were able to benefit from all available services in one place. As a result, the number of individuals being re-institutionalization dropped significantly. Under Christine’s leadership, the Fairfax model was taken to its neighboring county with the eventual result that the nonprofit Arlington Community Residences (ACR) was established. Serendipitously, at the same time Virginia’s Governor Linwood Holton provided the first state funds for community mental health centers. His successor, Governor Mills Godwin, established legislation that led to the formation of the state’s Community Service Boards, which today continue to manage and direct funding for mental health, intellectual disability and substance abuse services.

With heavy volunteer involvement, ACR supported dozens of local residents, improving the lives of individuals, their families and the community as a whole. Today, as Community Residences, we continue with the same passion as that of our founders. Over the past 40 years CR has seen growth in the population served and the needs addressed. In six counties throughout Virginia and Maryland, CR promotes the health, happiness and general well-being of individuals with mental illness, intellectual disabilities and/or HIV/AIDS and those who are chronically homeless.

With leaders like Nancy and Christine, advocates from around

our nation have driven a movement which has influenced the evolution to community integration and increased

empowerment and independence for all individuals with disabilities. Case in point: according to the National Council on Disability, while in 2009 there was a reported 33,000 individuals with intellectual or developmental disabilities residing in state-run institutions, this was a decline of 167,000 from a peak of nearly 200,000 individuals in 1967.

Another example of this evolution is the words we use. Since 1975, those supported by CR

have been called patients, then clients, consumers, and finally a deserved and simple individuals. We no longer use the term “retardation.” Each person we support is respected for his or her individuality and provided with personalized support, sincere compassion and devoted care.

Today, there remains a great need for community-based residential care and non-residential support services. Hundreds of individuals are being transitioned from Virginia’s Training Centers while thousands more are on waiting lists for services. As Virginia’s largest nonprofit provider of services to people with intellectual disabilities, it would seem that CR should be poised for continued expansion. Yet we struggle to find solutions, challenged by a lack of funding and societal disagreement over appropriate living situations and needed supports.

Still to this day inspired by Nancy and Christine, CR will continue to grow, seeking ways to support more individuals while maintaining an unparalleled excellence of care. I am grateful for the community’s enduring support, for the evolution of understanding, and for the engagement of the many individuals we are so honored to serve.

We take pause to celebrate the lives of those we lost.

Brenda Cantrell Brenda Cantrell lived at our North 8th Road home, where her contagious laughter and zest for life brought much joy to those surrounding her. Brenda was

a collector of little treasures. Whether it was

papers, applesauce tops, spoons, paper towels

or sandwich bags, Brenda found in every new

collectable something special. Christmas was Brenda’s favorite time of year.

The warmth of hot cocoa on a winter’s day and the idea of Santa bringing her

more prized toy cars to add to her collection kept her in the spirit year round.

While Brenda is gone from our everyday lives, her spirit lives on and brings

smiles to the faces of everyone who had the pleasure of knowing her.

Charles HicksCharles Hicks will be remembered as an all around energetic, fun loving, charismatic man. Just hearing his name puts

a smile across the faces of so many people -

especially the staff of CR’s Maryland programs, in

which Charles participated. Charles was fiercely

independent – even wonderfully opinionated. A

diehard Eagles fan, he was often heard in a high-spirited row defending his

team – especially against a Ravens’ fan. He loved taking road trips with his

sister and relished spending time with his nieces and nephews. Charles was

never without a good story, always keeping a captive audience entertained

with tales of his work day or by sharing his knowledge of the Civil War.

Charles touched the hearts of many people around him and we are deeply

honored to have known such a wonderful man.

Christine Poole NallChristine Nall, one of the founders of Community Residences, passed away in June at the age of 79. A long-time Arlington

resident, Christine worked tirelessly on behalf

of individuals with mental illness. She and dear

friend Nancy Falck (a long-time resident of

Fairfax) created a housing program for individuals

being discharged from psychiatric hospitals. But it wasn’t only housing; the

discerning women also brought together various other support services,

including vocational and re-socialization training, to be offered in conjunction

with stable housing. Christine and Nancy’s efforts evolved into Community

Residences. Christine was dedicated to family and community and enjoyed

caring for people. Christine’s husband Julian Clark Nall, daughter Sallie Long-

Frazer, a granddaughter and three great grandchildren reside in the Nashville,

Tennessee area where Christine lived with her husband.

John PurpleJohn joined the Community Residences family in September 2010 when he moved into our Parliament Drive home. Later, in 2012, he relocated to our Oak

Street home.

Describe John Purple in one word? – Fashion!

John took great pride in his appearance –

matching shoes and belt, matching shirt and tie, sunglasses in place. John

never left the house without looking his best. A smart and confident man,

John enjoyed challenging himself with crossword puzzles and reading and

writing. He especially enjoyed hand-writing words and definitions from his

dictionary. Though John left us this past spring, his vigorous presence forever

remains a part of CR.

Kathleen McCarthy Kind, warm and charming, Kathleen McCarthy is missed dearly by the residents and staff of Lake Jackson, where she resided since 1996. Known as

the darling of the home, Kathleen participated

in myriad activities with her peers. Days before

her passing, Kathleen’s family and friends, along

with CR staff members, enjoyed sharing stories of Kathleen as together they

planned for her 54th birthday party. Kathleen will be fondly remembered by

friends and family for her positive energy and inspiring presence.

Robert WoosleyRobert Woosley was an honorable and humble man who was loved upon the minute of introduction at Roosevelt Street. He enjoyed jokes, loved to giggle and had

a passion for making those around him smile. A

dedicated man, Robert worked at The Department

of Engraving mail room for over 30 years. Robert

left us after a sudden illness that shocked and saddened the staff and residents

at Roosevelt Street. Our hearts are lightened by the memory of many jokes

and giggles shared throughout the years with our dear friend Robert.

Art Imitating LifeLast year, Alex Miller, Program Manager of Arlington ArtWorks – CR’s therapeutic art program – enjoyed afternoons in the studio with Sarah. A painting hobbyist who is passionate about her work, Sarah longed for full-time employment and the sense of empowerment, pride and self-sufficiency it would bring. Studio time with Alex provided Sarah the opportunity to work through fears and self-doubts (symptoms of her chronic depression) while learning new proficiencies. They discussed Sarah’s current skill set, past experience, what job might appeal to her, and the process by which to find a job. Studio time also allowed Sarah the opportunity to study her medium and she began exploring different subjects. ArtWorks offers its artists a venue for selling their work, budgeting supplies and tracking customer appeal. Sarah enjoys painting whimsical animals and discovered that these paintings were selling particularly well. Sarah intensified the tracking of her work; not just expenses, but production time and colors that most appealed to her audience. Sales of her paintings grew as she continued to heal. Not only did Sarah’s success in ArtWorks advance her well-being, but Sarah was able to include her experience and new skills in her professional profile. Subsequently, Sarah earned a full-time administrative position, providing the stability and life-balance she longed for.

Adopt-A-HighwayThis year, CR’s Richmond programs came together to give back to their community through Virginia’s “Adopt-A-Highway” program. We adopted Route 641 in Chesterfield County beginning at Hopkins Road and ending on Dalebrook. And so it was on this stretch of highway where CR’s first cleanup was recently held on a warm, sunny morning. Eighteen residents and a handful of CR staff wore bright orange vests and were announced by a sign reading “road work ahead.” All enjoyed chatting and the fresh air while giving back to their community.

CARE – Community Alternative for Recovery and Empowerment – is CR’s new crisis stabilization programThomas, a man in his mid-forties and chronically homeless, was admitted to CARE with a cancer diagnosis. Depressed and suicidal, Thomas reluctantly accepted the medical and therapeutic supports offered. Subsequent medical checkups facilitated by CARE revealed that Thomas’ condition had been misdiagnosed as cancer when it was in fact a condition readily treated with antibiotics. CARE secured and administered these medications, and Thomas’ physical health dramatically improved. The dark cloud of cancer lifted, Thomas became emotionally ready to invest in his own success. CARE staff even negotiated with the county to help Thomas receive permission to move in with his brother in an adjacent county while retaining his benefits, something previously deemed not possible.

16th Street Loves to RideAn active group living in a bustling Arlington community, residents at North 16th Street chose to drink in all that summer has to offer without leaving their neighborhood. But it’s not just strolls around the block! Rather, 16th Street decided to step out of their comfort zone and learn how to ride bikes! With just a little help from staff, two residents got the hang of bike riding very quickly. Jody, who had never been on a bike in his life, jumped right in with a smile on his face and by the end of the day was a pro! Jack used to ride regularly, but as of late had become a bit unsteady and riding was difficult. Nonetheless, after a day of focused practice with staff’s support, Jack was elated to be again tackling what he loved. Upon seeing the joy in his eyes, Jack’s sister purchased a three-wheeled bicycle for North 16th so that all could enjoy.

Strong Enough to Escape the ViolenceLisa had been working with a CR case manager when, this past spring, we opened our Psychiatric Rehabilitation Program (PRP) in Maryland. Lisa was identified as a promising candidate for the new program because both she and her case manager believed it would better address her mental health needs. Lisa’s teenaged daughter also struggled with mental illness, and Lisa found it difficult to manage her own mental, emotional and physical needs as she raised her daughter while living with domestic violence.

With the support and encouragement of PRP, Lisa made the decision to seek a protective order against her partner. Frustrated because she was able to secure only a temporary order against him, Lisa was not deterred. She was able to convince him to leave the apartment and then signed a new lease as sole renter. Lisa will continue to grow and heal with the help of PRP yet she has made such great strides - developing from a self-described “door mat” into a strong, resilient woman who knows what she wants and is determined to achieve it.

Following a nutrition seminar led by CR’s Registered Dietitian Julia Yuskavage, Alyssa, a resident at CR’s Vaughn Ball program, decided she wanted to make a life change for the better. Alyssa’s new determination to get healthy was not just about her weight – it was about avoiding further joint pain and the possibility of diabetes. In monthly meetings with Julia, Alyssa learned about healthy habits including intuitive eating and the critical importance of adding exercise to her weekly routine. Eliminating “junk” food entirely can be difficult for anyone, but Alyssa realized that by cutting portion sizes and substituting some healthier snacks, she could enjoy some treats without feeling deprived. Like anyone who has ever tried to change habits, Alyssa found herself frustrated at times. But staff’s positive energy, support and encouragement helped keep Alyssa on track. Alyssa focused on small goals to avoid further frustration and was excited to watch her BMI decrease from obese, to overweight, to normal in a matter of months. “It took commitment and focus for Alyssa to achieve her goal, and it was very rewarding to be able to facilitate Alyssa’s positive changes, both in her health and her outlook on the future,” smiles Julia. Alyssa continues to work out three days a week and make healthier food choices. Best of all, she is healthier than ever and staying positive.

All exercise and dietary decisions were cleared with Alyssa’s doctor prior to starting this transformation.

Every Gift Makes an Impact CR thanks Commonwealth Community Trust (a non-profit that administers third party and self-funded trusts for people with disabilities) for providing Teresa with a brand new reclining wheelchair. The new wheelchair offers the support that Teresa needs and the happiness she deserves. Without a reclining chair, Teresa is unable to keep her chin up (a symptom of an underlying medical condition). Teresa’s new wheelchair allows her to sit in a comfortable, upright position. It has made a dramatic, positive difference in Teresa’s life; she is now better able to be part of the group and is constantly smiling.

In Maryland, CR provides case management services, shelter supports and individual counseling to children and adults with mental health needs. CR also provides support case management support to individuals living with HIV/AIDS.

Vaughn Ball, Arlington

Finding IndependenceEmily’s high school counselor watched as she struggled through her senior year of high school. Finally after a meeting, the counselor learned that Emily had been homeless for the past year. Emily was referred to CR’s youth program

– LIFT (Living Independently for Tomorrow). LIFT offers housing, case management and therapeutic support aimed to foster independence and self-sufficiency. In July, depressed and having missed her graduation date, Emily entered LIFT dreaming of a high school diploma, a better paying job, managing her anger and learning the skills needed to someday have a place of her own.

Through LIFT, Emily obtained an apartment with a roommate, attended regular group therapy sessions and met weekly with her case manager.

LIFT’s case manager helped Emily access community supports and enroll in summer school. Emily began to face the loss of her mother and the feelings of abandonment by her family. She began to develop meaningful and achievable goals.

Months after entering LIFT, Emily graduated from high school, was feeling better about herself, began enjoying her peers, and even served as an inspiration to other young women in LIFT. During this time, Emily maintained full-time employment and even began to save.

With new skills, improved confidence, and better financial stability, Emily began to look for an apartment of her own. Less than a year after entering LIFT, Emily was accepted into a supportive housing program that requires only 20 percent of one’s income for rent, allowing Emily to continue pursuing an education and job advancement. Emily says she loves the feeling of independence and of conquering her goals. For the first time in years, she is looking forward to her future.

We consistently explore new ways to improve the lives of the over 600 individuals we serve each year. Likewise, we are abundantly aware that

many adults and youth in our community are not receiving the support they need. Through staff, volunteers and our community’s combined

capacity and collective expertise, we continue to explore new opportunities while never losing sight of the happiness and well-being of each

individual we currently support.

Apartment LivingParamount to CR’s mission is individual opportunity and independence. Appropriately, CR recently began supporting six adults, previously living in congregate settings, who now hold leases to their own apartments. Buchanan Gardens, owned and operated by the Arlington Partnership for Affordable Housing (APAH), is a newly renovated garden apartment community offering proximity to welcoming parks, public transit, and a host of community supports. Prior to apartment living, each of the men and women resided in one of CR’s many community-based, family-sized homes serving adults with intellectual disabilities. While living in these homes, each of the residents, with the support of CR’s proactive mentoring for independence, demonstrated a forward level of self-sufficiency. CR’s senior clinical staff thus introduced the concept of movement toward even greater independence. With each individual’s accord, intense conversation between individuals, family and staff followed. Those who made the personal decision to make the move and who are now succeeding at Buchanan Gardens report greater happiness and self-satisfaction. At Buchanan Gardens, CR provides individualized support allowing for a life of independence and pride. Staff makes daily visits and is on-call and available 24-hours a day. One resident, Ester, is living on her own for the first time at age 53. She enjoys hosting visitors and says, “I just love living here.”

Crisis Stabilization – CAREIn April, CR opened CARE – Community Alternative for Recovery and Empowerment – located in a quiet Fairfax County residential community. CARE is a place where those who are experiencing a psychiatric crisis can heal in a safe home environment that is welcoming, comforting, and accepting. The goal of CARE is to stabilize individuals in crisis while preventing hospitalization and future crisis situations. CARE’s staff of counselors, registered nurses, psychiatrists and nurse practitioners provides immediate, individualized and intensive support. The crisis team coordinates professional supports, natural supports, and community resources that form a foundation for continued personal growth and recovery. Over an average of about 12 days, individuals who arrive in crisis are supported to return to their own homes and communities with the skills and resources to better manage their lives and the challenges they may face. CARE has proven immensely successful and has been extremely well received by the local mental health community. Since its opening, the program has more often than not maintained a waiting list.

Psychiatric Rehabilitation ProgramExpanding current services in Maryland, CR was granted clinical licensure by the State to open a Psychiatric Rehabilitation Program (PRP). PRP provides clinical support to adults and children impacted by serious emotional or mental health challenges. Through on site group and home-based individual counseling sessions, community involvement and psycho-education, individuals improve their level of functioning or resume their former level of stability. The program provides practical training in social skills building, symptom management, conflict resolution, daily living skills, time and household management and wellness practices.

Vital to the success of PRP is the mini-van funded by our donors through the CR Foundation. Reliable transportation is crucial as it ensures consistent participation in PRP’s clinical group sessions – essential to an individual’s success. And, with PRP social workers as the van drivers, casual drive-time conversation fosters relationships and helps maximize success during formal therapy.

What’s NextIn the spring of 2014, CR will support an additional six adults with the opening of a new community-based, family-sized home in Fairfax County. Rather than a retro-fitted home to support the needs of residents with intellectual disabilities, the new “Burke Road” program is original construction that has been specifically designed to suit the needs of those who will call it home. Moving forward, as we seek to support more of our community members, CR will use Burke Road’s success as a model for continued expansion.

Clinical Highlights (Continued)Continued Focus on ExcellenceOver the next year, we anticipate Commission on the Accreditation of Rehabilitation Facilities (CARF) accreditation for our Chantilly-based Newbrook Place Day Program. Since its opening in June of 2012, Newbrook Place has witnessed a host of personal triumphs for those in the program, and it has been heralded as a model for individuals with intense behavioral needs. Unique to the program is a daily community-based activity that complements Newbrook’s efforts to teach coping mechanisms, social dynamics, improved self-esteem and independence. CARF accreditation will allow CR to definitively convey Newbrook’s outstanding standards and performance, while recognizing CR as an organization that is focused on person-centered planning in all that we do. CARF International is a highly respected independent, nonprofit accreditor whose seal signifies excellence in the fields of health and human services.

CR’s unwavering commitment to be the leader in offering the highest quality services and programs is ensured through our Mission Effectiveness efforts, which consist of the work of our Mission Effectiveness staff and the oversight of our Board’s Mission Effectiveness Committee. This team’s sole purpose is to provide close internal oversight and guidance to dozens of programs and hundreds of employees. Through regular internal audits and monthly metric tracking, our Mission Effectiveness efforts ensure optimum performance in supporting the well-being, health and safety of the individuals we support. As a part of its ongoing efforts in this area, CR is expanding its quarterly metrics to include indicators that will help us ensure maximum independence and self-fulfillment for the individuals we support. By tracking personal participation in activities such as grocery shopping, cooking, housekeeping, individualized recreational pursuits, and religious, spiritual and cultural activities of an individual’s choosing, our team will have greater awareness and insight to support strategies and actions that foster personal growth within our entire agency.
