ThisformshouldbecompletedandreturnedtoMEOWwithin30daysfollowingthecompletionofyourevent.
4. DonationTrackingSheetThisformmustbesubmittedinorderforeventdonorstoreceivecharitabletaxreceiptsfromMEOWFoundation.Pleaseensureyoureadtheenclosed“TaxReceiptingGuidelines”beforesubmittingthisformasnotallgiftsmayqualifyforataxreceipt.

GUIDELINEScontinuedWhatisaThirdPartyFundraisingActivityorEvent?
ThirdpartyactivitiesandeventsarefundraisersthatinwholeorinpartbenefitMEOWFoundation,itsmissionanditsgoals,andwhichareorganizedandexecutedbyanindividualororganizationexternaltothecharity.
ApprovalProcess
• OrganizersmustcompleteandsubmittheThirdPartyFundraisingApplicationtoMEOWFoundationpreferablynolessthan3weekspriortothedateoftheproposedfundraisingactivity.Approvalmustbereceivedbeforeadvertisingispermitted.
• Applicationsarevalidforone(1)eventonly.Annualeventandactivityorganizerswillneedtore-applyeachyear(perevent).
• MEOWFoundationmustbenotifiedofotheractivity/eventbeneficiariesaswellasactivity/eventsponsors.
• MEOWFoundationreservestherighttodenyanyapplicationforafundraisingactivitythatisnotinlinewithMEOW’smissionand/orgoals,orwhichdoesnotprojectapositiveimageofMEOWFoundation.
EventPromotionandUseoftheMEOWNameandLogo
• Yourorganization/groupmustreceivepermissionfromMEOWFoundationtouseournameand/orlogoinconjunctionwithyourevent.MEOWFoundationrequeststhatallpromotionalmaterialsaresubmittedtothefoundationforapprovalpriortodistribution.HighresolutionfilesoftheMEOWlogoareprovideduponrequest.
• Pleasenote:TheMEOWFoundationlogocannotbeusedforanypurposesotherthanthosedescribedintheThirdPartyFundraisingApplication.
• Allpromotionalmaterialsshouldclearlystatethattheactivityoreventis“InSupportof”or“Proceedsto”followedbytheMEOWFoundationlogo.MaterialsmustNOTstatethatMEOWFoundationisasponsororco-sponsorofanactivityoreventunlessotherwiseapprovedbyMEOWFoundationinwriting.
Liability
• Ifyouareconductingraffles,lotterysalesoranyothertypeofactivitythatinvolvessellingtothegeneralpublic,youmayrequireaspeciallicenseorpermit.Pleasemakesureyoutakethenecessarystepstoobtainthese.MEOWFoundationwillnotprovidelicensesorpermitsforthirdpartyactivitiesoreventsandwillnotbeheldliableshouldyoureventoractivitybefoundinviolationoftheserulesandguidelines.
• MEOWFoundationwillnotprovideanyinsurancecoverageforthirdpartyeventsoractivities.Eventorganizersmusttakethenecessarystepstoobtainproperinsurance.
• MEOWFoundation,itsemployees,volunteersandmembersarenotliableforanyaccidentsorinjuries,damageorthefttoindividualsorpropertyandcannotassumeanytypeofliabilityforyourevent/activity,participants,volunteersoremployees.

GUIDELINEScontinuedCharitableTaxReceipts
MEOWFoundationwillissuedonationreceiptsforanyqualifyingmonetarydonation.Ifanindividualhasmadeadonation,thereceiptwillbeissuedinthenameofthatindividualattheirhomeaddress.Ifacorporationhasmadeadonationandareceiptisrequested,thereceiptwillbeissuedinthecompany’snameatthecorporateaddress.Anindividualorcorporationcannotreceiveacharitabledonationreceiptformonetarydonationsmadebyotherparties.Forexample,anindividualororganizationcannotreceiveareceiptforthetotalproceedsofaneventoractivity(asmoniesweredonatedbymultipleparties).
Ifyourequiredonationreceiptsissuedtoparticipantsofyoureventoractivity,weaskthatyouincludeMEOWFoundation’scharitablenumber(#895012813RR0001)onallpromotionalandpublic-facingmaterials.Weaskthatyoustate“taxreceiptswillbeissuedfordonationsof$20ormore,oronrequest.”Within30daysfollowingyoureventoractivity,pleaseprovideMEOWFoundationwithalist(makingsurethatallinformationislegibleandcomplete)ofnames,mailingaddressesincludingpostalcode,phonenumbersandemailaddress(optional)aswellasthecorrespondingamountofeachgifttobereceipted.Donationreceiptswillbeissuedforallmonetarydonationsof$20ormore.
MEOWFoundationwillissuecharitabledonationreceiptsforin-kinddonations(donationsofpropertysuchasprizesormerchandise)equaltotheFairMarketValueoftheitem(s)donatedprovidedsupportingdocumentationverifyingFairMarketValueisreceived.QualifyingdocumentationofFairMarketValueincludes:purchaseinvoice(s)orotherproofofpurchase,thirdpartyappraisalandotherCRA-approveddocumentation.AsperCRAregulations,itemsoflittlemarketvaluesuchashandmadecraftswillnotqualifyfordonationreceipts.Inkinddonationsofservicesarealsonoteligiblefordonationreceiptsforincometaxpurposes.
Goodsandservicesnoteligibleforadonationreceiptincludeitemssuchasraffleorlotterytickets,generaladmissionorentrancetickets,golfgreenfees,andthecostofdonatedservices.Thisincludesspecialservicesthatnecessitatehiringsomeoneforanevent(i.e.tentset-upandentertainment).
Ataxreceiptmaybeissuedforaportionoftheadmissionpricewhenticketsorentrancefeesexceedthecostperpersonrequiredtoputontheevent.However,receiptscanonlybeissuedfortheportionoftheadmissionpriceexceedingFairMarketValueandcannotbeissuedformorethan80%oftheadmissionprice.
CorporationsandbusinessesthatsponsoraneventwillreceiveanofficialinvoiceorthankyouletterfromMEOWFoundationthatwouldenablethemtoclaimbackthefullamountoftheirsponsorshipasamarketingexpense(whenapplicable).
