    I. SAFETY POLICY

    M-4 Construction & Maintenance Services, Inc. (M-4CMSI)believes that it is in the best interestsof all to promote and preserve the safety of the community, M-4CMSI employees, the environment and theproject. Implementation of an effective Safety Environmental & Health program is a proper function ofmanagement. This humanitarian gesture of the company is paramount in all its undertakings, because werecognizes that proper management lessens the potential hazards associated with every project. M-4CMSIpolicy was developed in recognition of the need to emphasize concern for these issues.

    Consistent with the foregoing, M-4CMSI s responsibilities are:

    (1) To comply with all applicable safety-related as well as environmental protection laws & regulations.

    (2)

    To conduct business according to recognized standards of the industry, including to those related topersonal safety, occupational health, fire protection and the environment.(3) To maintain a safe and healthy working conditions and environment;(4) To acknowledge the EH&S objectives are of equal importance with other key operational objectives.(5) To educate all employees, on an ongoing basis, with respect to hazards, programs, regulations and

    practices to consistently adhere to ensure effective control & prevention of accidents & illnesses;

    (6) To hold each employee responsible and accountable for EH&S related performance; and(7) The conscientious observance of all clients safety requirements as stipulated in every contract, and

    compliance with the HS safety regulations.

    A. SAFETY PROMOTIONS AND AWARDS

    Maintaining inherent in safety is a factor of major importance on the success of the safety program.Cognizant of the value of safety promotion in the inculcation of safety consciousness amongemployees, the company through its Safety Department, has developed a recognition award programas a tool for generating effective results in safety.

    (1) Bulletin Boards and Posters

    Safety Bulletin Boards and Posters are strategically installed and posted to serve as reminders andencourage employees to become safety conscious. These bulletin boards and posters are updated/

    replaced/changed periodically. Signs include general safety specific hazards, safety slogan,announcements, & other items on safety.

    (2) Safety Contests

    It is the intention of the management to sustain interest and healthy attitude towards safety bysupporting a constructive competition or special campaign whereby the deserving projects and/or

    personnel are awarded tokens of appreciation and recognition. To stimulate accident preventionprogram, the safety Department administers safety contests such as Inter-Project Safety Competition,Safety Slogan Contest, and the like.

    M-4CMSI, a company duly existing almost 25 years now, is keeping its safety records for every project itundertakes, condition established its safety track from the beginning as well as its maintenance as a safecontractor.

    II. PERSONAL PROTECTIVE EQUIPMENT (PPES)

    A. DIFFERENT TYPES

    The company provides all employees with the necessary protection they need in the performance oftheir work. It sees to it that adequate quantities of protective devices are provided that include thefollowing:

    (1) Minimum Personal Protective Equipmenta.) Safety Helmet (Hard Hat)

    b.)

    Safety Shoesc.) Safety Shirts (short sleeve)d.) Safety Glasses / Gogglese.) Eye & Face Protective devices (such as those used in welding , cutting, grinding, chemical

    handling, etc.)f.) Safety Trousers

    (2) Hearing protection Is required in the premises & construction areas where noise level exceeds in (85)db decibels.

    (3) Dark Glasses or tinted mono goggles shall not be worn inside covered or enclosed structures and atnight.

    (4) Use Safety Belts / Full Body Harness and lifelines when working from high places (6 feet above).(5) Working gloves shall be worn when engaged in activities where danger of hand injury exists.(6) Respiratory protection devices (dust masks, cartridge-type, respirator, gas mask, incases of fire use

    SCBA or Self-Contained Breathing Apparatus, among others)

    B. ISSUANCE AND REPLACEMENT

    Safety shoes and helmets are issued prior to start of every project. Replacements are issued providedthat these PPES are already worn-out. However, if the early worn-out is due to the negligence of theuser, replacement is charged to the user. All other PPES can be acquired from the project tool room orwarehouse as the need arises.

    C. PROPER USE, CARE, AND MAINTENANCE

    It is the responsibility of every worker to use their PPE properly. Utmost care is expected from

    everybody. Cleaning must be done regularly to prolong its life span and give maximum protection to theuser.

    III. WORK PERMITS

    In compliance with the clients work permit system for the job Description & Specifications the following guidelines is being observed:

    (1) Work Permits such as : Hot work Permit for cutting, grinding, welding processes and all other hotworks on site; Safe Work Permitpermit for confined space entry to vessels & the likes; Start-up WorkPermit permit to work on all systems/ common areas of the plant, tank farm & jetties & all worksrequiring lack-out & tag-out of the systems; Excavation Permits Ionization Radiation & Radiographic

    Ray Permitsmust be served prior to the start of every scope of work.(2) Only authorized supervisor, is allowed to process work permit or his authorized designee

    (3) It shall be the workers responsibility to ask their supervisors whether they have secured a work permitbefore starting to work especially on the first hour of every morning during overtime.

    (4) Work permits must be displayed at all times in a designated area within the job site.

    (5)

    JHA (Job Hazard Analysis) is required in applying work permit(6) If the work is within the Restricted Area, the work is subject for approval of Site-Managers or his

    designee.(7)All lights and heavy equipment must be inspected and approved prior to use at the construction site.

    IV. RESPONSIBILITY ON SAFETY

    A. PERSONAL RESPONSIBILITIES

    Each employee in the organization has his own responsibility for safety, broken down as follows:1. Use the proper PPE provided for the job always.2. Keep tools in good condition; Use the right tool or equipment on the right job.

    3.

    If you dont know, ask.4. Think before you act.5. Report on unsafe condition to your supervisor & stop unsafe actions immediately.6. Dont horseplay, distract others, or abuse safety devices in the worksite.

    7. Attend the scheduled toolbox meeting regularly for safety information.8. Perform work safely and obey all safety rules and signs properly.9. Ensure your work area is clean and tidy.10.Dont take short cuts; your safety and that of others is more important. 11.Always refer to the work instructions regarding Safety Procedures & the Quality Safety Manual.

    B. RESPONSIBILITY FOR CO-WORKERS

    As a worker, you have also to look after the people around you especially your co-workers. Make surethat the things or actions you do will not endanger your co-worker.

    V. ACCIDENT REPORTING

    A. SUPERVISORS RESPONSIBILITY

    The supervisor of the employee involved in any accident is directly responsible for preparing theaccident report. Report forms are available at Project Safety Offices.

    The Safety Engineer or Officer will conduct a separate investigation of the accident & will come outwith his analysis of the accident. Corrective & Preventive Measures should be taken-up & revisedanytime an accident occurs. Non-Conformance Reports shall be used to identify a report qualityproblems on the site.

    B. REPORTING OF MINOR INJURY

    Any injury, no matter how minor it looks, must also be reported for proper investigation and so thatcorrective measures are made to avoid future occurrences.

    C. MEDICAL FIRST-AID TREATMENT

    All project sites are provided with a Clinic or First-Aid Station with assigned nurses, first-aider or adoctor to attend to the medical needs of all employees. So, if you were involved in an accident and / orhas incurred an injury no matter how minor it is, see the Clinic.

    Fire-prevention is the term used to indicate measures directed toward averting the occurrences of fire.To prevent fire is to protect lives and properties and ensure the continuity of operation. Importantsafety and operational

    is emphasized by a system of classification using the words: DANGER, WARNING & CAUTION describedas follows:

    DANGERis used to indicate the presence of a hazard which will cause severe personal injury, deathor substantial property damage, if the warning is ignored.

    WARNINGis used to indicate the presence of a hazard which can cause severe personal injury,death or substantial property damage, if the warning is ignored.

    CAUTIONIs used to indicate the presence of a hazard which will or can cause minor or personal

    injury or property damage, if the warning is ignored.

    The Fire Trianglebelow illustrates the components necessary for combustion that normally occurs onsite:

    KINDLING POINT OR HEAT

    All of these components may be present when oxygen is recompressed into a regulation quickly.

    OXYGEN: The oxygen is provided by the cylinder in very pure form. High purity oxygen affectscombustion differently that does atmospheric air. High purity oxygen accelerates the rate ofcombustion, increase heat output, & lowers the combustible point of any material.

    FUEL: The fuel for combustion may be the regulator itself if enough heat is produced to reachthe kindling temperature of the regulators components.

    KINDLING POINT (TEMP) OR HEAT: Enough heat may be generated by the friction created

    when recompressing high pressure oxygen to ignite the regulator components. This heat is known astheHeat of Recompression.

    The Safety Department is equipped with adequate fire fighting equipmentGeneral Fire Safety Rules & Precautions measures are periodically circulated through safety and toolboxmeetings, posters & bulletin boards.

    INSTRUCTIONS WHEN THERE IS FIRE OCCURRENCE:

    When a fire unavoidably breaks out, all workers are advised to do the following:

    A. WHEN YOU ARE WHERE THE FIRE BREAKS OUT:

    Immediately turn on the fire alarm or yell and shout to inform others of the fire.2. Grab the nearest fire extinguisher in the area and try to extinguish the fire. USE THEAPPROPRIATE FIRE EXTINGUISHER FOR THE RIGHT KIND OF FIRE,-OR-

    3. Call the Fire Safety & Environmental Managers Office & report t he fire. State your nameand exact location where the help is needed.

    4. Clear the area as soon as the fire fighters arrive.

    B. WHEN YOU ARE WITHIN THE VICINITY OF THE FIRE:1. Stop working immediately; shut off all equipment .2. Stay calm; take out on a priority basis important items in the work area.3. Walk fast to the fire exit and assemble at the predetermined assembly / evacuation area.4. When the fire fighters arrive, clear off the area.5. Wait for instructions; do not return to work until it is safe.

    VII. GENERAL SAFETY RULES :

    A.

    GENERAL SAFETY RULESTo help ensure your own safety, you should become familiar with the following general safety ruleswhich you are required to observe completely:

    1. Follow instructions, take no chancesif you do not know, ask for safe job instructions.2. Report immediately to your supervisor any condition or practices which you think could

    cause injury to employees damage to property.3. Put everything you use or handle in its proper place. Disorder causes injury and waste of

    time, energy & materials. Keep your work area clean & orderly.4. Use the right tools & equipment for the job; use them in a safe manner.5. If you or the equipment use is involved in any accident the results in personal injury or

    damage to property, regardless of extent or graveness, you must immediately report it toyour supervisor. Get first-aid promptly in the clinic.

    6.

    Use, adjust, alter & repair equipment ONLY WHEN AUTHORIZED to do so.7. Approved PPEs must be worn by everyone working in or walking into the plant or

    construction premises/area.8. When lifting, bend your knees; gasp load firmly; then raise the load, keeping your back as

    straight as possible.9. Dont horseplay; avoid distracting others.10.Obey all specific rules, signs,& instructions around the site must be followed by all

    concerned.11.No cooking or open flames are allowed except in designated areas.12.Firearms, deadly weapons, lighters & matches must be surrendered to the gate guard upon

    entering the project sites.13.Company IDs must be worn visibly at all times when inside the project site premises.

    14.

    Entering / working within the site premises under the influence of liquor, prohibited drugs& other intoxicating substances is strictly prohibited.

    15.Use of shoes with steel heels (Boston) is strictly prohibited inside t he premises.16.Nobody is allowed to enter restricted areas.17.Parking shall be at designated places only.18.Left lanes shall be used when walking along roadways within the project premises.19.Urinating at places not designated for the purpose is strictly prohibited.

    20.Littering is prohibited. Observe good Housekeeping always.21.Sleeping & eating in project areas & construction area can be dangerous & is not allowed.

    B. UNSAFE PRACTICES:

    The following are some of the unsafe practices w/c may warrant a disciplinary action:

    Personal Protective Equipment - PPE(Pansariling Kagamitang Pangkaligtasan)

    1.

    Minimum Personal Protective Equipment (PPE)(Minimum na pansariling kagamitang pangkaligtasan)

    a. Safety Helmet with Chin Strap(Matigas na sombrero na may Chin Strap)

    b. Safety Shoes(Sapatos na gawa o yari sa goma o may proteksyong bakal o steel-toed)

    c. Safety Shirts (short or long sleeve - depends on the work to be done)(Kamisetang maikli / mahaba ang manggasdepende sa gawain)

    d. Safety Trousers(Pantalong Panggawa)

    2. Safety Glasses(Salamin sa mata na may sanga sa tabi)

    3. Hearing protection is required in the premises and construction areas where noise level exceeds 85decibels.(Ang proteksyon sa pandinig ay kailangan sa mga lugar sa konstruksiyon kung ang ingay sa paligid ayhigit sa 85 decibels.)

    4. Dark glasses or tinted mono goggles shall not be worn inside covered or enclosed structures or at night.(Ang mga madidilim na salamin ay hindi dapat isuot kung ang lugar ng paggagawaan ay madilim o kaya aysa gabi.)

    1. Work permit/s must be secured prior to start of every scope of work.(Kailangan kumuha muna ng work permit bago magsimula sa paggawa.)

    2. Only authorized supervisor is allowed to process work permit (Safety Cert./Clearance Cert.)(Ang mga supervisor lamang na may alam sa trabahong gagawinang pinapayagang kumuha ng work

    Radiation Safety

    (Kaligtasan Pamaraan Laban sa Radiation)

    1. Secure work permit and ionization radiation permit before performing a radiation activity. This must beapproved by concerned parties (Beezy Seven, Inc. and subcontractor, if there is any).(Bago magsimula sa pag X-ray ay dapat mayroong kaukulang permits (WP & IRP) ng Beezy Seven, Inc. atng subcontractor.)

    2. Proper handling, transportation and use of radioactive source must be observed at all times.(Pag-iingat sa pagdadala, pagbibiyahe at paggamit ng radioactive source ay dapat sundin.)

    3. Install required barricades and warning signals strategically at all working areas that may be affected ofradioactive source.(Lagyan ng barikada o babala ang bawat lugar ng radiation source.)

    4. Do as much radiography on off hours possible.(Gawin lagi ang pag x-ray sa oras na wala nang ibang nagtatrabaho.)

    5. Have at least one (1) observer outside of barricaded area to keep others away (usually done by one ofradiography personnel).

    (Maglagay ng tagabantay sa labas ng lugar na binabarikada.)

    6. Secure an Ionization Radiation Permit for every distance of 3 meters radius on all joint to be shot.(Kumuha ng Ionization Radiation Permit sa bawat tatlong (3) metro ang distansya sa lahat ng joint nakukuhanan ng x-ray.)

    7. Only authorized and qualified personnel are permitted to conduct radiography.(Ang awtorisado at kwalipikadong manggagawa ang pinapayagang magsagawa ng radiography.)

    Fire(Sunog)

    1. Be prepared. Ensure that you know what to do in case of fire.(Maging laging handa at alamin kung ano ang dapat gawin sa oras ng sunog.)

    2. Make certain you know the escape route established and the assembly point.(Alamin ang lagusang daraanan at ang pagtipon-tipon sa itinakdang lugar.)

    3. Dont obstruct access to fire extinguishers; learn to operate them.

    (Huwag lagyan ng anmang bagay na makakasagabal sa pagkuha ng kasangkapang pamatay-sunog; alaminang paggamit nito.)

    4. Do not smoke in forbidden areas.(Huwag manigarilyo sa mga lugar na walang pahintulot.)

    Mechanical and Hand Tools

    (Mekanikal at Kagamitang Pangkamay)

    1. Provide covers or guards on exposed gears and other moving parts.(Lagyan ng takip ang mga nakalabas na gears at mga gumagalaw na parteng makina.)

    2. Always use the correct tools for the job.(Gumamit ng tamang kagamitan sa bawat uri ng gawain.)

    3. Keep hammer heads tightly wedged on their shaft.(Panatilihing maayos ang pamukpok ng martilyo sa kinalalagyan nito.)

    4. Never use tools and equipment which are damaged or excessively worked.(Huwag gumamit ng mga kasangkapang may pinsala.)

    5. Never make temporary repairs to equipment.(Huwag gumawa ng pansamantalang pag-aayos sa mga kagamitang mekanikal.)

    Heavy Equipment Cranes

    (Makinaryang Mabibigat Cranes)

    1. Never ride on hoist, slings, or loads; it is strictly prohibited.(Hindi dapat sumakay sa hoisr hooks, slings, o sa kargamento; ito ay mahigpit na ipinagbabawal.)

    2. Ensure the area is clear of people before any lifting. Provide barriers and entry signs.(Siguraduhing walang tao sa paligid bago panimulan ng pagbuhat, maglagay ng barikada at karatulang

    bawal pumasok.)

    3. All crane hooks must have safety latches.(Lahat ng crane hooks ay dapat mayroong safety latches.)

    4. Never hitch a lift in a hoist bucket.(Huwag umangkas sa hoist bucket.)

    5. Do not allow anybody to stand under an elevated load.

    (Pinagbabawalan ang sinumang tumayo sa binubuhat na kagamitan.)

    6. Heavy equipment must only be operated by trained personnel.(Ang mga cranes ay kinakailangang patakbuhi ng mga dalubhasang tauhan.)

    7. The heavy equipment must be inspected and approved prior to use at the construction site.(Ang mga makinaryang mabibigat ay kinakailangang dumaan sa inspeksiyon at aprubado ito bagogamitin sa construction site.)

    Excavation

    (Paghuhukay)

    1. Always use ladders to get into and out of excavations. Dont climb on thetimber and never jump across.(Laging gumamit ng hagdan sa pagbaba at pag-akyat. Bawal umakyat sa pormang kahoy. Huwag lundagan

    ng patawid ang hukay.)

    10.Always fence off excavations and clearly indicate their existence with adequate signs.(Laging lagyan ng harang at karatulang pangkaligtasan kung saan may hukay.)

    11.All trench must have the approved tag before anyone goes down. RedTagmeans only qualified shorer areallowed to work. Green Tagindicates that shoring is complete and safe to go down the trench.(Ang hukay ay dapat may approved tag bago bumaba ang isang manggagawa. Ang kahulugan na Red Tag

    ay ang mga qualified shorer lamang ang maaaring gumawa sa ibaba. Ang Green tag naman ay nagbibigaypahintulot kahit kanino na maaari nang bumaba sa hukay.)

    Scaffolding

    (Iskapold/Pala-pala o Tuntungan)

    1. All scaffolds must be tagged prior to use. Red tag indicates that scaffold is being erected, dismantled or isnot to be used. Green tag indicates that scaffold is okay for use.(Ang mga iskapold ay dapat may tag bago gamitin. Ang pulang tag ay nagsasaad na ang iskapold ay

    binubuo, kinakalas at hindi dapat gamiyin. Ang berdeng tag ay nagsasaad na ang iskapold ay maaaringgamitin.)

    2. Suitable handrails for personnel shall be installed on all scaffolds. Guardrails and toe-boards must beinstalled.(Kailangang may angkop na gabay ang mga iskapold. Ang guardrails at toeboards ay dapat ilagay.)

    3. Scaffold must be fully planked, properly supported and properly tied off.(Ang iskapold ay kinakailangang kumpleto ang plataporma at suportado at nakatali ng maayos.)

    4. Rolling scaffolds shall not be moved with anyone on it. Remove or secure all tools and materials beforemoving.(Amg iskapold na may gulong ay hindi dapat galawin kapag ito ay may tao. Alisin o itabi ang lahat ng gamitat materyales bago galawin.)

    5. Wheels of rolling scaffolds must be locked.(Kalsohan o ipirme ang mga gulong ng rolling iskapold.)

    2. Speeding and overtaking on the road is prohibited.(Walang dapat lumabag sa itinalagang bilis at walang dapat makipag-unahan sa daan)

    Seat belts must be worn when the vehicle is in motion.(Isuot ang seat belt kapag tumatakbo ang sasakyan.)

    4. Never reverse your vehicle without assistance.(Huwag i-atras ang sasakyan kung walang tumutulong sa likuran.)

    5. Ensure your lights, brakes and horn are regularly serviced or regularly inspected.(Siguraduhing ang ilaw, preno at busina ng inyong sasakyan ay laging nasa maayos na kondisyon.)

    6. Look around before crossing a road.(Tumingin muna sa paligid bago tumawid sa kalye.)

    2. Always check for cables, plugs, or sockets. If found defective, do not use. Report them.(Laging suriin ang mga sirang kable ng kuryente, plugs o sockets. Kung makikita, huwag gamitin. Kaagadipagbigay alam sa superbisor.)

    3. Never overload electrical equipment.(Huwag magpasobra ng load ng kuryente sa kagamitang electrical.)

    4. If a fuse bows, report it. Do not fit makeshifts.(Kapag sumabog ang fuse, ipaalam ito. Huwag maglagay ng pansamantalang fuse.)

    5. Switch off and disconnect any equipment that sparks or stalls. Tag them.(Patayin at alisin ang kawad sa pagkakasuksok kapag ang makina ay nakitang kumikislap o sobra ang load.

    1. Check the load to be lifted. If the load is too heavy, get assistance. Use hand truck always for heavier

    Marine Safety

    Area must be secured and barricaded when pile driving is in progress.(Ibarikada ang boung lugar bago magsagawa ng pile driving.)

    4. The pile driver must be placed in a stable position and must be properly supported.(Ang pile driving equipment ay dapat nakatayo sa tamang lugar na pantay at kasiguruhan.)

    5. When and where pile driver is in operation, the area must always be cleaned of any obstruction andpersonnel. Loitering around the area is strictly prohibited.
