Reporting to the Manager of Clinical Services, the Coordinator of Case Management is responsible for the overall coordination of the activities of the Case Management program and team.

QUALIFICATIONS: Education requirements: Completion of a bachelor’s degree in health, social services or related discipline. Consideration will be given to those who are working towards completion of a degree. Professional Designation/Certification (one of the following are required):

I. Registration, in good standing, with a regulating professional body (e.g. OCSWSSW); or II. Certification, in good standing, with a professional certifying organization* within health

or social service fields (e.g. CPRRP from PSR Canada) *Organization must have a written code of ethics, core values and standards that are consistent with those of CMHA-S/M. Certification must require members to meet a core set of competencies, have a requirement for continuous learning to maintain certification and be approved by CMHA-S/M. Experience: Min. of 2-5 years’ experience working in a health, social services or related

discipline required. Related experience in a leadership role and support services.Experience within case management programs is an asset.

Language requirements: Fluency in English is essential. Fluency in French is an asset. Priority will be given to candidates who are bilingual in French and English as per our French Language Service Plan. Other requirements:

Advanced proficiency with Microsoft Office software (Word, Excel and Outlook) required.

Certified Health Executive (CHE) designation preferred.

Experience with client record management software/database an asset.

Applied Suicide Interventional Skills Training, Safe Talk, Non-Violent Crisis

Intervention Training, and Mental Health First Aid are considered an asset.

Relevant de la gestionnaire des services clinique, le/la titulaire du poste sera responsable de la coordination générale et la mise en oeuvre de toutes les activités du programme et l`équipe de gestion de cas.

COMPÉTENCES : Exigences en matière d’études: Posséder un baccalauréat en santé, en services sociaux ou dans un domaine connexe. Nous examinerons aussi les demandes de candidats qui travaillent actuellement à l’obtention d’un tel diplôme.

Désignation ou reconnaissance professionnelle (un des éléments suivants sont requis): I. Inscription, en règle, auprès d’un organisme professionnel de réglementation (p. ex.,

l’Ordre des travailleurs sociaux et des techniciens en travail social de l’Ontario); ou

II. Agrément, en règle, auprès d’un organisme professionnel d’agrément* oeuvrant dans ledomaine de la santé ou des services sociaux (p. ex., praticien agréé en réadaptationpsychosociale et en rétablissement auprès de Réadaptation psychosociale Canada).

*L’organisme doit posséder, par écrit, un code de déontologie, des valeurs fondamentaleset des normes de pratique qui cadrent avec ceux de l’ACSM-S/M. L’agrément doit exiger que les membres possèdent un ensemble de compétences de base et qu’ils poursuivent un apprentissage continu afin de maintenir l’agrément. L’agrément doit être approuvé par l’ACSM-S/M.

Expérience : Minimum de 2 à 5 ans d’expérience de travail dans un domaine de la santé, des services sociaux ou dans un domaine connexe. Expérience connexe dans un rôle de leadership et des services de soutien. L’expérience des programmes de gestion de cas est un atout.

Exigences linguistiques : Une maîtrise de l’anglais est requise et une maîtrise du français est considérée comme un atout. La priorité sera donnée aux candidats bilingues (français et anglais) selon notre plan de services en français.

POSITION SUMMARY:

Reporting to the Manager of Clinical Services, the Coordinator of Case Management is responsible for the

overall coordination of the activities of the Case Management team and program.

ESSENTIAL JOB RESPONSIBILITIES:

Program Coordination o Develop and maintain program standards, procedures and outcomes for the Case Management

and Intake programs. o Manage referrals to the programs to minimize wait times for service. o Review and update program forms, letters, templates and care plans as required. o Assist staff with resolving client issues and escalate to Manager as required. o Review documentation and care plans. Provide feedback and guidance as required. o Ensure timely case notes and time entry are completed and entered in client database. o Oversee client information and ensure it is updated and maintained in client database. o Ensure OCAN, OPOC, consent forms and support plans are completed by staff. o Develop and review program outcomes, performance metrics and statistics. o Identify special projects and quality improvement opportunities for the program. o Work collaboratively with community partners to improve community mental health and addiction

services. o Initiate, monitor and renew program related service agreements. o Submit regular program reports to the Manager.

Team Coordination

o Provide guidance, direction, mentoring and problem solving to the Case Management Team on client and program related items.

o Direct operations of the team by scheduling staff hours, approving time off requests, assigning and monitoring work, gathering resources and implementing productivity standards.

o Maintain and update staff schedule.

o Identify team and individual learning needs; coordinate training for staff. o Post job vacancies, participate in interviews and provide feedback to the hiring committee. o Develop orientation plans, schedule orientation session items as needed and conduct new hire

orientations. o Coordinate sessional hours and update staff professional designation information as required.

o Conduct supervisions, goal planning and annual performance reviews with staff. o Escalate staff issues to Manager as needed i.e. attendance or performance issues. o Ensure regular communication with staff and Manager.

Other

o Approve one-time purchases within the defined scope for the team and/or program through a purchase order process.

o Provide On–Call support from 4:30pm to 8:30am, 4-8 weeks per year. o Maintain quality-assurance activities by reviewing and evaluating program objectives.

o Internal and external committees support may include: being an active participant and/or lead on committee(s), preparing meeting agendas and distributing meeting minutes; and implementing and following up on action items.

o Ensure monthly, annual and other required reporting documentation are completed. o Maintain Agency’s values, vision and mission by ensuring that clients’ rights are respected and that all

programs and activities meet legislative requirements, policies and procedures, funding body criteria, and professional and Branch standards.

Qualifications

o Completion of a bachelor’s degree in health, social services or related discipline. o Registration, in good standing, with a regulating professional body (e.g. OCSWSSW) is required; or

Certification, in good standing, with a professional certifying organization** within health or social service fields (e.g. CPRRP from PSR Canada).

o 2-5 years’ experience working in a mental health, social services or related discipline required. o 2 years supervisory experience strongly preferred. o Experience within Case Management programs is an asset. o Fluency in English is required.
