Experience in Hospital Management

GENERAL DIRECTOR OF BENHA CHILDREN’S HOSPITAL (1996 - 2004). Benha children’s hospital was established in Benha, Egypt, in 1996 to help upgrade medical & surgical care of children in the Nile Delta under the supervision of Prof. Hadidi. Today, it is a tertiary referral centre for children from all over Egypt and is ranked as one of the top 4 Pediatric Surgical centers in Egypt. The hospital has organized 3 international congresses and workshops with participants from all over the world. There are more that 3000 operations performed every year including 30 esophageal atresias, 20 diaphragmatic hernias, 40-50 Hirschsprung’s disease and 100-120 hypospadias repair every year. The hospital is famous for its specialized centers for Hypospadias Repair, Hirschsprung’s disease and Cleft Lip & Palate and esophageal surgery. It is a well established centre for training of pediatric surgeons in training and has accommodated several young doctors from Italy, Germany and Greece to gain experience in Hypospadias surgery and Hirschsprung’s disease.

Distale Hypospadie schonend beheben

Postoperatives Management ohne Harnröhrenschienung, Katheter und Wundverband

Für die Behandlung der distalen Hypospadie steht mit der modifizierten Mathieu-Plastik mit

YV-Glansplastik nach Hadidi nunmehr eine Technik mit guten funktionellen und

kosmetischen Ergebnissen zur Verfügung. Die postoperative Belastung der Kinder ist deutlich

geringer.

Die Hypospadie zählt mit einer Inzidenz von 1 zu 300 zu den häufigsten angeborenen

Fehlbildungen des Urogenitaltraktes. Mehr als 300 verschiedene Operationstechniken mit

unterschiedlichem Komplikationsrisiko und kosmetischem Ergebnis wurden beschrieben. Die

komplexe angeborene Fehlbildung der männlichen Harnröhre und der sie umgebenden

Strukturen entsteht als Folge einer Entwicklungshemmung in der 9. bis 13.

Schwangerschaftswoche. Ein nachgewiesener Faktor dabei ist ein Rezeptordefekt auf

Testosteron und Dihydrotestosteron. „Weiters scheint eine hormonelle Störung auf

testikulärer Ebene vorzuliegen, da eine Testosteronsubstitution zu Größenwachstum des Penis

sowie zu einer Milderung der Penisschaftverkrümmung führt“, beschreibt OA. Dr. Elisabeth

Frigo von der Abteilung für Kinderchirurgie des SMZ-Ost-Donauspitals in Wien die

Fehlbildung. Klinisch ist der Meatus urethrae externus – je nach Schweregrad – zwischen

Perineum und Glansspitze lokalisiert zu finden. Die Dysplasie der Corpora cavernosa und des

Corpus spongiosum sowie das Ausmaß der Penisschaftverkrümmung sind ebenso wesentliche

Faktoren für den Ausprägungsgrad der Hypospadie. Die Glans ist bei dieser Fehlbildung nach

ventral gekippt und weist breit ausladende Glansflügel auf. Die Tunica Dartos und die

Buck´sche Faszie sind ventralseitig miteinander verbacken und bilden lateral des Corpus

spongiosum derbe fibröse Stränge, die so genannte Chorda. Das Präputium liegt der Glans

kapuzenförmig an und ist ventral gespalten.

Einwandfreies Resultat

Ziel der chirurgischen Therapie ist ein funktionell und kosmetisch einwandfreies Resultat mit

einem aufrechten Penis und einem an der Glansspitze lokalisierten Meatus. Im Laufe der Zeit

wurden zahlreiche Operationsmethoden beschrieben, wobei prinzipiell eine Meato- und

Glansplastik, Penisschaftaufrichtung, Harnröhrenplastik sowie Hautdeckung durchgeführt

werden. „Zur Korrektur der distal penilen Hypospadie hat sich an unserer Abteilung die

modifizierte Mathieu-Plastik mit YV-Glansplastik nach Hadidi durchgesetzt“, berichtet Figo.

„Wir führen zunächst eine Y-förmige Inzision an der Glansspitze durch und vernähen diese

V-förmig. Dadurch erzielen wir einen weiten Neomeatus.“ Anschließend erfolgt die

Umschneidung der Urethralrinne, zur Formung der Neourethra wird ein U-förmiger Flap aus

der ventralen Penisschafthaut gebildet. Ein bindegewebiger Lappen wird zur Deckung der

Neourethra darüber genäht. „Zuletzt führen wir die Glansnaht über einen Katheter durch, den

wir danach entfernen“, erläutert die Kinderchirurgin. Im Vergleich mit der ebenso

angewandten Urethroplastik nach Snodgras zeigte sich ein exzellentes kosmetisches Ergebnis

und gute Blasenfunktion in beiden Behandlungsgruppen bei gleicher Komplikationsrate. Die

häufigsten Komplikationen nach Hypospadie-Korrekturoperationen sind Fisteln, Strikturen,

Meatusstenosen und Divertikel. Deutliche Unterschiede finden sich jedoch bei der

postoperativen Behandlung sowie der Dauer des stationären Aufenthaltes in den

Behandlungsgruppen. Nach Snodgras operierte Kinder benötigen postoperativ einen

Urethralstent, ein Zystofix und einen Wund-verband. Die Verweildauer im Spital beträgt im

Durchschnitt zehn Tage. Ohne Harnröhrenschienung, Katheter und Wundverband erfolgt

hingegen das postoperative Management der Patienten, die mit einer modifizierten Mathieu-

Plastik mit YV-Glansplastik nach Hadidi versorgt werden. Sie werden in der Regel nach vier

Tagen in häusliche Pflege entlassen. „Große Bedeutung kommt hier der aus- reichenden

Analgesie zu, um eine problemlose Miktion zu gewährleisten. Zur Wundpflege applizieren

wir regelmäßig Gentamycin-Salbe. Das Verfahren hat sich als weniger belastend für die

Kinder erwiesen“, fasst Frigo die Vorteile der neuen Methode zusammen, die gleich gute

Ergebnisse und einen deutlich kürzeren stationären Aufenthalt ermöglicht.
