Executive summary This deliverable reports in detail on CareWell dissemination activities during months 25 to
36 of the project, as well as offering a comprehensive overview of the entire progress and completion of the project's dissemination work tasks..
CareWell dissemination activities have been based on an elaborated multi-channel
dissemination and communication strategy. The strategy has employed different media
to reach the relevant target audiences. The thematic focus of dissemination activities has followed the progression of the work tasks closely. The dissemination strategy aimed to
communicate the CareWell rationale, objectives and approach, and offer an
understanding of how these have been put into practice.
The major dissemination achievements of the third period are:
Continuing to publish news on the project website, 28 items in 2016, adding to a
total count of 59 items as of January 2017.
Initiating the production of an animated dissemination video focused on the
benefits of integrated care services piloted as part of CareWell.
Attracting over 259 followers on Twitter, and consistently using the platform to
interactively promote project events and outcomes.
Having a project-wide press release on the start of the pilots circulated in the May
2016 EIP on AHA newsletter
Participating in 28 public events, including several high profile international
conferences, reaching a total count of 110 public events.
The six awards received by the Basque Country pilot site.
The inclusion of two research papers presenting results from CareWell in
academic journals.
Strengthening synergies with BeyondSilos, SmartCare and the EIP for AHA, both
online and through public events.
Five CareWell pilot regions being awarded EIP on AHA 2016 Reference Sites
status.
Hosting an Advisory Board meeting during the 7th Project Assembly meeting in
February 2016.

1 Introduction
1.1 Purpose of the document
Dissemination refers to activities directed outwards which sustain the results of the project. The aim is to communicate the results and intermediary steps to different target
groups that can be assumed to be interested in CareWell. Resuming from the conclusions
of the previous report, the present deliverable evaluates the continuation of the project’s
established dissemination strategy, and offers a complete overview of the efforts
undertaken over the entire duration of the project.
1.2 Structure of the document
The document is structured into six sections, followed by five appendices:

Section 2 offers an overview of the underlying dissemination strategy of the project and its building blocks, as well as a condensed summary of the complete progression of the
dissemination work package, divided along the lines of the three reporting periods of the
project.
Section 3 is the most extensive, covering the totality of CareWell’s dissemination activities over its third reporting period, with a comparative view to the previous two.
The activities are split between:
The project’s broad online presence covering: the status of the project’s websites
(including web analytics), use of social media and press releases.
The various public events the project has been part of, such as: conferences, presentations and workshops, pilot site press conferences, events organised with
the project’s involvement, awards and accolades received by the project.
Published CareWell research papers.
Promotional material produced.
Section 4 focuses on the reception of the project in the third reporting period, as
reflected in the media coverage and the multiple awards the project received throughout
its lifespan.
Section 5 shows how dissemination activities in the third reporting period strengthened
the project’s synergies with sister projects and key initiatives such as the EIP on AHA.
Section 6 presents the plans for the Joint Final Conference with BeyondSilos.
Appendix A lists all public events with participation from CareWell partners.
Appendix B lists all the headlines of news published on the CareWell official web site.
Appendix C lists all CareWell-related scientific publications produced by the consortium.

2 Overview
2.1 Strategy summary
The previous two dissemination reports highlighted the multi-channel strategy agreed upon by members of the Consortium with regards to raising awareness of the project.
Dissemination activities have been a horizontal activity within the CareWell project, and
have been strongly related to all the other work packages. The dissemination work
package has received input from multiple work tasks, depending on the project phase,
and has closely followed the progress registered. Project aims, plans and (interim) results have been disseminated and communicated to all interested parties from the
project kick-off onwards through a large set of different dissemination channels. In order
to be effective and efficient, the dissemination strategy and channels addressed have:
Been oriented towards the needs of the audience, using appropriate language and information levels.
Included various dissemination methods: written text including illustrations, graphs
and figures; electronic and web-based tools; and oral presentations at community
meetings and (scientific) national and international conferences.
Leveraged existing resources, relationships, and networks.
Interacted with and effectively linked to other relevant projects and initiatives.
Been effectively conducted at several geographical levels, using appropriate
dissemination channels.
CareWell’s multi-dimensional, large scale dissemination approach is depicted in the figure below (Figure 1).

2.2 Reporting periods summary
It is the aim of this final deliverable to present both the most recent reporting period and
the success of dissemination across the whole span of the project. Therefore this section
offers a review of the previous two reporting periods and concludes with the most recent
one.
2.2.1 First period
Establishing CareWell’s visual identity was a major achievement in the first reporting
period (M1-M12). Along with the development of the project’s official website and the
activation of its official Twitter account, it laid the groundwork for a sustained and
coherent approach to dissemination.
The first year also marked the production of 12 videos describing the initial phases of the
project and the ongoing plans and preparations in deployment regions. Multiple local
press articles drew attention to the project and sites. The first report on dissemination
revealed the partners’ unanimous commitment to making the project known both
regionally and internationally, tallying 47 public events with CareWell participation.
During the first period, the project’s contribution to and interaction with the EIP on AHA
was stressed, touching upon synergies with CareWell’s sister projects SmartCare and
BeyondSilos, working within the same framework. Several experts were contacted to
take part in the prospective project Advisory Board.
Potential dissemination target groups such as elderly people and their informal carers,
health and social care providers and professionals, public authorities, EU and local policy
makers, and ICT industry representatives were identified during the first period of the
project.
2.2.2 Second period
The second reporting period (M13-M24) continued in the same direction for
dissemination, and revolved around employing the established strategy in order to draw
attention to the newly operational CareWell sites. In order to keep partners informed of
the project’s many dissemination activities in various parts of Europe, as well as maintain an awareness of potential new dissemination opportunities, an internal dissemination
newsletter was initiated in 2015, to be circulated every two months.
An important milestone for dissemination in the second period was the preparation of a
project-wide press release announcing the start of the services, to be disseminated through official EC channels. The text was accompanied by the creation of individual
factsheets for each of the sites which made essential information readily available to the
public at large. Sites also raised awareness by organising local press conferences once
the services became fully operational. In total, the project participated in 33 public
events throughout 2015, including several high profile international conferences, and co-organised a one day conference with BeyondSilos and SmartCare. 2015 also marked the
project’s first awards, for the Basque Country, and the publication of several academic
papers.
2.2.3 Third period
The third and final reporting period of the project (M25-M36), which is the main focus of the present report, gradually oriented the established dissemination strategy towards the
announcement of the project's final results. The second half of 2016 was particularly
marked by taking concrete steps in the organisation of the Final Conference of the
project, (WT8.10), an upcoming milestone in CareWell's timeline.
The first major event of the second reporting period was the CareWell Advisory Board
meeting, which took place early in 2016, in the presence of four international experts
assembled as part of the project's Advisory Board. In May 2016, a comprehensive press
release highlighting case studies from the CareWell pilots was circulated in the official EIP

on AHA newsletter. With regards to event participation, CareWell partners have
continued throughout the entire year to participate in national and international
conferences, tallying 28 events in 2016, among them the 16th International Conference
for Integrated Care and the European Health Forum Bad Gastein. An open workshop
dedicated to CareWell's specifically tailored service innovation cost-benefit analysis tool, ASSSIST, was organised together with BeyondSilos as part of ICIC16. Two scientific
papers have been published. Following the reviewers' recommendations, the task of
promotional video production was taken up again by both the Dissemination WP leader,
empirica, and partners from each of the sites. An additional six awards were added to the excellent track record of the Basque Country site. The end of the year was also marked
by five of CareWell's regions becoming EIP on AHA Reference Sites as the result of the
most recent Call for Reference Sites.
2.3 Visual identity
CareWell's visual identity has remained consistent over the entire project duration. This section recalls the steps taken during the first reporting period in order to establish it.
2.3.1 Project logo and colour scale
The project logo that was agreed upon among all CareWell partners offered the basis for
further designs and layouts for all dissemination channels. It was circulated to all project
partners at the beginning of the project in different formats.

2.3.2 Document templates
Once the final logo was agreed upon, templates for Word and PowerPoint documents
were produced based on it, in order to consolidate a recognisable visual identity.

CareWell partners used these templates whenever they wrote project-related documents
or presented CareWell at conferences, events, seminars, and workshops, as well as
internal meetings such as consortium, board or review meetings.

3 Dissemination activities
3.1 Online activities
Online dissemination activities primarily supported and accompanied the numerous public events the project took part in; a public record of these has been kept on the project
website and the project’s Twitter account. A project-wide press release drafted in the
second reporting period, marking the start of the CareWell pilots’ operational phase, was
officially made public in the Newsletter of the European Innovation Partnership on Active
and Healthy Ageing (EIP on AHA), in May 2016. Partner websites have also highlighted the project on several occasions. Online activities have consistently strengthened
synergies with other projects and the EIP on AHA. The sub-sections below report on the
many online channels the project employed.
3.1.1 Official website
The CareWell official website (http://carewell-project.eu/) has been at the centre of the projects’ dissemination strategy since the very beginning. The layout of the home page
has remained unchanged since its official launch in July 2014, but sections have been
added to reflect the progress in the project. The current structure is illustrated below.

All public deliverables of the project are available for download in the “Output” section of the website, along with the CareWell flyer, pilot factsheets and the general CareWell
dissemination poster. All downloadable material is sorted into categories which are easily
accessible through an accordion-style menu.

3.1.1.1 Final Conference page
A new dedicated page for the CareWell - Beyond Silos Joint Final Conference has been
added to the official website in December 2016, in order to facilitate advertising the event. The page provides essential information about the agenda, venue and directions.
During the event, live streaming will be available. An identically structured dedicated
page has been added to the BeyondSilos official website as well. Registration for the
conference is done via the official CareWell and BeyondSilos websites.

3.1.1.2 Factsheets
CareWell pilot factsheets were compiled to provide the general public with condensed up-
to-date information about each CareWell pilot region at the start of piloting in 2015. The
six factsheets span two pages, and are available for download in the “Output” section of the official website. They are structured along the following lines:
The broader healthcare system.
Service components supporting the two CareWell pathways:
Patient empowerment and care support.
Integrated care coordination.
Patient recruitment and service rollout.
Contact information.

Each factsheet also contains an info box which gives basic information, such as the
population in the region, the local CareWell partners, the main innovation brought along
by CareWell, and the number of patients and professionals participating in the pilot.
3.1.1.3 Website news
The news section covered, as before, the project’s recent conference participation and
organised events, synergies with the EIP on AHA and fellow integrated care projects, as
well as important events in the project’s internal schedule. News about public events
usually included a link to presentation slides. Particular attention has been paid in 2016 to the results of the EIP on AHA Call for Reference Sites.
The news section has been updated regularly, with an additional 28 items published in
the third reporting period. The two most recent headlines are featured on the website’s
homepage. In total, 59 news items have been published so far, with several more planned for the months leading up to the final review of the project.

3.1.1.4 Analytics
Analytical data in Figure 8 shows that the official CareWell website has attracted over
2,900 visits during 2016, generating almost 6,500 page views. This a considerable
increase compared to the previous year, when visits topped at 2,200 and 6,200 page
views were amassed.. Most traffic was registered in December, when the CareWell - BeyondSilos Joint Final Conference was announced. Higher levels of traffic were
registered again in March and April, in the weeks leading to the project’s second review
meeting.

The vast majority of visitors are European. However, the single country with the highest number of visitors is the United States. The European countries with the highest number
of visitors are Spain, Italy, UK and Poland. These are the countries where CareWell
regions are present; this shows that the local dissemination activities are also reflected
on the website. The interest from the United States shows that CareWell is also well
received outside of Europe.

3.1.2 Auxiliary websites
Aside from CareWell’s official page, a joint webpage for CareWell, BeyondSilos and
SmartCare, entitled Integrated eCare in Europe, was created in 2015 under the domain
www.integrated-ecare.eu. The website highlights the three initiatives, explains their
common goal, and provides brief separate overviews, as well as listing all the partners involved. The project logos are featured prominently; clicking on any of them redirects to
the respective project webpage.
This was accompanied by the launch of Integrated eCare Wiki page
(http://wiki.integrated-ecare.eu/). Access to the page requires creating an account.
Partners are encouraged to join the Wiki in order to add and edit content.
3.1.3 Social media - Twitter
CareWell’s Twitter account (@Carewellproject) has continued to grow, following 169
accounts and reaching 258 followers as of mid January 2017. CareWell’s tweet count has
reached 154, with 158 likes. The focus has been, as before, mainly on announcing
upcoming events and reporting on past ones, as well as engaging in live-tweeting of events on several occasions. The account regularly interacted and engaged in mutual
retweeting with @BeyondSilos and @PilotSmartCare, as well as with those of CareWell
partners (@osakidetzaEJGV, @Kronikgune, @AresPuglia, @Dolny_Slask, @IFICInfo,
@Consorz_Arsenal), consortium members, and other relevant initiatives such as the EIP on AHA, and the Scale-AHA study. Particular attention was also paid to the results of the
2016 EIP on AHA Call for Reference Sites, which awarded five CareWell regions this
status.
Relevant hashtags such as #integratedcare, or specific event hashtags (#AgeingSummit) have been featured in many of the project’s tweets and retweets. The CareWell Twitter
account has also been used to increase traffic to CareWell’s official page, each recently
published news item being also announced and linked to on Twitter.
3.1.3.1 #YouAreCareWell
One of the more prominent hashtags employed on Twitter in 2016 was #YouAreCareWell. Following recommendations from the most recent general review,
CareWell attempted to make the people involved in the project more visible by
highlighting their role and including their Linkedin profile in a series of weekly tweets
between July and September 2016. The hashtag generated increased traffic.

As analytical data in Figure 12 and Figure 13 shows, CareWell’s most active month
overall on Twitter in 2016 was October, coinciding with the announcement of a new
publication in a peer reviewed journal, and reports on the European Health Forum
Gastein. October was also the month with the most profile visits in 2016.

The highest number of impressions registered was in September (Figure 14), when the

3.1.4 Project-wide press release
The press release announcing the beginning of pilot operations in the six CareWell
regions was circulated in the May 2016 issue of the Newsletter of the European
Innovation Partnership on Active and Healthy Ageing. It gave an overview of the project,
highlighting features from the Basque Country and Croatian deployment regions. Prior to the press release, the factsheets for each region (see section 3.1.1.2 above) were made
available for download in the “Output” section of the project website.

3.1.5 Partner press releases
Since the beginning of the project, CareWell partners have supported the project’s
dissemination strategy by featuring the project on their own websites. Coordinating
partner Kronikgune published two press releases in 2016 concerning CareWell, in both
English and Spanish, on their official website:

3.2 Public events
Throughout its 36 month duration, CareWell partners participated in over 110 events (for
a complete list see Appendix A) where the project was showcased through presentations, discussions or posters. Public events, from large international conferences to local
workshops, have proven to be a very effective way of raising awareness of the project.
The events that CareWell participated in or organised during 2016 are discussed below.
3.2.1 Conference presentations and workshops
The project was presented at several high-profile international conferences, often together with SmartCare, BeyondSilos and the EIP on AHA B3 Action Group, which
enabled CareWell to reach a large number of stakeholders at the same time, from policy
makers to medical practitioners, ICT industry representatives, patient group
representatives and medical researchers from different countries in Europe. CareWell partners also attended many other national conferences, as well as international, national
and local workshops. Several of these international events were high-profile events with
a large turnout (Med-e-Tel, ICIC, European Health Forum Gastein), attracting a multitude
of stakeholders. Table 2 lists all 2016 international events chronologically:

3.2.1.1 The 16th International Conference on Integrated Care
The annual International Conference on Integrated Care is organised by the International
Foundation for Integrated Care (IFIC). Over the course of the project, CareWell was present at the two previous iterations of the conference, and there are plans to attend in
2017 as well.
CareWell was well represented at this major 2016 event in the integrated care
international community. Three presentations and one poster highlighted achievements from the Basque Country:
Up-scaling of an Integrated Care Model for frail elderly patients. (Marisa Merino,
Session 1.1 on Frailty).
Modelling approach to managing the deployment of integrated organisational models for multimorbid patients in the CareWell study. The Basque Country case.

CareWell was also highlighted during the interactive workshop on business model development "How to make an integrated care service sustainable" organised by
CareWell consortium member empirica on Tuesday 24th May. The workshop served to
strengthen synergies with sister projects BeyondSilos and SmartCare.
3.2.1.2 19th European Health Forum Gastein
CareWell participated in the prestigious European Health Forum Gastein (EHFG), held between 28th-30th September 2016 in Austria. The European Health Forum Gastein is a
high-profile health policy event which has been taking place every autumn starting in
1998 in the Austrian spa and mountain resort of Bad Gastein. The forum is officially
supported by the European Commission, and engages key actors and experts from the 28 Member States, as well as the remaining countries in the WHO European region. This
year's event discussed the challenge of Demographics & Diversity in Europe - New
Solutions for Health.
CareWell was the focus of a presentation on “EIP on AHA –Integrated Care and Sharing Experience from Implementing Innovative Solutions”, delivered by project coordinator
Esteban de Manuel Keenoy (Kronikgune) on the last day of the conference. The talk was
part of a session titled “Reality meets Reality” organised by DG Connect, which
addressed the need to move towards digitisation in healthcare systems, and how this can

be best approached and coordinated by considering interoperability, data protection and
data management. The significant lessons CareWell shared with its audience emphasised
the challenges of defining new organisational models and integrating new care pathways.
3.2.2 Organised events
CareWell has been consistently organising public events, most frequently in cooperation with its sister projects, in order to disseminate the newest results and offer stakeholders
various opportunities to engage with the project. Most prominently in 2015, CareWell
hosted the Technology Innovations: Supporting Integrated Care at Home and in
Communities, on 20th October 2015 in Odense, Denmark, as part of the WHINN Week of
Health and Innovation. Two pilot regions, Zagreb and Lower Silesia, also organised high-profile press conferences in the second reporting period to mark the start of piloting.
These events strongly underlined the local political engagement of CareWell sites in
bringing integrated care a step closer, and were attended by local and national officials.
The most important events organised in 2016 are reported below.
3.2.2.1 ICIC16 Workshop: How to make an integrated care service
sustainable
Integrated care projects CareWell, BeyondSilos and SmartCare shared answers and
lessons through an interactive workshop on business model development at the 16th
International Conference on Integrated Care on 24th May 2016 in Barcelona. Participants were given the chance to discuss barriers and solutions for the implementation of
integrated care services. The purpose of the workshop was specifically to familiarise the
audience with the ASSIST tool and method, based on cost-benefit assessment, which has
been employed in all three projects. A model case scenario for a service supporting older people suffering from chronic obstructive pulmonary diseases (COPD) was simulated
using the ASSIST tool.
3.2.2.2 Advisory Board Meeting
One of CareWell’s tasks in during the first two years of activity was reaching out to and
seeking support from a group of experts who would form the CareWell Advisory Board.

An Advisory Board Meeting took place on 17th-18th February 2016 in Treviso, Italy, as
part of CareWell’s 7th Project Assembly, hosted by Consorzio Arsenal. The Advisory Board meeting took place on the second day; three discussion sessions were led by Puglia,

Veneto and the Basque Country. The main points of discussion were patient
empowerment, organisational models and implementation. Prof. Dipak Kalra, Dr. Albert
Alonso, Walter Atzori and Prof. Jean Bousquet were the experts who took part in the
meeting. The goal was for the advisors to support the project in eliciting lessons learned
and good practices. The conclusions of the Advisory Board meeting have been discussed within the consortium, and served to inform the deliverable D8.6 Deployment Guidelines.
3.3 Publications
The publication policy agreed upon during the second period by the consortia of
CareWell, BeyondSilos and SmartCare guided the publication efforts undertaken by
CareWell partners throughout the project. This section focuses on papers submitted for publication in academic journals, conference proceedings or as a book chapter by
partners of the project.
3.3.1 Publication policy
The publication policy was drafted jointly and agreed upon by the partners of all three
integrated care projects: CareWell, BeyondSilos and SmartCare. A final version was issued at the end of October. The document covers publications from within each project,
as well as publications between the projects, setting out a set of guidelines. It
establishes:
The role and responsibilities of the authors with regards to the publications.
The role and responsibilities of the Project Coordination Committees with regards
to the publication.
Guidelines for authorship.
A payment scheme for publications (in principle, no submission to be made to journals requiring a publication fee).

3.4 Project videos
The production of promotional videos has been part of CareWell’s dissemination strategy
from the beginning. In total, 18 CareWell videos have been produced (with an additional
one in production at the moment, to be disseminated after the end of the project). Video
production has usually been carried out by empirica, the Dissemination WP leader, but
additional videos have also been produced by project partners. During the first reporting period, 13 CareWell videos were produced to highlight pilot site visits and important
events in the early stages of the project. They were uploaded to YouTube in a dedicated
playlist for the project and embedded on the project website. The videos shared the
same intro and outro, using elements of CareWell’s visual identity, as well as partner logos. Videos were accompanied by the same signature musical theme.

Video production was taken up again as a task during the third reporting period following
reviewers’ official recommendations from the second project review meeting. The results
of this endeavour are reported below.
3.4.1 Integrated Care Promotion Video
One of the reviewers’ recommendations at the second project review meeting was to
reach out to an even broader public and ensure the dissemination of CareWell results
after the end of the project. To this end, the concept for a general animated
dissemination video has been drafted. The video addresses both the general public as well as healthcare professionals and policy makers, and argues for the broader
implementation of integrated care services at European level. It is grounded in the
organisational models and care pathways applied in CareWell which make use of ICTs to
improve integration and communication between professionals, as well as between
patients and professionals. It is styled as a narrative focused on Mary, an elderly multimorbid patient whose quality of life improves after she is able to access integrated
care services.
The video was finalised and published on the project website during the last week of
January 2017. The cartoon video was also published on YouTube and Twitter.

3.4.2 Videos by partners
Several CareWell regions have committed to each produce a video highlighting results of
the intervention in the respective local language. So far three videos have been
completed, and all will be embedded on the official CareWell website before the Final Conference in February.

Basque Country
Kronikgune and Osakidetza created a video showcasing CareWell objectives, the new
integrated pathway for multimorbid patients, the experiences and perceptions of
managers, clinicians and patients, and the results of CareWell's evaluation in the Basque
Country. It includes interviews with both patients and professionals involved in the project.

Veneto
Veneto is one of the pilot regions which had previously produced a video in Italian
introducing CareWell during the project’s first year. A new video highlighting the local results of the trial has been produced at the end of 2016; a version translated into
English will also follow.

Lower Silesia produced a 4:50 minute animated video in English which presents the
broader objects of CareWell, offers an outline of the state of healthcare services available
in the region prior to the intervention, and showcases the telecare and mHealth innovations implemented through the project in the region.

Puglia
Puglia produced a video wih three objectives: Provide information to wider audiences
about the regional experience for Puglia in the CareWell project; inform local stakeholders about the added value of the final CareWell project outcomes for Puglia;
and explain why Puglia was a partner in the CareWell consortium, and why it should be a
partner in other European projects.

Powys
The video was created with the aim of explaining the CareWell project, the collaborative
European approach and partnerships that have aided the project, and some of the
benefits that Powys can expect as a result of the project. The video explains the rural
challenges faced in Powys, along with the CareWell related services that have been deployed to support Powys in achieving the project objectives. It includes key local
stakeholders and feedback received from patients themselves, along with a
demonstration of the solutions in use.

3.5 Promotional material
This section reports on the promotional material produced from the beginning of the
project which was aimed for repeated use over the duration of the project.
3.5.1 Flyer
A short flyer was produced in October 2014. 2,000 copies have been printed and
distributed at various events. It was developed centrally in English, with input from all project partners. Its aim was to introduce the following information:

A pull-up banner was designed during the project’s first reporting period. It has since

3.5.3 General public presentation
A general 13 slide PowerPoint presentation in English was created at the beginning of the
second reporting period; it introduces the scope and objectives of the project, as well as the six pilots. It has been distributed to and used by all CareWell partners many times as
basic support for familiarising the public with CareWell.

3.5.5 Give-aways
For the Final Conference, 250 USB drives shaped as plastic cards have been ordered.
This shape allows for the drive to be conveniently carried in a wallet. The two sides of the
USB cards display the logo of CareWell and that of BeyondSilos respectively. Each drive will store a copy of D8.6 CareWell Deployment guidelines.

4 Reception
Monitoring CareWell’s reception has been a useful method of assessing the outreach and
effectiveness of CareWell’s dissemination strategy. The following section lists awards received by CareWell, as well as mentions in various media.
4.1 Awards and accolades
The Basque Country CareWell service has received formal recognition on six occasions in
Spain as a well as in Europe in 2016:

In the previous reporting period, two news reports on Croatian and Polish TV networks
respectively were dedicated to the beginning of local piloting operations. One news report on national Croatian TV station Nova TV covered the Zagreb pilot in February 2016, in a
01:42 minute long segment. The segment was also made available online accompanied
by a short article.

5 Synergies
Creating synergies between the three integrated care projects CareWell, BeyondSilos and
SmartCare, and, one step further, between the projects and other initiatives such as the European Innovation Partnership on Active and Healthy Ageing, has been a clear
objective for CareWell throughout its progress. This section reports on these efforts, that
include the creation of common websites, joining forces in organising events addressing
the advancement of integrated care, and actively participating in EIP on AHA initiatives.
5.1 BeyondSilos and SmartCare
5.1.1 Online synergies
As previously reported in section 3.1.3, Twitter is a space where the three projects have
routinely interacted, functioning almost like a shared platform to address a common
follower base. The development of two joint websites for the projects, which took place in
2015 (see section 3.1.2), also contributed to online synergies.
Additionally, the CareWell BeyondSilos Joint Final Conference page made public in
December 2016 is structured identically and updates simultaneously on both project
websites. Both websites will also be live streaming the conference.
5.1.2 Joint events
5.1.2.1 SmartCare Final Conference
CareWell sister project SmartCare held its final conference on 6th July in Trieste, Italy.
The conference marked the culmination SmartCare’s three years of work advancing ICT-
supported integrated care in 23 European regions. CareWell and BeyondSilos were also
present, showcasing interim results and contributing to synergies. CareWell displayed an informational poster, and was the focus of a presentation delivered by its Project
Coordinator, Esteban de Manuel Keenoy (Kronikgune): Moving ahead - Multi-level
integration for patients with complex needs.
5.1.2.2 ICIC16 Workshop
The workshop on “How to make an integrated care service sustainable” co-organised by CareWell, BeyondSilos and SmartCare at ICIC16, described in more detail in section
3.2.2.1, strengthened synergies between the three initiatives.
5.2 EIP on AHA
Over the entire duration of the project, CareWell has worked closely in collaboration with
the European Innovation Partnership on Active and Healthy Ageing, particularly with the EIP on AHA Action Group B3 on Integrated Care, by exchanging relevant materials,
making targeted contributions based on project outcomes, and participating in meetings,
conferences and open calls.
All action areas of the Action Group B3 have been addressed in CareWell. Many of the CareWell pilot regions and partners have been committed from the beginning of the
project specifically to the B3 Action Areas.
5.2.1 Call for 2016 EIP on AHA Reference Sites
CareWell participated in a particularly important event at the end of the previous
reporting period, namely, the 4th Conference of Partners of the European Partnership for Active and Healthy Ageing which took place on 9th-10th December in Brussels. Members
from all three integrated care sister projects were in attendance, and were able to join in
and contribute to discussions about the achievements of the EIP on AHA so far, and its

remaining path towards its 2020 goals, including the 2016 Call for EIP on AHA Reference
Sites.
The study on “Support to scaling up of innovations in Active and Healthy Ageing” (Scale
AHA) aims to accelerate the scaling-up of innovative approaches and practices in Active
and Healthy Ageing in Europe by fostering active knowledge exchange among regions through dedicated mentoring sessions. At the end of January 2016, ScaleAHA launched
the 2016 Call for Reference Sites.
On 14th July 2016, ScalaAHA made the results of the Call public. Five of CareWell’s
regions, Basque Country, Feltre (as part of Veneto), Apulia, Zagreb and Powys (as part of Wales), were among the 74 regional and local organisations officially awarded

highest rating in the peer review stage of the applications, followed by Veneto with three
stars.
5.2.2 European Summit on Digital Innovation for Active & Healthy
Ageing
CareWell was represented by five of its sites at the second European Summit on Digital
Innovation for Active & Healthy Ageing, which took place on 5th-8th December 2016 in
Brussels. A special award ceremony took place on 7th December, where the selected
regions, including CareWell sites, were officially awarded the "Reference Site" trophies. The work conducted by CareWell was highlighted as part of the B3 Action Group on
Integrated Care.

Conference
As agreed in the CareWell’s Description of Work, the end of the project will be marked by
a high profile final conference. The conference will be held jointly with BeyondSilos on
28th February in Barcelona, at the CosmoCaixa Science Museum in Barcelona, Spain.
The conference is officially sponsored by the "la Caixa" Banking Foundation, who have
generously provided the venue free of charge.
The date was selected to coincide with the GSMA Mobile World Congress taking place
between 27th February - 2nd March in Barcelona, with a view to attracting attendees of the congress to the Final Conference.
Registration to the conference is processed via the CareWell and BeyondSilos websites,
which will also live stream the event on dedicated pages.
