RESEARCH PROFILE
My research focuses on cognitive and functional recovery in geriatric surgical patients. My long
term goal is to enhance understanding of the disproportionate risk associated with surgery for
the older patient and create interventions to improve recovery while maintaining a strong track
record of extramural funding. In 2011, I received an Alzheimer’s Disease Research Center Pilot
grant and then shortly afterwards an NIH R03 GEMSSTAR/Jahnigen research award to
investigate anesthetic choice and the occurrence of postoperative delirium and cognitive
dysfunction after major surgery in the elderly. This pilot project became the basis for my current
NIA Beeson K award; I am the second anesthesiologist to have received the award in the 30
year history of the program. My current grant investigates the role of intraoperative burst
suppression in development of POCD and PD and utilizes a more nuanced approach to the
analysis of cognition, examining executive function and memory separately. As my work has
progressed I have expanded the scope of my research to focus on preoperative risk factors
which may be amenable to intervention and patients who will require additional supportive care
in the immediate postoperative period. To this end I have published a study examining
preoperative frailty as a predictor of postoperative outcomes, and have created a tool in EPIC
which will allow us to perform a prospective study to measure frailty in the pretesting clinic. I
have also been actively investigating the use of short form cognitive testing to screen patients
for the strongest predictor of postoperative cognitive decline: preexisting cognitive impairment.
CLINICAL PROFILE
I am a neurosurgical anesthesiologist at The Mount Sinai Hospital. Our tertiary care center has
provided me with expertise in anesthesia for wide variety of neurosurgical procedures including
functional neurosurgery including deep brain stimulation, interventional radiology management
of intracranial aneurysm, spinal cord stimulators, craniotomy for tumors, skull based procedures,
acute stroke, and complex spine. I have been the requested anesthesiologist by many of the
neurosurgeons and regularly am consulted to see patients at their preoperative visits for
evaluation and planning. In 2009, I received a secondary appointment in the Department of
Neurosurgery. In the previous year I have served as the division head; in that time our case
volume has increased by 700 cases over the past year alone (total 9134 cases, over 8%
increase in volume). Currently I have transitioned to a supportive role as much of my leadership
effort is occupied with research and administration. My national presence in the Society for

Neuroanesthesia continually presents opportunities which I share with my colleagues for clinical
trial participation and scientific presentation at the local and national level.
IMPACT
My research has significant potential to identify treatable clinical risk factors for PD and POCD
in older adults. These findings will also facilitate resource allocation and patient/provider
decision making which I believe will have a positive impact on patient experience and delivery of
care. I am a leader in the national public health campaign sponsored by the ASA to raise
awareness and educate patients, providers, and stakeholders regarding brain health in the
perioperative period for older adults. In the time that I have become Interim Vice Chair for
research I have focused on improving the departmental research infrastructure with the of goal
increasing the number of high quality and funded projects. To this end, I have instituted a
RedCap data system to catalog and track all Departmental research projects (productivity,

Neuroanesthesia: Over the years I have worked with the neuroanesthesia team and surgeons to
ask critical questions and publish our experience in the operating room. My work in
neuromonitoring and anesthetic technique has clarified the relationship between anesthetic
techniques during spine surgery and evoked potential signal loss. My paper, “Patient
characteristics and anesthetic technique are additive but not synergistic predictors of successful
motor evoked potential monitoring,” examines the relationship between preexisting patient
comorbidity and preoperative difficulty in recording motor evoked potentials. This study helps
anesthesiologists to prospectively identify difficult-to-monitor patients and use this information to
guide their choice of anesthetic. I have also published multiple reviews and chapters that
synthesize the existing literature regarding anesthetic technique and signal loss. My work in this
area has increased awareness among anesthesiologists regarding our ability facilitate

monitoring of the spinal cord during surgery. My group has published a retrospective study
regarding whether rising intraoperative lactate during craniotomy is associated with poor
outcomes and is currently embarking on a prospective study based on our findings. My
leadership in our ongoing perioperative committee with the neurosurgeons has encouraged
process related communication about difficult or high risk patients prior to surgery. Our goal is
to improve patient and surgeon satisfaction and decrease the number of day- of- surgery delays
and cancellations.
Teaching (impact on trainees)
I work with residents to consider their future plans for fellowship and review for their oral board
exams. I have had several clinical mentees; we continue to maintain a neuroanesthesia
fellowship slot which we reserve for top notch residency graduates with research interests in
neuroanesthesia.
