MSBA or Agreement. Current statistics shows that approximately 20% of total
invalids for FY07 were contributed by Missing/Inconsistent Contractual Information.

What is the role of the Signature Form?
The signature form replaces the signature block on individual documents and
consolidates the signature of all documents associated with a customer‟s program.
Each program type (e.g. EA, EAS) requires a separate signature form.
When is a Signature Form required?
The Signature Form is required for signing an agreement, enrollment, and some
optional forms.
Note: the Signature Form may be used for tracking/recording documents associated with a single deal
but not all optional forms require a signature form.

Overall Guiding Principles to completing
Signature Form
One signature form per program per customer entity. Cannot use same signature
form for both a Select and an Enterprise program.
Any document with a document number must have a customer signature through
the signature form.
Supporting documents such as media form, supplemental contact information form
etc when listed on the signature form, must be attached and submitted to Microsoft.
The MBSA and Master agreements need not be sent to Microsoft. Microsoft
already stores copies based on the document number. However, a Volume
Forecast Form must be submitted if a customer enters a new Select Master
agreement.
The first four rows of the left column of the signature form table are pull down
menus. Documents not listed in the pull-down menu are to be listed starting in the
fifth (5th) row.

(MBSA)
What is the role of the MBSA?
It sets out the high level terms and conditions that apply to all Microsoft volume
licensing agreements and enrollments entered into under it. It also sets out the
core terms that apply to Services from Microsoft.
An active MBA or MBSA must be in place for your customer or one of its global
affiliates in order to sign a volume licensing agreement or enrollment.
It is an “evergreen” agreement. This means that it does not have a fixed term or
expiration date.
When is an MBSA required?
The MBSA is required for new customers when they or one of their affiliates does
not have an active MBA or MBSA in place already.
Customers with an existing MBA may sign a new MBSA to have the latest terms
apply. This is optional; however, new agreements and enrollments will also be
necessary to associate the EA or EAS program agreements with the new MBSA.

(EA Agreement)
What is the role of the EA/EAS Agreement?
The EA/EAS Agreement sets out the core terms and conditions that apply
to volume licensing purchases under the EA or EAS program.
An active EA/EAS Agreement must be in place for your customer or one
of its global affiliates in order to enroll and place orders.
When is an EA/EAS Agreement required?
The EA/EAS Agreement is required for new customers when they or one
of their affiliates does not have an active EA/EAS Agreement in place
already.
A customer may opt to enter into a new EA/EAS Agreement at any time to
benefit from the expanded product offerings. This is not mandatory.
An EA/EAS Agreement is perpetual, meaning it does not expire.

What is the role of the EA/EAS Enrollment?
An Enrollment allows a customer to order and purchase Microsoft licenses under an active EA/EAS Agreement.
An active EA or EAS Agreement must be in place for your customer or one of its global affiliates in order to sign an Enrollment.
When is an EA/EAS Enrollment required?
An enrollment is required for all new customers:
A new customer submitting a new Agreement
A renewing customer submitting a new Agreement
An enrollment may be required for renewing customers:
An enrollment is required if a customer elects to change the configuration of the previous enrollment at the end of the enrollment term.
An enrollment is required at the end of a customer‟s renewal term.

What is the role of the Renewal Form?
The Renewal Form documents a customer‟s decision to renew the same configuration of the enrollment for an additional 12 or 36 months.
The Renewal Form must be submitted to Microsoft along with the customer‟s renewal order.
A new enrollment is required if the customer has already completed a 12 or 36 renewal month term.
When is an EA/EAS Renewal Form required?
The Renewal Form is required at the end of the customer‟s initial enrollment term to renew its enrollment without change. The form documents the customer‟s choice to continue its enrollment for another term.
A customer may also opt to renew by submitting a new EA/EAS Enrollment. In this case, the Renewal Form is not required for renewal.
Optional Forms
Attachments to the EA/EAS Enrollment:
Enterprise or Enterprise Subscription Desktop Addendum:
Automatically incorporated by reference into the enrollment if the Enterprise or Enterprise Subscription Agreement is version 6.4 or earlier. The addendum provides for product mix, step-up, and other features contained in the new agreements but not available in the 6.4 or earlier agreements.
Multiple Previous Enrollment Form (MPE):
Identifies if the customer is renewing Software Assurance from multiple previous enrollments or agreement into a new single Enrollment. The MPE lists the previous enrollments and conditions for identifying it to the new Enrollment.
Media Form:
Identifies if the customer elects to receive physical media with its license purchase. It asks for the choice of media, language requirements, and the type of subscription the customer chooses to receive.
Qualifying Government Entity Form:
Identifies the customer as a government entity, as that term is defined in the form and includes certain alternative terms.
Supplemental Contact Information Form:
Identifies the customer‟s selection to have separate contacts for certain documents, special products, and product support.

A Select enrollment renewal only needs an order within 30 days of expiration for SA only orders.
A customer may also opt to submit a new Select agreement. In this case, the renewal form is not required for renewal.

When is an Academic Select Agreement required?
The Academic Select agreement is required for new Institutions when they or one of
their affiliates does not have an active Academic Select agreement in place already.
A Institution may opt to enter into a new Academic Select agreement at any time to
benefit from the expanded product offerings. This is not mandatory.
An Academic Select agreement expires after the initial 3 year term. The agreement
can be renewed one time for an additional 1 to 3 years. After that, a new Academic
Select agreement is required. All enrollments are renewed automatically when an
Academic Select agreement is renewed. If a new Academic Select agreement is
executed, new enrollments must also be completed.
What is the role of the Academic Select Agreement?
The Academic Select agreement sets out the core terms and conditions that apply to
volume licensing purchases under the Academic Select program.
An active Academic Select agreement must be in place for your Institution or one of
its global affiliates in order to enroll and place orders under an Academic Select
enrollment.

What is the role of the Academic Select Student Enrollment?
A Student enrollment enables students (and faculty) to acquire Microsoft software in the course of the students (and faculty‟s) enrollment (employment) at the institute.

(Renewal Form)
When is an Academic Select Renewal Form required?
The Renewal Form is required at the end of the Institution's initial agreement term to renew its agreement without change. The form documents the Institution's choice to continue its agreement for another term.
A Institution may also opt to renew by submitting a new Academic Select agreement. In this case, the Renewal Form is not required for renewal.
What is the role of the Renewal Form?
The Renewal Form documents a Institution's decision to renew the same configuration of the agreement for an additional 12 or 36 months. All enrollments are then automatically renewed with the renewal of the Academic Select agreement.
The Renewal Form must be submitted to Microsoft before an order for Software Assurance can be placed.
A new agreement and enrollment is required if the Institution has already completed a 12 or 36 month renewal term.
