Qualification Document Checklist of Items Required to be submitted. One (1) original, one (1) electronic single PDF version on a CD or Flash Drive of the original RFQ in its entirety

A Certificate of Insurability, acceptable to the County, shall accompany each Qualification or alternate qualification, in the amounts as prescribed by State and Sumter County Board of County Commissioners.

Florida Hospital Rollins Campus, Orlando, Florida

Our initial task for this project was to provide TopographicSurvey of select sites throughout the campus for engineeringsupport. This task was expanded to eventually include over100 acres including the main campus. DRMP prepared

boundary surveys of six sites including the Main Campus, Alden Garage, McRae Garage, Medical OfficeBuilding, Translational Research Institute and Florida Hospital Reference Lab, as well as right of waysurveys for Rollins Street andMcRaeAvenue. Preparedmultiple Sketch andDescriptions for easementsand right ofway acquisition throughout the project site. Subsurface utilitiesweremarked by designatecrewsat specified locations through theprojectasdirectedby thedesignengineers.Weset theprimarycontrolusedfortheconstructionoftheGinsburgtowerandtheAldenandMcRaeParkingstructuresandperformedConstructionlayoutandAs‐builtsurveysforconstructionoftheMedicalOfficeBuildingandtheTranslationalResearchInstitute(TRI).

Department of Military Affairs Continuing Services Contract, Statewide, Florida

DRMPwasselectedbyTheDepartmentofMilitary Affairs to provide SurveyingServices on a Continuing Survey andMapping services Contract which consistof but not limited to boundary survey,property survey, topographic survey,wetland survey, tree survey, utilitysurvey, As‐Builts plans, geotechnicalsurveyandlegaldescriptionsonprojectsbeingadministeredbytheDepartmentofMilitaryAffairs.DRMPwas also contracted with C.G. Celio & Sons to perform Boundary and Topographic Surveys for FloridaNationalGuardsitesthroughouttheStateofFlorida.Theprojecttasksforthiseffortincludedestablishingthe project sites on UTM 17N projection, WGS84 Datum utilizing Global Positioning System (GPS) andcompilingtheboundary,easements/leaseagreements,andtopographicdataincludingsubsurfaceutilitiesintoageo‐referencedGISdata‐baseinventoryutilizingSpatialDataStandardsforFacilitiesInfrastructure,andEnvironment(SDSFIE)standards.Wehavecompleted10ofthe12sitescontracted.

B. Mapping and Surveying Team Overview TheProjectTeamwillbeledfromstarttofinishbyAllenL.Quickel,PSMwhohasbeenwithDRMPforover12years.Mr.QuickelwillbetheprincipalpointofcontactbetweenDRMPandSumterCounty.AsaSenior Project Manager he is responsible formanaging continuing contracts, committing firmresourcesandassuringclientsatisfaction.Hebringsthe experience, commitment, and DRMP historyneededinordertoeffectivelymanagethePurchaseOrdersthatwillberequiredbythiscontract.Hewillbe supported by a talented group of ProjectSurveyors representing a variety of surveying andmapping areas. Allen L. Quickel, PSM Project Manager

Allen L. Quickel, PSM Mr. Quickel is a Project Manager in DRMP’s Surveying and Mapping Department specializing in Transportation, Geodetic Control, Land Acquisition Surveying Services, and 3D Laser Scanning. He is currently responsible for client coordination, field crew assignments, office production, and quality control on a variety of projects ranging from private development design and as-built surveys, platting, municipal services, and roadway design survey and right-of-way mapping projects for local municipalities and FDOT Districts. Mr. Quickel also leads DRMP’s 3D Laser Scanning service line, using FARO Focus 3D scanners, and Certainty 3D TopoDOT software to produce point clouds, engineering design survey drawings, and 3D models of existing structures. Mr. Quickel draws on his field survey and office production experience to lead field crews and office technicians on a wide range of projects utilizing Trimble Access and EFB data collection and processing software, and CAICE, Geopak, and Autodesk Civil 3D software. He is proficient in AutoCAD and Microstation as a production drafter and survey project technician, is experienced with ESRI ArcView and ArcMap GIS software, and is proficient with FARO Scene and Certainty 3D TopoDOT software to produce 3D design surveys.

RELEVANT PROJECT EXPERIENCE Wendy Lee Road Sidewalk, Brevard County Public Works, Titusville, Florida: Survey Project Manager responsible for topographic and right of way surveys supporting the design and construction of a new sidewalk along Wendy Lee Drive between Sisson Road and Imperial Estates Elementary School. North Banana River Drive Pedestrian Improvements, Brevard County Public Works, Merritt Island, Florida: Survey Project Manager responsible for topographic and right of way surveys supporting the design and construction of new sidewalks and improvements to the existing bridge over a canal providing access to the Banana River for Merritt Island residents. Continuing On Call Engineering and Surveying Services, City of Deltona, Volusia County, Florida: Project Surveyor responsible for providing quality control of surveying and mapping services in support of City engineering projects related to site development, public park design/permitting requirements and minor road/intersection design. Thornby Park Stakeout, Volusia County, Florida: Project Surveyor responsible for providing

quality control of survey services to correctly establish the east line of the park along Providence Boulevard and the wetland buffer line in order to meet required setbacks to stake out the proposed Gazebo for construction.

Dewey Boster Sports Complex, Volusia County, Florida: Project Surveyor responsible for providing quality control of survey services for construction of a new maintenance building on the western edge of the sports complex. Surveys included: topographic of the construction site and SUE using electromagnetic (EM) and ground penetrating radar (GPR) to designate and locate the waterline running east and west across the property in support of engineering for the project.

Continuing Surveying Services, Florida Hospital, Orange, Osceola, Seminole, and Volusia Counties, Florida: Project Surveyor responsible for providing an array of surveying and mapping services on numerous Florida Hospital sites. Survey services include: boundary, topographic and sectional work, along with record plats, sketches of description and legal descriptions, right of way taking and abandonment exhibits and sketches, design support services, and condominium documents for medical offices. St. Johns Heritage Parkway, Brevard County, Florida: Project Surveyor responsible 5-miles of design survey and mapping for right-of-way acquisition for the design of a new 4-lane roadway alignment east of I-95. The project started at the City of Palm Bay's northern limits and runs north for approximately 5 miles before turning east to cross I-95 and ending at the existing John Rodes Boulevard and Ellis Road intersection. One of the largest parcels acquired for the parkway came to be known as the Platt Ranch property, which made up approximately a quarter of the project. The roadway right of way was acquired as fee parcels in several sections by the County. Responsible for a boundary survey for acquisition of the required property, including identifying different parcels as right of way, permanent pond, drainage, slope, or sidewalk easements, or temporary construction easements. Secondary Canal Inventory, Orange County, Florida: Project Surveyor for a canal inventory project utilizing sub-meter mapping grade GPS to locate existing drainage canals and inflow and outflow structures for this project. GPS data was linked to digital photos of structures and canal conditions, using intermediate software. The GPS data and linked images were then converted to ESRI Shapefiles to be incorporated into the Orange County GIS. SR 5 (US 1) and New Haven Blvd. from South of Roosevelt Ave. to South of Crane Creek Bridge, FDOT District Five, Brevard County, Florida: Survey Project manager and Subsurface Utility Engineering Manager providing for the coordination and management of ASCE 38.02 Quality Level “A” Subsurface Utility Engineering Services to designate utilities using Ground Penetrating Radar (GPR) and Electromagnetic (EM) designation equipment and assisting in the location of utilities by vacuum excavation at Test Holes at specific conflict locations to verify exact locations of underground utilities. Mast arm locations will also be cleared at 3 locations. Project limits are on State Road 5 (US1) from south of Roosevelt Ave. to South of Crane Creek Bridge, and on New Haven Ave. from SR 5 (US1) to SR 500. The project was designed as an ultimate four-lane divided roadway, with initial construction of the two northbound lanes. Professional Engineering Services, Town of Belleair, Pinellas County, Florida: Project Surveyor responsible for providing surveying and subsurface utility engineering services in support of civil engineering design on an as needed basis in support of the Town’s public works projects. Osceola Road/Varona Street, Town of Belleair, Pinellas County, Florida: Project Surveyor

The design survey included vertical control, aerial targets for L.A.M.P., SUE designating and locating, drainage survey, bridge survey of two bridges and jurisdictional line survey. The Right-of-Way Survey consisted of the retracement of the existing 1974 Right-of-Way Map of SR 93 (I-75) Section 13075-2403 and 2004 and 2006 Right-of-Way Maps of SR 64 Section 13050-000 alignment and references. The project alignment was created from two separate existing right-of-way control surveys, two separate existing right-of-way maps and one existing right-of-way monumentation map. SR 55 (US 19) from NE 1st Terrance to Cornflower Drive, FDOT District Seven, Citrus County, Florida: Project Surveyor for a right-of-way monumentation survey and the preparation of a right-of-way map and a right-of-way monumentation map. This project covered an eight mile section of US 19 starting in the City of Crystal River and running north in Citrus County. The right-of-way map was created to reflect the compilation of five separate existing right-of-way maps to cover a 70-year span of time. SR 35 Right-of-Way Monumentation, FDOT District Five, Sumter County Florida: Served as the Project Surveyor for the monumentation of the right of way for three map sections of SR 35 (US 301). The right of way sections extended along US 301 from south of Wildwood, Florida, to the Sumter/Marion County line, approximately 6.6 miles. This project consisted of recovering and resetting all centerline and offset baseline stations and references shown on the original maps. Survey data analysis verified the mapped alignment was in agreement with the field monuments. Following District Five’s approval of the alignment, the right of way was set for the length of the project. Right of way monumentation was displayed in a set of the original FDOT Microstation DGN files as a guide for District Five mapping staff that would prepare the final drafted maps. SR 483 (Clyde Morris Blvd), FDOT District Five, Volusia County, Florida: Survey Technician for this 3.4 mile roadway project, south of Beville Road (SR 400) to north of International Speedway Boulevard (SR 600). The project involved widening an existing undivided 4-lane roadway into a 6-lane roadway with a 12-foot multi-use path.

The project started at the City of Palm Bay's northern limits and runs north for approximately 5 miles before turning east to cross I-95 and ending at the existing John Rodes Boulevard and Ellis Road intersection. The project was designed as an ultimate four-lane divided roadway, with initial construction of the two northbound lanes. SR 25 Monumentation Maps, FDOT District One, Polk County, Florida: Survey technician responsible for preparing nine map sets for over 23 miles of SR 25 from SR 60 in Lake Wales to I-4. Centerline points and reference points were field located and the centerline alignments were reestablished using the found monumentation based on the maps. Alignments were submitted to the District for approval then monumented where not found. Over 2700 points were calculated, found or set for this project. RELEVANT PROJECT EXPERIENCE Howell Creek Drainage Study, Seminole County, Florida: Served as Project Surveyor for this Seminole County drainage study for the Howell Creek Drainage Basin and is the largest in the county. DRMP provided Survey and Mapping services for the purpose of inventorying all drainage pipes, culverts, outfall structures, and bridges within the basin. In addition, DRMP provided structure and bridge details along with creek/canal cross-sections and the detailing of roadway drainage systems, all of which was tied to the Seminole County Vertical Control System. Town of Edgewater Drainage Study, Edgewater Florida: Served as Project Surveyor for this city wide drainage study consisting of the establishment of a city wide vertical control network, placement of staff gauges, drainage structure details and cross-sections of various waterways such as creeks, canals and retention areas. The survey provided an inventory of the existing drainage facilities and determined their effectiveness for particular design stormwater flooding events. The study covered 21 miles of canals and nearly 200 culverts. The report outlined recommendations to alleviate flooding problems and set design standards for future development. Little Wekiva River Drainage Study, Orange County, Florida: Served as the Project Surveyor for this drainage study of the Little Wekiva River Basin consists of 49 square miles located in Orange and Seminole Counties with the Seminole County study area encompassing 29 square miles. DRMP performed an extensive Survey and Mapping effort for the basin which included over 250 structure details, channel cross-sections, bridge details, roadway drainage systems, and the establishment of vertical control throughout the entire river basin. This project consisted of data collection along neighborhood streets, and between two existing ponds to create a project Digital Terrain Model and survey base files. 3D laser scanning was used on this project to quickly collect data so that engineering design and drainage modeling could be completed on a tight schedule.

Hartley Road Subsurface Utility Mapping, Jacksonville, Duval County, Florida: Served as Senior Project manager and field supervisor for this 1.5 mile widening project under the Better Jacksonville Plan. The subsurface utility tasks for this project consisted of utility designation and approximately 250 subsurface utility reports along with 40 exploratory trenches along the corridor to identify any unforeseen design conflicts. Chaffee Road and Cahoon Road, FDOT District Two, Duval County, Florida: Project surveyor for construction layout and right of way staking for utility relocation project. Tasks included staking of proposed new roadway, guardrail, drainage structures and right of way lines for the installation of underground electric and communication lines. Palm Coast Parkway, City of Palm Coast, Florida: Served as the Survey & Mapping Project Manager for the widening of this four-lane suburban section to a six-lane urban facility for the City of Palm Coast. The project began 500 feet west of Cypress Point Parkway/Boulder Drive and extends to Florida Park Drive. Survey tasks for this project included horizontal and vertical control for design surveys, roadway topography, bridge details; we also implemented the use of Low Altitude Mapping Photography (LAMP) “Remote Sensing” due to high traffic volumes for the safety of our field staff safety and motoring public while collecting roadway data. DRMP also provided Right-of-Way Control Surveys, and Subsurface Utility Engineering (SUE) services for the designation and location of subsurface utility conflicts. The right-of-way mapping services included legal descriptions and title search reviews, and TCEs to support the right-of-way acquisition for this project.

West King Street, St. Johns County, Florida: Served as the Survey Project Manager for various portions of this ongoing project for St Johns County and supervised various elements of this project consisting of GPS network establishment, right of way control surveys, Maintenance Maps for West King Street and St Johns Street, Right of way maps, Parcel Sketches and legal’s to support the county’s Real Estate Division. Survey activities to support the engineering design included topographic / DTM surveys, underground utility location including subsurface utility engineering (SUE), Jurisdictional surveys. Professional services are ongoing for this project. St. Johns Street and Duval Street Improvement Project, St. Johns County, Florida: Project Manager this neighborhood improvement project for St Johns County including horizontal and vertical control survey, right of way determination and design surveys. Other various tasks also included boundary surveys, wetland survey geotechnical support services. Professional services were completed in 2010. CR 305 Extension, St. Johns County, Florida: Mr. Lusink served as the Project Manager in charge of all the Survey and Mapping activities for this St Johns County 4.5 mile new roadway corridor extension of CR 305. The survey effort consisted of Design, Wetland and utility surveys to support the engineering design team. Right of way Control Survey was also preformed to support the Right of way mapping effort which was also provided by DRMP. (Cost $220,000). SR A1A, Vilano Beach, Florida, Scour Analysis Survey, St. Johns County, Florida: Served as Project Manager for topographic/digital terrain model (DTM) survey and bathymetric/beach profiles surveys at an eroded scarp area along SR A1A in Vilano Beach, Florida. This effort supported the design and construction of a bulkhead along the easterly right of way of SR A1A to protect the roadway and utility infrastructure. ACA was contracted  to secure new aerial  imagery and produce color and black and white digital  imagery of Miami‐Dade Airports within the Miami‐Dade  Airport  system.  These  airports  included Miami  International,  Opa Locka, Tamiami and Homestead General Dade Collier, and Opa Locka West locations. Updated  imagery  is  the primary  tool used  for general planning for upcoming MDAD airport projects. ACA also provided all survey related activities.  Each  task  order  project was  extremely  time  sensitive  and met within budget and on time.  2010 Martin County Six Inch Imagery Orthophotography Project Martin County Board of County Commissioners 

ACA was tasked to provide digital ortho imagery for 261.1 square miles within Martin County, Florida. Digital imagery was secured utilizing our UltraCamXX  large  format  digital  sensor.  The  sensor  collected  aerial imagery at a 0.47 foot (5.7  inch) pixel resolution. ACA provided the six inch  pixel  resolution  ortho  imagery  in  true  color  and  color  infra‐red bands.  TBW Digital Imagery and Remote Sensing Project; Tampa Bay Water. The project  consists  of  1563  square miles,  including  the  City  of  Tampa.  ACA utilized our Vexcel UltraCamX high resolution digital sensor to secure new digital  photography  in  both  the  Spring  and  the  Fall  of  each  year. ACA  is providing  8  bit  (24  bit)  FCIR  pan‐sharpened  18  inch  pixel  ortho  photo mosaic  in ECW  format and an 8 bit  (24 bit) RGB  color pan‐sharpened 18 inch pixel ortho photo mosaic  in ECW  format. All  image data will be used for wetland condition assessment.

ACA was selected by the City of Ocala to update their existing map  files of the 86 square mile service area.  After consulting with the City Engineers and Surveyors, ACA reviewed the City’s existing data and formulated a cost  effective  approach  to update  and  correct  the City’s database.   Additionally, ACA  secured digital  color orthophotography for the City service area.  Final delivery included 344 ortho corrected black & white digital images at 1’ + 100’ target scale.  Alachua County Countywide 30 cm GSD Ortho Imagery Update Alachua County Property Appraiser's Office  

ACA was  awarded  this  high  resolution  ortho  imagery  project  by  the  Alachua  County  Property  Appraiser's office, monitored by the US Geological Survey Department. Collection of the ortho imagery was funded by the American Recovery and Reinvestment Act of 2009 program. This project area was 1242 miles and covered the area with a minimum of 300 meter buffer on all sides.  Imagery was collected using UltraCamX at 30 cm pixel resolution.    Osceola County Digital Orthophotography Project Florida Department of Revenue 

ACA  secured  color  digital  orthophotography  for  Osceola  County,  Florida  through  our  contract  with  the Department of Revenue. The purpose of this project was to provide orthophotos for a 2,000 square mile area. Direct  geo‐referencing  through  ABGPS  and  IMU  data  produced  camera  positions  and  rotation  information. Imagery was mosaicked and geo‐referenced using field survey control and ABGPS IMU data.   Okefenokee National Wildlife Refuge and all of Charlton & Ware Counties  University of West Georgia  

This project consisted of 1684 square miles in Charlton and Ware Counties, Georgia. ACA collected static base station data  to  support Airborne Global Positioning  (ABGPS)  and  aerial  targeting  survey  control, processed ABGPS/IMU data to produce a Smoothed Best estimate of Trajectory to support the generation of LiDAR mass point data, acquired & delivered unclassified LiDAR, digital aerial imagery utilizing our UCX at a 0.5‐foot pixel resolution. Each task was time sensitive and was met within budget and on time.    Jefferson Digital Ortho and Oblique Imagery Project Jefferson County Appraisal District ACA was selected by Jefferson County Appraisal District to provide digital ortho imagery for their Countywide Program. The County encompasses 1155 square miles of which 715 square miles required a six inch pixel resolution and the remainder of the County required a 12 inch pixel resolution. Digital true color image tiles were provided in GeoTIFF and Mr.Sid format with corresponding metadata. The deliverables met all FDGC requirements.

ForatypicalTaskAuthorization,thetimelinewouldbeginwiththeCountynotifyingDRMPinarequestfor proposal. This can be as simple as an email ortelephone call. The DRMP Survey projectmanagerwill immediately contact the appropriate County

projectmanager to discuss specific project details,in order to prepare an accurate scope and feeestimate, including a proposed schedule forcompletion. Quality Assurance & Quality Control (QA/QC) is avital part of any project deliverable. DRMP takesQA/QC seriously and delivers high‐value finishedproducts. QA/QC does not begin at the end of theproject, just prior to delivery. QA/QC starts at theproposal stage, when the DRMP project managercontacts the County to discuss project needs andexpectations. These expectations are lined‐out forthe project surveyor assigned to the TaskAuthorization. Field data and field notes arechecked daily. ThistechnologyallowsDRMPsurveyorstogatheralargeamountofpreciseprojectdatapoints,apointcloud,ina shortperiodof time.Thisdata canbeused tomodel surfaces for a variety of uses and easilyconverted to standard design platforms such asAutoCAD or MicroStation resulting in detailedsurvey as‐built information. The equipment alsoallowsforminimizingmanpower,whicheliminatesmany health and safety issues expected withtraditionalsurveyfieldwork.

Geographic Information System (GIS) DRMP offersgeographicinformation system(GIS)services tomeetthe needs of a varietyof clients. Bymaintaining the latestreleases ofEnvironmentalSystemsResearch Institute’s (ESRI)ArcGIS software package, DRMP maintains theabilitytoprovideafullrangeofGISservices.Someexamples include cartographic design, spatial andthree dimensional mapping and analysis, LiDARprocessing and digital terrain modeling,geodatabasedesign,aerialphotointerpretationandrectification,andmanymore.

DRMP has been contracting with the FloridaDepartmentofMilitayAffairs toprovide surveyingandGISmappingofvarioussitesacrossFlorida,andpreviously worked with Pasco County on a pilotprogram to update a portion of their CountyProperty Appraiser online GIS of propertyinformation.Bothof theseprojects involvedDRMPmigrating information from Autodesk files (LandDesktop for Pasco County and Civil 3D forDepartment of Military Affaires) into specificallyformatted databases. suitable for use on the existing County PropertyAppraiserwebsite.

Construction Layout and Construction Support Survey Services DRMP’s Surveyserviceswillhelpwithevery aspect of theconstruction processwhether its controlsurveys, constructionlayout, right‐of‐waydetermination,quantity surveys, as‐built surveys, temporaryroadway easements or other specific purposesurveys. With our survey personnel’s previousconstruction survey experience we can assist toresolveanyconflictduringtheconstructionphase.

Our staff is fully capable of coordinating withCounty staff members and the project contractorsto perform incidental engineering surveys asnecessarytoverifyandconfirmtheaccuracyoftheconstruction stakingprovidedby the contractor. Ifwe are called upon to provide these services wewould set up a tailgatemeeting at the project sitewith all parties involved to discuss the items ofinterest and the procedures used by thecontractor’s surveyor. Harmony with Property Owners and Public We fully understand the importance for our fieldand office employees to perform their dutieswithprofessional decorum so as to represent SumterCountywell. DRMP field surveyors and field crewmemberswill wear uniforms, and all vehicleswillbe marked with the company logo and contactinformation. can be made to continue survey activities underamiablecircumstances.

DRMP has extensive experience working onprojects which put us in contact with multipleproperties and property owners of varyingpersonalities.Fromtimetotime,surveyorswillruninto a property owner who is unwilling to allowaccess to his/her property. We instruct our fieldsurveyors tonot engage in argumentative, rudeorconfrontational behavior or communication withproperty ownerswho are upsetwith the presenceof surveyors on their property. We realize thattaking a rigid approach with a land owner whiletrying to defend our right “to enter lands of thirdparties” (Fla. Statute 472.029) will not result in apositive outcome for our client, the project or ourfield survey crews. Brief Description of Project: Our initial task for this project was to provide Topographic Survey of select sites throughout the campus for engineering support. This task was expanded to eventually include over 100 acres including the main campus. DRMP prepared boundary surveys of six sites including the Main Campus, Alden Garage, McRae Garage, Medical Office Building, Translational Research Institute and Florida Hospital Reference Lab, as well as right of way surveys for Rollins Street and McRae Avenue. Prepared multiple Sketch and Descriptions for easements and right of way acquisition throughout the project site. Subsurface utilities were marked by designate crews at specified locations through the project as directed by the design engineers. We set the primary control used for the construction of the Ginsburg tower and the Alden and McRae Parking structures and performed Construction layout and As-built surveys for construction of the Medical Office Building and the Translational Research Institute (TRI).

HOLD HARMLESS AGREEMENT

The Contractor/Vendor is required to purchase and maintain minimum limits of $1,000,000 per occurrence for all liability, which includes general liability and, if applicable, automobile liability. Other coverage may be required where applicable.

The Contractor/Vendor agrees to hold the Board of Sumter County Commissioners harmless against all claims for bodily injury, sickness, disease, death or personal injury or damage to property or loss of use resulting there from, arising out of the agreement, unless such claims are a result of the County’s sole negligence.

The Contractor/Vendor shall purchase and maintain its own Worker’s Compensation Insurance and Employers Liability in accordance with Florida Statute Chapter 440.

The Contractor/Vendor shall also purchase any other coverage required by law for the benefit of employees.
