After each doctor’s office visit, you (and your covered dependents) will receive an information therapy (Ix) prescription notification (by mail or email) from MedEncentive that directs you to proven, valid, unbiased medical information on the MedEncentive website.

Flexible Spending Accounts (FSA) Dade Moeller’s FSA provides the opportunity to obtain favorable tax treatment for payment of certain health care and/or dependent daycare expenses. Full-time employees are eligible to participate on the first day of the month following the date of hire. Participation requires re-enrollment each plan year during open enrollment.

Healthcare Reimbursement Account

• Pre-tax contributions up to $2,550 per year • Use for unreimbursed medical, dental, vision and hearing expenses

Dependent Daycare Account

• Pre-tax contributions up to $5,000 per household per year • Use for daycare for eligible children up to age 13 and other disabled adult tax dependents (such

as spouse, parents and children)

This benefit is paid by the participant through pre-tax payroll deduction.

401(k) Profit Sharing Plan Full-time and scheduled part-time employees may enroll at any time. Your contributions may be supplemented by semi-annual employer matching contributions. The amount of the employer match is determined annually by the Board of Directors. New employees become eligible for the employer match after six months of employment. See the Summary Plan Description on DadeLink for more information.

Your contributions to the plan are made through the convenience of automatic payroll deductions and will occur on each paycheck. You may contribute from 1% to 100% of your pay as pre-tax and/or Roth contributions. Currently, Dade Moeller is providing a discretionary one-for-one match up to 4%.

Plan Limits

The federal government sets the dollar limits applicable to qualified retirement plans annually. The 2016 limits are:

Employee Stock Ownership Plan (ESOP) Full-time and scheduled part-time employees are automatically enrolled as of the date of their eligible employment. New employees become eligible after six months of employment. The amount of the discretionary contribution is determined annually by the Board of Directors. Group Term Life Insurance Full-time employees are automatically covered by Dade Moeller’s group term life insurance plan as of their date of hire or their change to full-time status. This benefit equals 2.5 times annual base salary up to a maximum of $332,000.

First time coverage in excess of $250,000 requires completion of an Evidence of Insurability form to determine eligibility to receive coverage greater than $250,000 up to the maximum of $332,000.

Coverage increases with annual salary actions up to the maximum. After an employee reaches age 65, coverage begins to decrease. Premiums are paid in full by Dade Moeller.

Holidays

Full-time employees (defined in the Employee Handbook) receive ten paid holidays. The eight scheduled holidays are New Year’s Day, Martin Luther King, Jr. Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Thanksgiving and Christmas. Employees are able, with prior management approval, to defer a holiday and take it at a later date within the same fiscal year.

The two remaining days (16 hours) are floating holidays and may be taken when convenient for employees. The floating holidays are immediately available at the beginning of the fiscal year and can be taken at any time within the fiscal year (with prior management approval). All holiday time may be taken in as little as ½-hour increments subject to a 1-hour minimum (see the Time Charging Policy for more on this).

Paid Sick Leave

Dade Moeller complies with sick leave laws in states such as California, Oregon, and Massachusetts and cities such as the District of Columbia, Philadelphia, and Seattle. If you are not a full-time employee eligible for Paid Time Off (PTO) and live in a city or state with its own paid sick leave law, you may be eligible to accrue sick leave per the specific city or state regulations. Paid Time Off (PTO)

Immediate accrual begins for full-time employees according to the schedule below. PTO is intended to be used for any time off (other than holidays) for which the employee wishes to be paid, including, but not limited to, vacation, personal time off, sick leave, doctor appointments, jury duty, etc.

• Less than 6 months of service: 3.25 hours per pay period • After 6 months of service: 6.5 hours per pay period • After 5 years of service: 7.5 hours per pay period • After 10 years of service: 9.0 hours per pay period

Maximum carryover is 160 hours (4 weeks) at fiscal year end. Employees with carryover balances may elect to reduce their PTO accrual and receive the cash equivalent (less applicable deductions and withholding). Accrual reductions may be elected in 40 hour blocks, must be matched by an equivalent or greater balance as of December 1st of the election year, and cannot exceed 120 hours. Dade Moeller’s objective is to maintain the basic PTO benefit while providing an option for managing PTO balances.

PTO (continued)

Irrevocable elections must be made during the annual benefits open enrollment period. Lump sum payments, subject to applicable deductions and withholding at the supplemental wage rate, will be paid on or before January 31st. The employee will not accrue additional PTO in the new year until the elected reduction has been reached (e.g., 40, 80 or 120 hours), at which point the accrual will resume at the employee’s regular rate.

For example:

1. An employee with a December 1st balance of 62 hours may reduce their following year accrual by 40 hours and receive the cash equivalent. That employee will accrue an additional 129 hours during the following year (or more depending on length of service), for a total of 191 hours.

2. An employee with a December 1st balance of 120 hours may reduce their following year accrual by 120 hours and receive the cash equivalent. That employee will accrue an additional 49 hours during the following year (or more depending on length of service), for a total of 169 hours.

Carryover balances are limited to a maximum of 160 hours, on a use it or lose it basis. The open enrollment election described above does not alter the carryover limit.

If an employee has a break in service of less than 5 calendar years, they will begin PTO accrual at the same level as achieved prior to the break in service when rehired or returning from unpaid leave. An employee will incur a break in service when less than 500 hours of work are recorded during any calendar year.

Spousal Medical Subsidy

Dade Moeller provides a subsidy to employees if their spouse obtains insurance through federal or state insurance marketplaces, Medicare/Medicaid, or directly from an insurance company or broker. This subsidy is only available to employees who participate in a Dade Moeller medical plan and have a spouse that does not have access to their own insurance through another group plan. This benefit becomes available after six months of employment. A Spouse Affidavit must be signed before receiving this benefit. The subsidy will equal 60% of the monthly premium for the spouse’s plan up to $285 per month. The subsidy will be distributed through payroll and taken out pre-tax to be submitted to a Portable Health Account (PHA), managed by LyfeBank, a third party administrator.

LyfeBank charges a monthly flat service fee of $10 per employee to maintain the PHA account. With the pre-tax savings, this monthly fee will be less than if the payroll tax rate was paid on the subsidy received. In addition, employees have the option to have the balance of their spouse’s premium deducted from their paycheck pre-tax and the total paid directly to the insurance company each month.

Spousal Medical Subsidy (continued)

How does the LyfeBank process work? An eligible employee provides Dade Moeller with a copy of the monthly premium invoice for the plan selected by the spouse to determine the amount of the credit. The employee will setup an online LyfeBank account. Dade Moeller will make monthly deposits of the subsidy (and premium payroll deductions, if elected) into the PHA account. From here there are two options:

1. The employee submits monthly proof of premium payment to LyfeBank to be reimbursed. 2. If the payroll premium deduction has been elected, LyfeBank can make payment directly to the

insurance provider. The full premium plus the $10 monthly fee is required to be in your PHA. You must direct your spouse’s insurance company to submit the monthly invoice directly to LyfeBank to take advantage of this option. Simply setup the mailing address for invoicing to: PO Box 697, La Conner, WA 98257.

Tuition Assistance

Reimbursement of tuition and certain fees may be available for academic college study. The course of study must align with Dade Moeller’s mission. Course work must be applicable to an employee’s current or anticipated job responsibility. Reimbursement is subject to available funds and pre-approval. Please see the Employee Handbook for more information.

Veterinary Pet Insurance (VPI)

With Veterinary Pet Insurance, you can purchase dog, cat or exotic pet insurance plans at a group discount rate.
