LeadingAge Washington invites you to participate in the 2017 Long Term Services and Supports Employee
Compensation Survey. The results of this survey will be reported based on facility type, operating budget size,
and geographic region. The reliability and use of the report will be enhanced with increased participation of all
facilities. We encourage you to complete the survey and mail it in with your order for a copy of the report.
Please complete & submit the attached compensation survey no later than Friday, June 30, 2017.
We must charge a fee to cover the costs of administering the survey and compiling and reporting the results.
Nonparticipants are able to purchase the report of survey results but for twice the fee.
Please return the completed survey timely. Complete the form below and return it to LeadingAge Washington,
with a check for $150.00, payable to LeadingAge Washington. The report of the survey results will be mailed to
you in July.
If you do not wish to complete the survey, but would like a copy of the report, please complete the form below
