Behavior Policy
All our staff are trained and dedicat-ed to serve you and you will be treated with courtesy and respect at all times. In return, we ask that you and any-one that you bring with you to the Practice treat our administrative and clinical staff with the same courtesy and respect. We have a ZERO TOLERANCE ap-proach to any verbal, aggressive and violent abuse and behaviour towards our staff or other patients. We respectfully advise you that abuse and/or violence will not be tolerated. If a patient or anyone a patient brings with them to the Practice, is abusive, aggressive or violent to-wards our staff and/or other pa-tients the police will be called and may result in you being removed from our Practice list.

Patients , need to be aware
that at times referrals to
other providers incur a gap
or some out of pocket
costs , or the fee for the
service may need to be paid
up front on the day .
We advise that it is the
patients responsibility to
obtain this information
from the provider.
If you are unhappy with any aspect
of our services, please feel free to
speak to, or write to, your Doctor or
the Practice Manager.

PATIENT INFORMATION & SERVICES
We are located on the western side
of Mt Gambier’s CBD. Ferrers Medi-
cal Clinic offers a wide range of
health care services to the local
community.
Ferrers Medical Clinic is proud to be
involved in GP training and educa-
tion programs involving some-
times , the hosting of Medical Stu-
dents through the Flinders Univer-
sity Rural Clinical School.
We also offer employment of Regis-
trars through GPex Education and
Training program.
Nursing Care :
At times your nursing care or
wound management may be clini-
cally assessed with the clinicians
Long term or complex dressings ,
or those patients with Bariatric
needs may be referred to Commu-
nity Health for home or hospital
care.
Dressing Charges for Consuma-
bles used in the management of
your care will be at your cost and
will be required to be paid in full on
the day.
The Practice only carries a basic
range of dressings and supplies .
Patients need to be aware that
out of pocket costs will be in-
curred for wound management
and or removals.
$30.00 Procedure fee does ap-
ply to all procedures .
Please make sure you ask your
Gp prior to the treatment about
all charges .
Results
Results are obtainable by phone
during practice hours.
Transfer of Medical Notes to another
provider will incur a fee to be paid in
full prior to the transfer.
Phone Calls
Doctors generally do not take
phone calls from patients
while they are consulting. If
you wish to speak to your doc-
tor, the reception staff will
take a message for the doctor
to return your call.
Prescriptions
, a Script phone appointment
will be made for you , this
means your Gp will telephone
you and speak with you about
your prescriptions at a desig-
nated time . A Script appt fee
may apply .
It is still 2 working days after
your script telephone appt for
your script to be emailed to
the chemist of your choice.
( Scripts are no longer collect-
ed from the practice by pa-
tients )
Referrals
A requirement of the Health
Care Act provides that a GP
assessment be carried out pri-
or to a referral to a specialist.
Please discuss any concerns
with your doctor. The practice
regularly liaises with local,
State & Interstate referrals to
meet patient needs
Reminder System
Ferrers Medical Clinic is com-
mitted to preventative care.
We may send you a SMS re-
minder notice from time to
time offering you preventative
health services appropriate to
your care. If you do not wish
to be part of this system,
please let your doctor know.
Information is limited to pa-
tients being advised that the
doctor has indicated the re-
sults are satisfactory or an ap-
pointment is required to discuss
the results.
Interpreter Services
Arrangements are made for pa-
tients requiring interpreter sup-
port. Please request support ser-
vices when you are making your
appointment. Childhood Immun-
isations or catch-up that require
an interpreter are asked that they
contact Community Health , as we
are no longer able to perform
those appts at the practice or ac-
commodate New migrant ap-
pointments at this time .
Hospital Care
If you are admitted to the Mount
Gambier Hospital as a public pa-
tient, you will be cared for by res-
ident doctors throughout your ad-
mission.
Patient Health Information
Patient health information is
stored electronically on the prac-
tice servers, backed up internally
and externally, all protected by
fire-wall and virus software only
with access by secure password.
Authorised release of information
by patients to other providers, for
referral purposes or under legal
direction, occurs using encryption
if available or registered mail.
Consent may be gained for send-
ing accounts or other information
via your email address, by giving
consent you accept that this is
not encrypted .
Request for more information , some-
times your doctor will require further in-
formation from your previous GP to be
able to prescribe medications especially
those that are an Authority Script to com-
ply with the legislation .

for urgent matters only and ap-
pointments can only be made
on the day.
Every effort will be made to ac-
commodate your preferred
doctor and appointment time.
Emergencies will always be giv-
en priority and our reception
staff will attempt to contact
you if you GP has been called
away.
A duty doctor is available at
the clinic for urgent problems
Longer consultation times are
available, so please ask our
receptionists if you require
some extra time.
After Hours Care Arrange-
ments After 6.30pm and
on weekends, the duty doctor
can be contacted by calling
0417 863 508.
Home visits are at times neces-
sary for palliative or very un-
well patients . Upon request
the Doctor will assess the
needs of the patient and deter-
mine if a home visit is needed.
Alternatively you can present
to the Accident & Emergency
Department at the Mount Gam-
bier Hospital.
After hours services do
attract a higher fee.

cash , due to covid.
Dressings and or consumables given
during the management of your
wound or injury, will incur an out
of pocket fee to be paid on the day.
Patient responsibility :
We advise patients to discuss the
out of pocket fees with their Gp
prior to the nursing treatment or
procedure , as additional fees may
apply.
A non rebatable $30.00 procedure
fee will also apply for all procedures.
All vaccines are to be paid in full on
the day .
Refusal to pay for outstanding ac-
counts may result in further action
or restriction to medical services.
ABOUT BILLING : Ferrers Medical Clinic is a Private Billing Prac-tice, that means charges will apply to most appointments requir-
ing payment in full on the same day of the service.

** Please be aware prices of all fees are reviewed in line with other CPI increases, each July , and as such may be changed without notice.
* All workover patients are required to advise us of their claim number for processing , all invoices will be emailed to your nominated employer. If for some reason your work cover claim is rejected or you do not provide us with a claim number , all outstanding accounts will become the financial responsibility of the patient to pay in full.
Medicals do not attract a Rebate from Medicare … dependent on the type of Medical , Prices may vary .
Reports/Forms or documents completed by the GP for Medicals w ill not be released until the account has been paid in full.
We encourage all patients to register their bank account with Medicare . Rebate cheques will not continue into the future, , so patients will need to nomi-nate your bank account details for your rebate to be forwarded to you.
Dr H Jayakody is a Non VR Doctor and those Non VR billings only attracts 80% of rebate from MC .
