ADVANCING RACIAL EQUITY TRAININGS SERVICE DESCRIPTION: The Center for Health Equity developed a racial equity training curriculum and is in the process of training all 6000+ LMG employees. CANCER SCREENINGSKENTUCKY WOMEN’S CANCER SCREENINGSERVICE DESCRIPTION: Provides breast and cervical cancer screenings thru Family Health Centers. We also partner with Norton Center for Prevention and Wellness for breast and cervical screenings and UofL Brown Cancer Center for breast screenings and diagnostics. The screenings are on mobile units with various hours. BABY AND ME ARE SMOKE FREESERVICE DESCRIPTION: Pregnant or post-partum (after delivery) women, regardless of income, can receive information on stopping tobacco use during and after pregnancy. Participants may receive services in their homes or other convenient locations. MATERNAL CHILD HEALTH
HEALTHY STARTSERVICE DESCRIPTION: Healthy start helps women plan for and have healthy pregnancies, deliver full-term babies, and raise healthy toddlers through home visits and coaching. Professional service providers can help women make reproductive life plans, make sure that prenatal care begins during the first three months of pregnancy, and have the skills they need to raise healthy, happy babies. Families with children up to two years can participate in home visits, telephone coaching, and parenting classes to improve skills and understanding of childhood development. Incentives such as Pack n’ Play Cribs, car seats, strollers, and diapers are offered for successful participation. HANDS*SERVICE DESCRIPTION: The Health Access Nurturing Development Services (HANDS) program is a voluntary home visitation program for new or expectant parents. HANDS support families as they build healthy, safe environments for the optimal growth and development of children. HANDS staff members provide support, information, and child development education from pregnancy through two years of age. Services can begin during pregnancy or anytime up until a child turns 3 months old.ELIGIBILITY: Any parent expecting a new baby and residing in Kentucky is eligible. SERVICE DESCRIPTION: Mothers who participate in WIC are encouraged to breastfeed their children through the following services: • At a WIC appointment, a nutritionist will provide
encouragement, coaching, counseling and education. Peer counselors will provide follow-up support through telephone calls or home visits to check in, offer tips, answer questions, coach, and troubleshoot.• Prenatal and postpartum mothers can participate in
breastfeeding classes to learn the basics and benefits of breastfeeding or Mothers Milk Club support groups that meet twice monthly.• Breastfeeding mothers are eligible to participate in WIC

FREEDOM FROM SMOKING (FFS) - STOP SMOKING CLASSESSERVICE DESCRIPTION: The American Lung Association’s FFS program helps people stop smoking through small group support classes. It does not focus on scare tactics or statistical information on the health effects of smoking. Most people know smoking is dangerous and plan to quit in the future. Instead, we will talk about health as a motivation to quit and focus on how to quit and stay quit. Phones are staffed by counselors trained to help smokers quit. so participants can buy nutritious foods such as cereals,milk, cheese, peanut butter, fruit, and vegetables. WICalso provides infant formula.
• Nutrition education: WIC provides health and nutritioneducation classes to help people understand theirnutrition needs and learn about health prevention andimprovement strategies.
• Referrals to healthcare and other social services: WICoffers referrals to other important services such asmental health, immunizations, and drug and alcoholtreatment programs.
• Breastfeeding support: WIC offers guidance and supportfrom a Certified Lactation Specialist and breastfeedingpeer counselors about the benefits of breastfeeding andproper breastfeeding techniques.
ELIGIBILITY: Applicants must reside in Jefferson County, be pregnant, postpartum, or breastfeeding women, infants, or children up to the age of 5 years. Applicants’ gross income (before taxes) must fall at or below 185 percent of the U.S. Poverty Income Guidelines. Applicants will have a nutritional risk assessment by a qualified health professional to determine final eligibility. SENIOR MEDICARE PATROLSERVICE DESCRIPTION: Senior Medicare Patrol (SMP) helps Medicare beneficiaries, their families and caregivers who have questions about billing issues or believe they may be a victim of healthcare fraud or scams. We act as a source of information and support for the self-management of those already diagnosed with diabetes to help them avoid complication and lead healthy lives. The program uses multiple avenues to achieve its goals including partnership building between area stakeholders, outreach, and public education programs. MOSQUITO CONTROLSERVICE DESCRIPTION: Conducts mosquito population surveillance, inspections, and investigates complaints of reported mosquito problem areas. Provides mosquito control education. OFFICE OF EMERGENCY AND PUBLIC HEALTH PREPAREDNESSSERVICE DESCRIPTION: The mission of the Office of Emergency Public Health Preparedness (OEPHP) is to plan and prepare for; coordinate the response to; and recover from public health threats or emergencies in coordination with local, state, and federal agencies. This may include planning how to staff and supply emergency shelters during a flood, or how to deliver life-saving medications after a bioterrorism attack. We are dedicated to preparing and leading the public health response to disasters and emergencies that affect Louisville Metro families, first responders, healthcare facilities, and our community as a whole. We are available to coordinate resources, and assist in developing and implementing emergency operations plans for Louisville Metro healthcare facilities, alongside our regional healthcare coalition, HERA. We also regularly provide public health emergency preparedness training and education to various groups throughout the community. MEDICAL RESERVE CORPSSERVICE DESCRIPTION: The Medical Reserve Corps (MRC) is a national network of both medical and non-medical volunteers, organized into local units to improve the health and safety of their communities during public health emergencies. Our local Louisville Metro MRC unit provides the greater Louisville community with volunteers who can assist emergency management and health professionals during a large-scale local disaster, but also serves to support public health preparedness initiatives.
