Emergency Action Plan Location: AUTZEN STADIUM 1. First Responder (defined as someone designated or trained to respond to an emergency) is in charge. Athletic Trainers are trained as first responders. Assess the situation and provide necessary emergency treatment for the patient(s). 2. The First Responder will designate a second person to activate EMS by calling 911. The Second Person needs to provide the following information (ask first responder for specifics if needed): Patient Specific Information Facility Specific Information Injured Person/Patient name Adjacent to 2735 Leo Harris Pkwy Type of emergency Enter into Lot 6 Type of suspected injury Drive between the Moschofsky Center & Autzen to the WEST Gate of the Stadium Present condition of injured person (conscious, unconscious, breathing, pulse, bleeding, etc) Park at top of the ramp (ambulance will not fit down the tunnel ramp). Care being rendered by First Responder Gurney down to the field *DO NOT HANG UP until Instructed by dispatch* 3. A Third person will be asked to get necessary equipment/splints. This may include an AED: During University of Oregon athletic team official practices and competitions the AED will be on the sideline or golfcart with the athletic trainer. 4. A person must be sent to wait for the ambulance to arrive at Entry 6 and direct ambulance to Autzen Stadium’s West Gate. Make sure the gate is open. If possible have another person at the top of the tunnel to help direct emergency personnel to the patient. 5. Once EMS has been notified, the second person can notify the Office of Public Safety at 541- 346-6666 of the emergency and actions taken. 6.     to the EAST side of the Casanova Center (across from the west gate of Autzen)

    Present condition of injured person (conscious, unconscious, breathing, pulse, bleeding, etc)

    Enter through Double Doors (someone will need to man the doors to keep open for entry)

    Care being rendered by First Responder *DO NOT HANG UP until Instructed by dispatch*

    3. A Third person may be asked to get necessary equipment/splints. This may include an AED:

    There is an AED located near the weight room front door in the main hallway. 4. A person must be sent to wait for the ambulance to arrive at Entry 6 and direct ambulance

    toward Autzen Stadium’s West Gate. If possible have another person at the at the Casanova Center double doors to help direct emergency personnel to the patient, have interior door

    propped or have access card available. 5. Once EMS has been notified, the second person can notify the Office of Public Safety at 541-

    346-6666 of the emergency and actions taken. 6. If possible someone should accompany the patient to the Hospital.

    2. The First Responder will designate a second person to activate EMS by calling 911. The Second Person needs to provide the following information (ask first responder for specifics

    if needed):

    West end of the Moschofsky Center Present condition of injured person (conscious, unconscious, breathing, pulse, bleeding, etc)

    There is a ramp that goes down to the field level

    Care being rendered by First Responder Ambulance is NOT able to drive onto the field, but can drive down the ramp onto the field apron.

    *DO NOT HANG UP until Instructed by dispatch* 3. A Third person may be asked to get necessary equipment/splints. This may include an AED:

    There is an AED located inside the open air training room (which is directly underneath the Football weight room) at the NorthEast end of Kilkenny field. It is mounted on the North wall as you enter the “room”.

    4. A person must be sent to wait for the ambulance to arrive at Entry 6 and direct ambulance to

    the West end of the Moschofsky Center. If possible have another person at the top of the ramp with the gate open, ready to direct emergency personnel to the patient.

    5. Once EMS has been notified, the second person can notify the Office of Public Safety at 541-

    346-6666 of the emergency and actions taken. 6. If possible someone should accompany patient to the Hospital.

    3. A Third person may be asked to get necessary equipment/splints. This may include an AED:

    There is an AED located in the Moschofsky Center Entry across from the restrooms. 4. A person must be sent to wait for the ambulance to arrive at Entry 6 and direct ambulance to

    the entrance/door closest to the patient. Make sure the door is propped open. 5. Once EMS has been notified, the second person can notify the Office of Public Safety at 541-

    346-6666 of the emergency and actions taken. 6. If possible someone should accompany patient to the Hospital.

    Enter gate on the West end of Pape Field (have spotter at entry closest to patient).

    Care being rendered by First Responder *DO NOT HANG UP until Instructed by dispatch*

    3. A Third person may be asked to get necessary equipment/splints. This may include an AED:

    During University of Oregon athletic team official practices and competitions the AED will be on the sideline or on the golfcart with the athletic trainer.

    4. A person must be sent to wait for the ambulance to arrive at Entry 5 and direct ambulance to

    the West end of Pape Field. Make sure someone is at the Gate/Entry closest to the patient and signaling EMS to that location.

    5. Once EMS has been notified, the second person can notify the Office of Public Safety at 541-

    346-6666 of the emergency and actions taken. 6. If possible someone should accompany patient to the Hospital.

    Enter gate on the South end of PK Park (have spotter at entry).

    Care being rendered by First Responder *DO NOT HANG UP until Instructed by dispatch*

    3. A Third person may be asked to get necessary equipment/splints. This may include an AED:

    During University of Oregon athletic team official practices and competitions the AED will be in the home dugout.

    4. A person must be sent to wait for the ambulance to arrive at Entry 5 and direct ambulance

    toward the West end of Pape Field. Go past Pape Field to the south entrance of PK park. A spotter needs to be placed at that gate to open it upon EMS arrival.

    5. Once EMS has been notified, the second person can notify the Office of Public Safety at 541-

    346-6666 of the emergency and actions taken. 6. If possible someone should accompany patient to the Hospital.
