1. STAYING SAFE IN THE COMMUNITY: PARENTS OR CHILDREN INVOLVED IN CRIME OR ANTI-SOCIAL BEHAVIOUR

TARGET GROUP

ASB 1.1 - A child who has committed a proven offence in

previous 12 months

ASB 1.2 - An adult within the household or a child who has

received an Anti-social Behaviour intervention in the last 12

months

ASB 1.3 - An adult prisoner (who will have parenting

responsibilities on release) or young person (who will be

returning to the family home) in a secure unit who is less than

12 months from his / her release date

ASB 1.4 - An adult with parenting responsibilities serving a

community order or suspended sentence

ASB 1.5 An adult who is currently subject to a licence or

supervision in the community following release from prison,

and has parenting responsibilities.

ASB 1.6 - An adult or child involved in a gang; county lines

linked crime and exploitation; and serious violence (including

offensive weapon crime such as knife crime, gun crime and

The family are actively participating in the community by (but not

limited to):

• Having access to and proactively engaging with community

services and/or local facilities and activities

• Engaging with a positive network of people and developing

increased their competence as a parent.

2. GETTING A GOOD EDUCATION AND SKILLS FOR LIFE: CHILDREN WHO HAVE NOT BEEN ATTENDING SCHOOL REGULARLY

TARGET GROUP

EDU 2.1 - A child whose average attendance over the last

three consecutive terms is less than 90 per cent (authorised

absence being excluded e.g. the indicator being above 10%

unauthorised absence)

EDU 2.2 - A child who has received at least 3 fixed term

exclusions in the last 3 consecutive school terms; or a child at

primary school who has had at least 5 school days of fixed

term exclusion in the last 3 consecutive terms; or a child of

any age who has had at least 10 days of fixed term exclusion

in the last 3 consecutive terms.

POSSIBLE OUTCOMES FOR TARGET GROUP

ALL Children to attend school at least 90% time excluding authorised

absence, and sustain this attendance for three consecutive terms or

have made a successful transition to post 16 education, training or

employment and sustained for at least six months

And/or A suitable elective home education provision is in place

2.1, 2.4, 2.5 In the view of an appropriate education professional,

Children to make from a low base a sustained and significant

improvement in their attendance. For this to apply starting attendance

suitable full-time education. This includes where the child’s

attendance is authorised but at a level that impacts on

learning and/or development

will need to be below 40% and improvement of at least 40% achieved

and sustained for six months.

2.2, 2.3,2.4 Children have fewer than 3 fixed term exclusions and no

permanent exclusions across 3 consecutive terms

2.3, 2.4, 2.5, 2.6 Children have a suitable Individual Alternative

Education Plan in place

3. IMPROVING CHILDREN’S LIFE CHANCES: CHILDREN WHO NEED ADDITIONAL SUPPORT, FROM THE EARLIEST YEARS TO ADULTHOOD

TARGET GROUP

CWNH 3.1 - A child/family with an Early Help assessment or

identified by a professional as needing one

CWNH 3.2 - A child who has been subject to an Child

Protection or Child in Need Enquiry

CWNH 3.3 - A child is subject to Child Protection Plan

CWNH 3.4 - A child assessed as ‘in need’ under section 17,

Children Act 1989

POSSIBLE OUTCOMES FOR TARGET GROUP

ALL Reduced risk / concern for the family or an individual within the

family over a six month period (for example reduced missing incidents)

And/or

Undertaking accredited parenting course and implementing new

strategies over a six month period

3.1 Early Help Assessment being implemented and successfully

completed and no accepted re-referral to services within six months

CWNH 3.5 - A family who is or has been entitled to 15 hours

free early education for two-, three- or four-year-olds and has

and mental health needs. This includes A child identified as

having a delay in speech language and communication skills

CWNH 3.7 - A child who did not achieve a good level of

development aged five.

CWNH 3.8 - A child experiencing or at risk of exploitation,

including sexual exploitation.

CWNH 3.9 - A child where there have been missing from home

incidents.

CWNH 3.10 - A child affected by parental conflict

CWNH 3.11 - A child nominated by professionals as having

problems of equivalent concern to the indicators above.

3.2, 3.3, 3.4, Child protection or Child in Need Assessment being

completed with issues resolved and no accepted re-referral to services

within six months

3.2, 3.3, 3.4, Child protection or Child in Need Plan being closed as no

longer required and no accepted re-referral to services within six

months

Assessment (e.g. the CCC MAP tool/Distance Travelled Tool /

assessment tool or relevant equivalent risk assessment and no

4. IMPROVING LIVING STANDARDS: FAMILIES EXPERIENCING OR AT RISK OF WORKLESSNESS, HOMELESSNESS OR FINANCIAL DIFFICULTIES

FE 4.1 - An adult in receipt of out of work benefits

FE 4.2 - An adult is claiming Universal Credit and is subject to

work-related conditions

FE 4.3 - A child who is leaving school with no or few

qualifications and no planned education, training or

employment.

FE 4.4 - A young person who is not in/at risk of not being in

education, training or employment.

POSSIBLE OUTCOMES FOR TARGET GROUP

4.1, 4.2 An adult within the family has secured continuous employment

by:

Working age/legacy benefits (such as Jobseekers Allowance):

move off out of work benefits and be in employment

continuously for 26 out of the last 30 weeks

Universal Credit: reach the Average Earning Thresholds (AET)

or above continuously for 26 weeks out of the last 30 weeks.

Not required to actively seek employment (i.e. in legacy

benefits under Employment Support Allowance / Income

Support type arrangement): be in continuous employment for

13 weeks.

FE 4.5 - A family at risk of homelessness (Inc. where there is a

high level of rent arrears or unmanageable levels of debt)

FE 4.6 Families at risk of homelessness or living in

accommodation which it is not reasonable for them to

continue to occupy.

FE 4.7 Parents and families nominated by professionals as

being at significant risk of financial difficulties. This may

include those with problematic / unmanageable levels and

forms of debts, those with significant rent arrears and those

experiencing in work poverty.

And/or

Being deemed by the DWP to be a) Working Enough or suitable for b)

Work Focused Interview c) Work Preparation and conforming to the

requirements.

And/or

An adult within the family has shown sustained and significant progress

to work by securing one or more of the following:

employment mentor support;

employment and training related information,

advice and guidance;

support with soft skills such as CV writing and interview

techniques;

support with basic skills such as Maths/English classes;

sector specific employability courses;

work experience;

apprenticeships;

work trials;

traineeships;

Work & Health programme and occupational training

Undertaking voluntary work

Achieving or working towards a recognised vocational

qualification

Family member has developed Curriculum Vitae and will be

ready to apply for jobs when this is possible,

Family member has engaged with support to reduce barriers to

work,

Success in removing blocks to employment and taking steps to

prepare for work

Progress in health or other areas of the plan increase the

employability of the adult.

Engaging in positive activities, showing confidence and taking

recognised steps to prepare for work

An improvement in commitment to accessing employment

support, for example attending interviews with JCP or work

related meetings with other professionals

An improvement in attitude and readiness towards going

into/returning to work

Improved scores against all areas of the Job Centre Plus Work

Outcomes Tool

Identifying they receive benefits not requiring they seek work

and that they have responsibilities that make them unavailable

for employment.

During Covid 19 crisis:

Family member is temporarily out of work due to Covid 19 and

will be work ready when the crisis subsides,

Family member (including young person) has achieved a

temporary job or volunteered supporting the Covid 19 effort

Young Person experiencing NEET but ready to attend

provision/work when the crisis subsides

4.3, 4.4 A young person who is NEET or at risk of being NEET is

engaging with support and is taking recognised steps to enter EET or

is now EET and progress is maintained for six months.

4.5, 4.6, 4.7 Families are working towards reducing the risk of financial

exclusion over a six month period by;

Putting a debt/rent arrear reduction plan in place and adhering

to it to manage debt

Eviction process suspended / on hold

Take up of Free School Meals and appropriate benefits

Securing appropriate housing

Family have engaged with financial support and have made

progress to financially stability,

4.5 No court action has been commenced and current tenancy is

secure six months after action to secure tenancy is completed

4.6 Family accommodation is deemed to be permanent and

5. STAYING SAFE IN RELATIONSHIPS: FAMILIES AFFECTED BY DOMESTIC ABUSE (In this section young person refers to 16 years old and above.)

TARGET GROUP

DV 5.1 - A young person or adult known to local services as

having experienced, currently experiencing or at risk of

experiencing domestic violence or abuse

DV 5.2 - A young person known to local services as having

perpetrated an incident of domestic violence or abuse in the

last 12 months

DV 5.3 -The household has been subject to at least one police

callout for domestic incident, including for “honour” based

violence, in the last 12 months

DV 5.4 - A young person or adult known to local services has

experienced, is currently experiencing or is at risk of

experiencing sexual violence or abuse.

POSSIBLE OUTCOMES FOR TARGET GROUP

ALL Reduced risk / concern for the family or an individual within the

family over a six month period (for example evidence of a safety plan in

place and working, young person disengaged from relationships and

behaviour placing them at risk)

5.1, 5.2, 5.3, CAADA-DASH risk score reduced at exit as assessed by case worker And/or

Engaging in domestic abuse programme

Victim or perpetrator engaging with support and has an understanding of their experience

Where a family has been discussed at MARAC there is no subsequent MARAC referral

5.4, 5.5, 5.6, 5.7 Evidence that there have been no further incidents.

6. LIVING WELL, IMPROVING PHYSICAL AND MENTAL HEALTH AND WELLBEING: PARENTS AND CHILDREN WITH A RANGE OF HEALTH NEEDS

TARGET GROUP

HEA 6.1 - An adult (who has parenting responsibilities) with

mental ill-health across mild, moderate or severe needs.

HEA 6.2 - A child with mental ill-health across mild, moderate

or severe needs.

HEA 6.3 - An adult with parenting responsibilities and a

substance misuse problem, a child with substance misuse

issues themselves or a child exposed to substance use in

their home environment.

POSSIBLE OUTCOMES FOR ALL TARGET GROUP

All Reduced risk / concern for the family or an individual within the

family over a six month period

And/or

Family member referred and attached to relevant health service and /

or family member is self-managed and has an improving level of

functionality and confidence in managing own health needs

6.4, 6.5 All household members registered with a GP

HEA 6.4 - Expectant or new parents, with a mental health or

substance misuse problem and other health factors which

may affect their parenting, or a young child where there are

concerns regarding their physical, social or emotional

development. This could include mothers who are receiving a

Universal Partnership Plus service or participating in a Family

Nurse Partnership.

HEA 6.5 - Adults with parenting responsibilities or children

who are nominated by health professionals as having any

mental and/or physical health needs of equivalent concern to

the indicators above. This may include unhealthy behaviours,

resulting in problems like obesity, malnutrition or diabetes.

HEA 6.6 - A child eligible for an Education Health and Care

Plan

HEA 6.7 - A child with a concerning level of A&E admissions

All children have had age appropriate vaccinations and screening

programmes

And/or

Reduced number of missed appointments over a six month

period

All children are registered with a dentist and have had a check-

up within the last six months

One household member who has been assessed as

over/underweight is maintaining a healthy weight

6.3, 6.4 Successful completion of substance misuse treatment with no

more than 1 re-lapses over six month period

And/or

Successful completion of alcohol de-addiction programme

6.6 Health checks are completed within the EHCP process and

medical intervention made available if required.

And/or

Any child supported by the Children’s Disability unit, has remained

open to the service for at least 12 months and have remained in the
