KNOWLEDGE MANAGEMENT
6
OBTAINING KNOWLEDGE
• Effective recruiting and retention (ROI on an average employee is 4 years)
• Appropriate level of new hire qualifications for required skills (increased base skills can reduce training time by 30-40%)
DEVELOPING KNOWLEDGE
• Training programs properly aligned to key skill gaps significantly reduce training costs
• Applying knowledge immediately is often overlooked and is critical to ensure competency
RETAINING KNOWLEDGE
• Initial qualification, requalification, and application of skills is critical to ensure knowledge is maintained.
• Work assignments, rotation and mentoring, even at the highest skill levels is often overlooked in maintaining competency of the workforce
LOST KNOWLEDGE
• Infrequently performed tasks, technology evolution and other issues can cause the inability of existing staff to perform critical work.
• If not properly captured and transferred, knowledge lost to attrition can increase error rates and inefficiency
An effective knowledge management system is a critical tool that reduces human cost, errors and preventable incidents while ensuring the success of your organization
Each of these elements impacts the Knowledge Management scope and approach
MISSING KNOWLEDGE
• Where areas of work are rarely performed, critical knowledge gaps can exist and must be remediated via simulation or other methods to ensure business continuity.
• Expertise in a large company is often embedded in single locations and can lead to significant skill gaps in other areas

CASE STUDIES
EMERGENCY PREPAREDNESS CASE STUDY
At 1930 hours, following a major lightening storm, a gas storage facility operator received notification from the local 911 call center of a large fire on their gas storage wellsite in Clinton
County, Pennsylvania
At that time, a column of intense orange flame was visible two miles away
15
Key Events:• Evacuate and care for impacted stakeholders• Establish command and control of the scene• Interface with local responders and well
control specialists• Extinguish and kill the well• Mitigate offsite pollution• Recover business continuity
“Interliance’s integrated response management systems were instrumental in allowing our well control provider to execute a successful outcome to this catastrophe”
HOW WE HELPED
16
With a clear response strategy, a well written response plan and highly-trained emergency response personnel the scene was controlled, the well was extinguished, and fatalities and catastrophic
pollution were avoided.
Prepare
- The Operator has prepared wisely by completing a new integrated emergency response plan specifically for this location
- The Operator conducted a community outreach training session for all local first responders
- The Operator reached out quickly for special assistance from well control specialists, air monitoring and pollution control experts.
- Thanks to coordination and shared service agreements, response resources were dispatched from three neighboring facilities.
- Sheriff’s deputies and firefighters were trained on how to set up an appropriate command post a safe distance from the wellsite.
- As a result, command and control of the scene were established within the first hour of notification
Respond
- While nearby stakeholders were being contacted, local firefighters checked the facility perimeter for additional sources of unignited gas.
- The Operator’s response team located and closed nearby flow valves, eliminating leaks and potential fires at other nearby well sites.
- Arriving regulatory officials were guided directly to the Operators Command Center for initial briefings
- Upon confirmation of what had happened, the Operator’s PIO established a JIC in the nearby community center for arriving media personnel.
- Commonwealth regulators expedited approval of emergency fire water extraction permits to allow for water shuttles to take place 24 hours a day.
- The well control activities were completed in 5 days without a single injury.
Recover
- A facilitated debrief was held to capture lessons learned about the incident.
- Due to the hazardous materials, the fire, and runoff into the nearby forest, a joint effort between the Fire Dept., State Police, Dept. of Environmental Quality, the EPA, US Chemical Safety and Health Investigation Board, and OSHA was successfully managed.
- The Operator’s business continuity plan was activated as a component of the incident response which allowed for other critical production operations to continue.
- The Operator held a facilitated tabletop exercise 4 months after this incident to-reevaluate their plan and assess the competency of their response personnel.
- The Operator conducted ICS training for their senior leadership personnel to help them better understand the process for all future emergency situations.
LESSONS LEARNED
17
The Clinton County storage well blowout reinforced a number of important lessons learned.
DISASTERS HAPPENLarge incidents such as this one can happen to anyone. Preplanning is necessary, as is having on hand a list of all resources that could be used. Call on your specialists early in any incident so actions can be taken quickly and safely. Make sure you have the right tools at your fingertips at all times
IMPLEMENT THE INCIDENT COMMAND SYSTEMThis allows all responding departments, agencies, and company officials, and others to know who is in charge and what is being done. Within minutes, a temporary command post was established 1,000 feet from the well location.
DO NOT RUSH!Hasty, heroic actions may result in dead heroes. Complex operations demand a well-thought-out, methodical approach with constant reevaluation and continual size-up. You don’t know what the well will do next – safety of on-scene personnel is paramount.
CONTROL THE SCENEKeep only essential personnel on scene during an incident like this. Legal counsel may be needed for litigation after the incident, but should not be at the command post when attempts to control the incident are in process. Similarly, O&G Senior Leadership should be allowed on the scene as long as their knowledge and cooperation are needed; when they begin to interfere with the emergency processes, they should be excused.
THE IMPORTANCE OF MANAGED STAGINGKeeping the scene clear of nonessential equipment allowed evacuation vehicles and arriving response equipment to get in position without having to fight against crowded roads.
THE IMPORTANCE OF INCIDENT MANAGEMENT TRAININGCompany personnel and first responders should be trained in the methods used to mitigate a complex incident. Community outreach training provided by the Operator was the major contribution to the success of this response.

KNOWLEDGE MANAGEMENT CASE STUDY
One of the largest energy suppliers in the United States found itself at a high
risk in its ability to find and hire sufficient levels of qualified workers to
meet its immediate and long-term needs
The firm’s executive team selected Interliance to design a workforce and knowledge management strategy…
20
Background:• Workforce development program lacked
clear vision, direction, and metrics• Imminent loss of skills in the workforce
due to a limited supply of skilled workers and pending retirements of high-value employees
• Annual unplanned outages across the seven plants totaled 1,600 – amounting to $212M in lost revenue
• 50% of these outages were directly attributable to avoidable human errors
HOW WE HELPED
21
We developed a comprehensive workforce and knowledge management strategy that guided the company through transformation of critical functions and drastically reduced training lead-times
and lost revenue due to human error
1. Capability Assessment• Performed a holistic workforce capability
development assessment and identified risks, issues, and opportunities across several key areas:• Knowledge Management• Strategic Management & Governance• Curriculum Development, Management,
and Delivery• Testing and Qualifications• Program Management and Infrastructure
2. Program Development• Developed and aligned workforce
management strategy with key stakeholders
• Developed strategic and operational performance metrics
• Re-chartered a steering committee to design the program
• Performed a functional and organizational assessment to determine the optimal future-state organizational structure
3. Detailed Analysis• Performed the “boots on the ground”
analyses of all high-risk areas across the organization, including:• Training Programs• Staff Loading Requirements• Hiring Plans and Staffing Strategy• Workforce and Impact Analyses• Job Progression Strategy• Plant Applicability• Existing Documentation Evaluations• Knowledge Transfer Strategy
4. Implementation• Developed optimized processes and
training manuals, maps & strategy• Created multi-year training plan• Implemented “Train the Trainer” and
