Introduction

The purpose of the Exact Mining Service General Induction is to ensure the effective integration of employees into the workplace. This induction intends on familiarising employees about the culture, acceptable practices and performance standards of the Company. The General Induction covers three key areas: - Human Resources; - Work, Health and Safety (WHS); and - The Environment. Duration: Approximately 30 minutes Assessment: Online Assessment Please note, you may be required to undertake additional site and job specific inductions and training.

‘Rules to Live By’ Never: 1. Be under the influence of alcohol or prohibited drugs. 2. Fail to follow company policies and procedures. 3. Fail to assess and control all risks prior to performing a task. 4. Fail to wear personal protective equipment suitable for tasks to be performed. 5. Knowingly undertake or give an instruction to perform an unsafe act. 6. Inspect or Work on equipment without isolating it first, by applying your personal

isolation lock. 7. Remove any personal tag or lock belonging to another person without

authorisation. 8. Knowingly operate equipment that is in an unsafe condition. 9. Operate equipment or undertake a task without appropriate training and

authorisation. 10. Work at heights or confined spaces without the correct training, equipment and

PPE. 11. Enter a barricaded area without authorisation. 12. Fail to report any incident, hazard or near miss immediately. 13. Work under unsupported ground or suspended loads. 14. Knowingly leave unsafe equipment without isolating with an ‘Out Of Service Tag.’

Corporate Policies and Procedures Exact Mining Services has implemented Policies and Procedures to ensure that all employees, contractors and visitors understand the acceptable practices and processes that manage the Company’s daily operations. These Policies and Procedures form our WHS Management System Framework and HR Management System Framework. All Employees should be familiar with these as they detail the core framework for employer and employee responsibilities. Exact Mining Services develops all Policies and Procedures in accordance with applicable legislation and regulations. Legislation that governs the Mining Industry includes: - Relevant State / Territory based Work, Health Safety (WHS) or Occupation Health,

Safety and Welfare Acts and Regulations (OHSW) - Relevant state based Mining Acts and Regulations - Fair Work Act (2009) - Equal Opportunity Act (2009) Policies and Procedures can be accessed via the Quality, Health, Safety and Environment (QHSE) system or via site safety and administration personnel.

- Highlights Exact Mining Services commitment to achieving a high level of pro-active WHS management in line with the Company’s organisational vision of zero harm.

- This policy aims to ensure: • A safe working environment; • Safe systems of work; • That plant and equipment is in a safe

condition; and • Employees are provided with information,

instructions, training and supervision necessary to improve safety in the workplace.

- Aims to minimise the potential occupational health and safety risks associated with employees whose fitness for work has been compromised due to their health or other factors.

- Highlights Exact Mining Services commitment to providing a safe, harmonious and productive work environment where grievances are dealt with sensitively and confidentially.

- Exact Mining Services ensures the effective management of grievances through a process of discussion, cooperation and conciliation.

EMERGENCY ALERTS & EVACUATIONS Familiarise yourself with site specific emergency documentation (Plans and Maps). On Exact Sites Emergencies are commonly communicated via UHF or VHF channels with the phrase:

“EMERGENCY-EMERGENCY-EMERGENCY” On hearing your site specific emergency alert ensure that you follow the designated plans and all relevant instructions.

KNOW WHERE YOUR EMERGENCY ASSESMBLY POINT IS AT ALL TIMES

Requirements

• To eliminate or minimise the risk of fatalities, injuries and incidents arising from the uncontrolled release of energy or hazardous materials.

• Applies to the isolation of all sources of energy (electrical, mechanical, hydraulic, chemical, gravitational, pneumatic, kinetic, stored energy etc.)

• Applies to Exact Mining Services sites and controlled activities, and to all Exact employees, contractors and visitors.

• Irrespective of whether you are inspecting or working on a machine it needs to be isolated, locked out and ‘Test for Dead’. This applies to all staff; including Managers.

Isolation & Lockout Procedure

Personal Danger Tag • There should be a Personal Danger Tag for each

person working on the machine. • If you find a Personal Danger Tag which has not been

removed from an item of plant do not start or operate that machine, even if you have been instructed to do so.

• The only person who can remove a Personal Danger Tag is the person who has their name on the tag. Unauthorised removal of another person’s Personal Danger Tag shall result in disciplinary action.

Isolation & Lockout - Cont

Caution – Out of Service • Use a CAUTION – Out of Service TAG to prevent

people from operating or using faulty equipment.

• The tag needs to identify the equipment fault and person applying the isolation.

• This tag stays on the isolation point until the job is finished and can only be removed by an authorised person once the repairs are complete.

• This tag is NOT to be used an alternative to your Personal Danger tag if you are working on the equipment.

• Out of service locks and tags can only be removed by workshop personnel. If tag is found on machine at start of shift call supervisor and report it.

Out of Service

• Zero tolerance to Drugs & Alcohol is EMS Policy. Any breaches will be

subject to disciplinary action.

• Smoking is restricted to designated areas only.

• Mandatory daily alcohol testing is conducted by Exact Mining Services and the relevant Client.

• Random drug testing is conducted by Exact Mining Services.

Fitness for Work

Fatigue Employees of Exact Mining Services shall not exceed the outlined requirements below without permission from the Area Manager or Project Manager: 13 hours per day for all employees excluding road transport drivers. 14 Consecutive shifts for all employees excluding road transport drivers. If you are working

additional shifts past your 14th consecutive day, you must have the following day as a rest day (24 hour period) offsite prior to recommencing any shift.

Maximum hours and shifts in accordance with fatigue management legislation for road transport drivers

All employees must also comply with any of Exact Mining Services and Client fatigue management policies, procedures and manuals. EXACT MINING SERVICES RESPONSIBILITIES AND COMMITMENT In order to assist with fatigue management Exact Mining Services will: Provide information relating to fatigue Assess the foreseeable risk of fatigue in the workplace on an ongoing basis. EMPLOYEE RESPONSIBILITIES All employees are responsible for: Maintaining their personal health and fitness for work. Ensuring that they do not undertake their duties if their decision making, judgment, concentration or

coordination is knowingly affected. Notifying their manager immediately if they are feeling the symptoms of fatigue.

• Headache • Dizziness • Rapid Pulse • Skin flushed and dry • Cramps • Nausea & Vomiting Seek immediate medical attention!! Heat Stroke can be fatal!!!!

Symptoms of Heat Stress/Stroke

PPE Exact Mining Services has a ‘Dress Code’ policy (HRPOL-003) as a minimum standard. In-conjunction with this policy there may be some site specific PPE requirements . Dress Code includes:

• Hi-Visibility Long Sleeve Shirt – Sleeves rolled down and buttoned up

• Lace Up Safety Boots covering the ankle and to comply with Australian Standard AS2210

• Hard Hat (must be worn in all areas except around crib room and administration areas)

• Safety Glasses

• No Jewellery of any description to be worn on site. E.g. wedding rings

• Baseball caps are not to be worn under Hard Hats

• Hair to be kept neat & tidy

Remember: PPE signs are ‘MANDATORY’ and have the same authority as an instruction from a

Supervisor. It is the responsibility of each person to ensure they use the appropriate PPE provided.

Jobs requiring the manual handling of rigging equipment should be carried out wearing leather gloves.

PVC chemical resistant gloves are suitable for working with chemical wash down solutions.

Gloves shall be worn whenever performing a task involving manual handling.

Gloves or any item of clothing should be kept clear of moving parts of machinery. Gloves are NOT to be worn when working with rotating equipment. e.g. drill press, grinder (Cairn

Hill).

Gloves must to be carried on your person at all time.

Cut rated gloves must be worn for certain approved cutting tasks.

Risk assessments are compulsory under WHS legislation throughout Australia. This applies to plant, tasks, hazardous substances and workplace change.

All Incidents must be reported immediately. If you have suffered an injury whilst at work or outside work, please report the injury details to your supervisor and/or site safety personnel. If injured whilst working at Exact Mining Services, all employees are to be accompanied by an EMS representative to all medical appointments / assessments.

MANUAL HANDLING Manual handling of materials is the largest single cause of industrial accidents. Mechanical or hydraulic lifting aids should be used at all times if the object or material is considered too heavy.

Use caution when carrying long materials such as piping or ladders. DO NOT carry materials above shoulder height and or in such a manner which can obscure your vision. Bend at the knee not your back.

Follow these basic instructions when lifting:

• Examine the object - determine its weight and look for sharp edges, check load is stable and decide how to hold the object.

• Ask for help or use a mechanical aid if in doubt about moving the object yourself.

• Plan the job - ensure a route free from tripping and slipping hazards and know where the object will be unloaded.

• Ensure correct protective equipment is used: gloves, safety shoes, etc.

Intersections on site are generally controlled by STOP signs, which apply to all vehicles. All personnel onsite are to abide by regulatory signage at all times: STOP GIVE WAY ENTER ONLY EXIT ONLY SPEED ONE WAY CALL UP SIGNAGE SERVICES SIGNAGE

Maximum Haul Road speeds (unless otherwise sign-posted or site specified): 60km for Light Vehicles (LV) 40km for Heavy Vehicles (HV) 10km in office and workshop laydown areas

Common Signage

MOBILE PHONES & ELECTRONIC DEVICES • Mobile phones must never be used whilst operating machinery or

undertaking any task within the workshop. (This includes: cameras, texting, social media, phone calls etc)

• Mobile phones should be left in your locker and not carried on your

person unless you are a Supervisor and authorised to carry a mobile. • Please provide your family with your Supervisors mobile number in the

event you need to be contacted in an emergency. • Please refer to any site specific requirements regarding mobile phones,

as some clients ban mobile phones from the workplace altogether unless you are a supervisor/manager with a company phone and authorised to have a phone onsite.

• Only ear plugs or ear muffs are permitted to be worn whilst operating

machinery. All electronic devices such as iPods and MP3 players must have an extension speaker or run through the AM/FM radio. Under no circumstance are you allowed to replace your hearing protection for ear-phone speakers.

• You must still be able to hear the UHF/VHF radio above any music.

ALWAYS DRIVE TO THE CONDITIONS Never: • Jump off equipment – always use ‘3 Point Contact’

• Lean out of, enter, or leave a Haul Truck being loaded

• Enter a Loader area unless the bucket is grounded

• Approach a Haul Truck whilst being loaded/tipping

• Attempt to alight from moving equipment

• Enter the area between operating equipment and digging face

• Travel as a passenger unless seating is provided

• Use a windrow as a ‘STOP’ unless in extreme emergency

Ensure: • All People and Vehicles are to be at least 50 meters from all operating plant. • When Hot seating through breaks etc. acknowledgement to enter the work area must be obtained before proceeding • Height of windrow must be at least half the diameter of largest truck wheel on site • Parking of trucks nose to tail is prohibited • Windows are wound up at all times. • When working in the Pit, stockpile area or tip heads, a minimum of 5 metres must be kept between all plant and

positive radio communications must be maintained at all times

QHSE is the Quality, Health, Safety & Environment Management system that Exact Mining Services utilises and is available to all staff via the Intranet Portal. QHSE is also utilised for the following:

Housekeeping Return all tools to their correct location after use. Roll up hoses and extension cords after use. Keep demarcated access ways clear at all times. Ensure access to safety equipment is not blocked. Clean up spillage when it occurs. Put scrap materials in the designated bin. Store and stack materials in an orderly and safe manner. Keep flammable liquids segregated & away from heat sources. Keep all vehicles clean and tidy. Ensure that meeting rooms are tidy following meetings. Ensure kitchens and crib rooms remain tidy at all times.

Due to the fragile nature of the environment:

Do not drive off designated roads/tracks unless authorised

Do not remove/clear any vegetation away unless authorised

Haul roads are to be watered to contain dust

Oil and/or chemical product spills are to be cleaned up immediately and reported

Endangering wildlife is prohibited

Access to sacred sites is prohibited
