Defects in the maxillofacial area require reconstructive techniques usingbone transplants, soft tissue transferand implant insertion inorder togetadequate oral function and aesthetic outcome. Whether to use free bonegraftsorvascularizedgrafts, whethertoinsertstandardimplantsorzygomaimplants, whethertochoosepedicledflapsorfreevascularizedflapstoclosethe soft tissue defect is dependent on the size and location of the defectandthegeneralhealthconditionsofeverypatient.Ameticulouspreoperative3Dplanningandthe fabricationofsurgicalguidesandtemplates leads topredictablegood results, especially indefectswithextensive lossofboneandsofttissue.Inaseriesofpatientsthedifferenttechniqueswillbeshownandexplained,includingtheprefabricationoffibularflaps.

oudSt.Jan-morus

To regenerate a tumour related defect or an atropic jaw instead of justrepairing it, is the issue of the 21th century. Bone substitutes, which areavailable on the market today, do not offer osteoinductive properties,on the other hand harvesting of autogenous bone brings up donor sitemorbidity.Growthfactoreslikeplateletrichplasmaorbonemorphogeneticproteinsalonecouldnotfulfilltheneedsandexpectations.InKiel,asaroutineprocedure,astemcellbasedapproachisappliedfortwoyearsnow.Morethan30patientsreceivedthiskindoftreatment.Besidestheclinicaloutcomes,CT-studiesandbonebiopsieswererecorded.The results show a superior outcome compared to patients receiving thestandardtechnique(iliaccrest/bonesubstitute).Inconclusionwecanstate,that thestemcellbasedapproachmighthelp to improve the regenerationtechniquesandreducethedonorsitemorbidity.
