SUDDEN CARDIAC ARREST Rs. 11,610/- Sudden Cardiac Arrest (SCA) is different from a Heart Attack. A heart attack blocks blood to the heart while a SCA STOPS the heart. It stops the heart thereby leading to death within minutes. This is caused by electrical disturbances of the heart where in the heart fails to function at a very fast heart rate. SCA kills approximately 4-5 million Indians every year. Hinduja hospital is the only hospital which provided specialized tests like ECG with late potentials, T- wave Alternans and Heart Rate Variability for assessing the risks of SCA. Package Scheduling: The SCA package is available on all days except Saturdays. It is a three visit package which includes the following: 1. On the first visit, the Executive is to undergo an ECG with late potential and 2 D Echocardiogram. After

which, the Holter monitor will be attached to the Executive 2. On the second visit, the Executive is to reach Health Check reception exactly 24 hours after Holter

monitor was attached, in order to remove the Holter apparatus. After which, the Executive will undergo the Stress test.

3. On the third visit, with a minimum of a days gap, the Executive is to collect the reports from the Health Check dept. before the consultation with the Cardiologist

IMPORTANT INSTRUCTIONS, PRE-REQUISITES FOR HEALTH CHECK

1. Kindly book an Appointment to ensure your choice of package, we recommend that you consult your family doctor in deciding an appropriate package based on your age, gender, medical condition.

2. You are required to report at the stated time to the Health Check Reception, 3rd floor, Hinduja Clinic, building to avoid delays in the day’s scheduling.

3. You can make an advance payment OR pay for the package on the day of Health check (Home collection) by Cash / Credit Card / Debit Card. Cheque, Demand draft, NEFT will not be accepted. Executives coming to the hospital for the first time are required to register at an additional charge of Rs. 200/-. 4. It is mandatory for Company sponsored Executives to bring a duly signed Authorization letter 5. If instructed to report fasting, please do not consume any food or drink from 10.00 pm the night before

your Health Check (including morning tea, green tea with or without milk, sugar, plain water with lemon, honey etc.)

6. Do not consume alcoholic beverages, including wine and beer, 72 hours prior to the Health Check. 7. Please bring your Stool sample on the day of check-up in Hinduja hospital container. If you haven’t

collected the Health Check up kit beforehand, you are required to bring it in a sterilized container and transfer the sample into our hospital container with the help of a plastic spatula provided to you or give the sample at the Hospital on the day of your package.

8. Please do not bring a Urine sample from home, as it needs to be deposited as close to the blood collection as possible for accuracy of test results.

9. Females kindly ensure that you do not take an appointment for a Health Check on the days of menses (periods) as PAP smear test and Urine Routine will not be carried out.

10. Kindly carry your past Medical Records (especially Mammography reports, films etc.) and Current Medications along with you for our Doctors to review (if need be), advice.

11. Kindly fill the On-line Medical History Form (applicable for HH registered patients), link available on our web-page - www.hindujahospital.com and save time on the day of your package.

12. Please have your blood pressure checked by your family doctor the day before your health check. 13. If you are known hypertensive, diabetic, have thyroid, please take your medication as per usual routine,

you can bring the medication along with you on the day of the package, our Nurses can advise if need be. 14. Please wear comfortable attire, foot wear and avoid jewelry on the day health check. 15. If you require a Stress test, kindly carry sports shoes. Male executive might be required to shave their

chest (collar bone to abdomen), kindly do the needful, to facilitate Stress Test leads sticking to the Chest. 16. If you require Ophthalmic consultation, kindly bring your Eye Glasses and remove your contact lenses (if

applicable) a day prior to the Health Check up 17. If you require an Audiogram / ENT consultation, please get your ears cleaned of wax prior to health check

to avoid repeat ENT consultation at an additional cost. 18. If you require a Mammography test a written prescription from a registered Physician is required for a

lady below 40years of age or if the same has been conducted within a year. 19. Most investigation reports will be made available on the day of package, via email as well (please update

your mail ID during Billing. Further, the reports can be accessed on our web-site for until a month after your Health check. Pending package reports (most likely Mammography, Sonography, PAP smear, Hepatitis B) will be sent to you later.

20. Typed medical summary reports, will be sent to you by courier (within Mumbai) and by registered post (outside Mumbai) after 3 working days, kindly update your address with us.

21. Any additional test / investigation / consultation done on the same day/s of the Health Check will be charged at a 10% discount. This is not applicable for Add-On packages.

22. One relative, accompanying the Executive, can avail of the Breakfast and Lunch facility for an additional charge of Rs. 210/-

23. All the rates are subject to change without prior intimation.
