Request and coordinate with FCM to ensure Comoptrailing and congested cables are being tidied up by reasonable means such as spiral conduit, fastening cables, trunking etc. and extension bars are kept / install off floor.

4 CPU of the computer was found wall-mounted under the desk in the doctor’s office # 0-735. The CPU hits the legs when a staff is working at the desk (see picture # 4).

Request and coordinate with IT & FCM to identify a suitable location for the CPU and relocate the CPU.

5.1 Glass panel doors of the cupboard in room # 01-714 were found broken. Sharp edges of broken glass (on the floor and attached to the cupboard) pose a risk of cut injury (see picture # 8).

2014 : Outstanding Action Items from EHS Internal Inspection last year13 Bedside Critical Equipment: A few

bedside panels in the Trauma Room in Peds ED do not seem to have electrical sockets that are connected to UPS. Therefore all bedside critical equipment e.g. monitors etc. are currently connected to non-UPS electrical sockets which may not be operational if a power shutdown occurs.

Peds ED to assess the need for having electrical sockets connected to UPS on bed side panels for patient safety and to coordinate with Eng. Kamal in FCM for installing electrical sockets that are connected to UPS if the assessment requires necessary.

14 Bedside Critical Equipment: Critical equipment e.g. bedside monitors in a few patient rooms were not found connected to electrical sockets backed by UPS. Therefore such bedside monitors may not be operational if a power shutdown occurs.

Peds ED to assess the need for connecting all critical equipment e.g. bedside monitors for patient safety in power shutdown situation and to ensure all bedside critical equipment are only connected to UPS backed electrical sockets if the assessment requires necessary.

Remind all staff in Peds ED on the importance of above mentioned necessity.

22 Overhead Hazard: Overhead shelves in the doctor’s room # 0-735 present risk of head injury to staff that are using the desk under these shelves. Note: Doctors stated that some of the doctors had already been injured from the overhead shelving.

Coordinate with Ashraf Darwish in FCM to access the current overhead shelving and to re-design, install protective padding on shelves or through other reasonable means to prevent staff from head injury.
