Terms and Conditions of “Gleneagles Hong Kong Hospital Premier Health Screening and Prevention Services”:1. To enjoy the offer, customer must submit the application of any plan(s) (excluding the ILAS plan) with designated total annualized first year premium during

the period of 1 July 2019 till 31 August 2019 (“Promotion Period”, both dates inclusive), and such application(s) must be approved by FTLife Insurance Company Limited (“FTLife”) on or before 30 September 2019.

2. The total annualized first year premium is calculated based on 1) the total premium amount of basic plan(s) and rider(s) (if any), or 2) 10% of the single premium of all eligible plan(s) under the same policyowner during the promotion period. Premium levy, prepaid premium (if applicable), loading premium (if applicable) or cancelled policies within the cooling off period is not entitled to the services.

3. All rider(s) must be attached to basic plan(s) applied within the promotion period; otherwise, it will not be calculated. 4. The reward is offered in the unit of each policyowner. Each policyowner can enjoy a maximum of one designated offer under the total annualized first

year premium.5. All eligible policies must be in force at the time when the reward is given out.6. FTLife will issue the redemption letter to eligible policyowner through financial consultant within 1 month after the last approval date. The reward is only

applicable to relevant policyowner or insured (if the relevant policyowner and insured are not the same person) and is not transferable. Also the reward must be used before the last date as stated. Reward cannot be replaced in the event of damage, loss of mail or loss.

7. The stock of HPV “9-in-1” Vaccine (3-dose) is limited and is available on first-come, first-served basis. If the HPV “9-in-1” Vaccine (3-dose) cannot be offered due to shortage of stock, FTLife reserves the right grant an alternative reward of equal value.

8. If any reward cannot be offered for any reasons, FTLife reserves the right to grant an alternative reward of equal value and will contact the policyowner for the arrangement.

9. All services are not transferable, resaleable, and not convertible to cash or other medical service.10. All services are provided by Gleneagles Hong Kong Hospital. Once the redemption letter is issued, FTLife shall not be liable or responsible for all matters

related to the “Gleneagles Hong Kong Hospital Premier Health Screening and Prevention Services” Reward (including but not limited to the quality, applicability, supply or service). All disputes arising from the “Gleneagles Hong Kong Hospital Premier Health Screening and Prevention Services” Reward should be resolved by the customer and the relevant service provider. FTLife assumes no obligation or responsibility for the disputes.

11. The customer should understand or seek the advice of professional medical doctors and consider their own needs and physical conditions before receiving the health screening test or vaccination services.

12. FTLife reserves the right to make all final decisions on the approval of the relevant policy application(s) and the Promotion.13. FTLife reserves all the rights to suspend or terminate the reward or amend the terms and conditions of the reward at any time without prior notice.14. No person other than the customer and FTLife will have any right under the Contracts (Rights of Third Parties) Ordinance to enforce or enjoy the benefit of any

of the provisions of these terms and conditions.
