CAMBRIDGESHIRE TROUBLED FAMILIES OUTCOME PLAN 2020
CAMBRIDGESHIRE TROUBLED FAMILIES OUTCOME PLAN 2020
Services need to be delivered in a way that engages at times hard to reach families, achieves the outcomes they want and delivers
interventions efficiently. There will be cases that do not use the EHA and Support Plan process but still work with Troubled Families. To be
included in the CCC Troubled Families scheme, there must be an assessment that covers the needs of the whole family and each individual
member, a planned intervention coordinated across more than one agency and an identifiable lead practitioner. How this assessment and plan
are recorded should reflect good practice in the area of service delivery but it does not need to be included in any specific written template.
