Presidents MessageThis past year has certainly flown by. I am happy to announce that Jennifer Parker has joined our Association as our new Executive Director. Jennifer has several years of experience working in the optometric profession, most recently as Executive Director of the Association of Regulatory Boards of Optometry (ARBO). She also served as Manager of AOA’s Sports Vision and Low Vision Rehabilitation Sections and, prior to that, was employed at the Missouri Optometric Association. If you have not met her, I encourage you to introduce yourself. We are excited to have her as part of our team and we are

sure she will be an asset to the AzOA for years to come.

Spring Congress is May 30 to June 1 at the Hilton El Conquistador in Tucson. There will be 15 hours of COPE-Approved CE offered featuring a faculty of distinguished and nationally-known speakers. There will be topics of interest to everyone. Friday’s Dinner Party will have a Chinese theme with Chinese Lion and traditional Chinese professional dancing to entertain us.

Jennifer Parker, CAE, Joins AzOA as New Executive Director

Jennifer Parker, CAE, has been selected as the new Executive Director of the AzOA.

Jennifer comes to Arizona from St. Louis, MO, where she previously served as the Executive Director of the Association of Regulatory Boards of Optometry (ARBO). With over 20 years of nonprofit association experience, her background includes experience in the optometric profession at the Missouri Optometric Association and at the American Optometric Association, where she served as the Manager of the Low Vision Rehabilitation and Sports Vision Sections.

Jennifer is an honors graduate of Columbia College, where she earned her A.S. and B.S. in Business Administration with emphasis in Management. She also earned her CAE (Certified Association Executive) designation conferred by the American Society of Association Executives (ASAE). In addition to ASAE, her other memberships include, Meeting Professionals International (MPI), the Society of Nonprofit Organizations, the Society of Human Resource Management, and the International Association of Optometric Executives. Jennifer and her husband, Howard serve as Membership Officers of the Gateway to the West Harley Owners Group.

“I am thrilled to be in Arizona and at the AzOA,” says Mrs. Parker. “I am really looking forward to meeting everyone at the Spring Congress.”

Wayward Medical Records Sold as Scrap Paper for $20 From the Yikes Dept.: When an enterprising Salt Lake City fourth-grade teacher paid $20 for scrap paper for her students, she walked away with a box of confidential medical records from a Florida hospital, reports the Deseret Morning News.

An investigation of the case of the wandering records is inconclusive. But this much is known: Central Florida Regional Hospital shipped three boxes of medical records in December via UPS to a Las Vegas auditor, the paper reports. When one of the boxes didn’t arrive, the auditor contacted hospital officials who, in turn, got in touch with UPS a couple of weeks later. A total of 28 patient records containing detailed medical histories, phone numbers, addresses, Social Security numbers and insurance information ultimately turned up at the scrap paper sale in Utah. Several files were for deceased patients.

Shipping companies often sell off packages that can’t be delivered, but a UPS spokesman told the Morning News that the company keeps packages for at least three months before getting rid of them. Hospital spokesman Craig A. Bair said officials were in contact with the shipping company and were looking to make sure the problem isn’t repeated. “Boxes of that type are shipped like that by hospitals all over the country all the time,” he said.

Patient awareness of computer-related vision problems and demand for care of such problems is virtually exploding. With computer use among patients certain to grow in the future, we have only reached the tip of the iceberg.

Many optometrists probably have thought about providing computer vision care practice. As with the addition of any service, it requires some time, some effort, and perhaps some expenditures, but it can be done. Here is a 9-point checklist to follow:

1. Add questions about computer use to the patient history form. Specific

2. Set up a computer-friendly office.

3. Invest in specialized equipment.

4. Begin the education process from the time the patient enters the reception area.

5. Involve staff in the process.

6. Educate the community on computer vision care.

7. Pitch the story of computer vision care to the media.

8. Stay abreast of the latest literature and attend professional seminars.

9. Remind patients that their children also need to be observed for problems.

Computer software to spot signs of glaucoma earlier than conventional tests is being developed by UK experts. Story from BBC NEWS:

The team at London’s Moorfields Eye Hospital say the test has the potential to prevent many patients from going blind. Diagnosing glaucoma can be difficult, as patients are often not aware of symptoms until a great deal of useful sight has been permanently destroyed. It is estimated glaucoma affects 67m people worldwide, including 500,000 in the UK - but only half are diagnosed. This has led to glaucoma being dubbed the “silent blinding disease”. It is estimated that if just 10% of UK glaucoma cases were detected and treated earlier it could save up to £1bn a year. If diagnosed in time, the condition can be easily treated with eye drops. The condition causes damage to the optic nerve which carries visual information from the eye to the brain.

Over the internet

The Moorfields Motion Detection Test (MDT) is designed to assess the field of vision. The software can be downloaded to a laptop computer, and eventually it is hoped to make it available directly from the internet. A central white spot and several white lines are displayed on a grey screen. The patient is asked to look steadily at the central spot and to press the computer mouse each time one of the lines is seen to move. The lines move at the same speed but move different distances as the test

Test To Spot Early Glaucoma Signs proceeds, meaning experts can detect the degree of visual loss. Moorfields say the test is affordable, portable, quick - and has the potential to spot glaucoma earlier than conventional tests, and with greater accuracy. Professor Vis Viswanathan, a consultant surgeon in glaucoma at Moorfields who developed the system, said conventional tests - which concentrate on the ability to see light - fail to pick up a patient’s ability to detect movement. However, the ability to perceive motion is one of the first things to vanish in people suffering glaucoma. He said: “A better test would be based on the ability to perceive motion and that is how this test came about. “In general terms, if somebody is perceiving very small amounts of motion, they are in pretty good shape.” Steve Winyard, from the RNIB, said current tests often inaccurately diagnosed a problem in people who did not have glaucoma. He said the new test promised to be more accurate. “This is a valuable step forward,” he said. Raise awareness From April, clinics in Toronto, Rome, Africa and Singapore will be testing the software with the aim of independently verifying how effective it is. Next year, researchers hope to be able to secure funding to roll out the software across the UK. The first World Glaucoma Day is being held on Thursday to raise awareness of the condition. Professor Peng Tee Khaw, a consultant ophthalmologist at Moorfields, said: “By the time people come to us they have often lost a lot of their sight and the damage is permanent. “If we could pick these people up at an earlier stage it would make a tremendous difference to their lives.”

Healthcare Provider CPR Re-Certification By Dr. Shannon Steinhäuser – AzOA CE Chair & Secretary/Treasurer

As of March 1, optometrists who bill Medicare electronically must include their NPIs on all Medicare claims, in addition to any older IDs they may be using. For most of the past year, the Centers for Medicare and Medicaid (CMS) have been allowing ODs and other Medicare physicians to use their older, “legacy” identifiers alone while they obtained new NPIs and made sure the personal information attached to all of the numbers matched correctly.

However, after May 23, Medicare physicians will be required to use only NPIs on all electronic claims submitted to Medicare and all other health care payers. That means any informational discrepancies must be ironed-out before then. Although the deadlines don’t apply to Medicare physicians who file only paper claims, those who send their claims to a clearinghouse that files electronically on their behalf must comply. The AOA is suggesting that ODs:

• Start using the NPI. Send a few small test claims right away using only the NPI to determine if Medicare and other payers will accept it.

• Watch for NPI warnings. Starting last summer, Medicare began issuing informational warnings on claims for physicians whose new NPI information did not match with information attached to their old IDs. Until these discrepancies are resolved, NPI use will result in claims rejections.

• Re-enroll in Medicare if needed. Physicians who find out they need additional Medicare PINs to match to their new NPIs can complete this process through their carriers.

Genetic variations that affect the production of various chemicals in the inflammation pathways could provide more information about age related macular degeneration (AMD), according to research released in the British Journal of Ophthalmology on February 28, 2008.

AMD is a progressive eye diseases which affects the center of the retina, known as the macula, at the back of the eye. This portion of the eye is most specialized for fine, central vision tasks, including reading or driving. It is most common among the elderly, and in developed countries it is a major cause of untreatable blindness.

investigated genes controlling the production and suppression of cytokines, which play an important role in the inflammatory response. They focused on four specific genetic variations, each related to control of expression of certain cytokines.

To this end, they examined DNA samples from people aged 55 or older. 478 were people presenting with AMD and 555 people showed no signs of the disease. One of the genetic variants examined (251A/T) is associated with the gene boosting production of interleukin 8 (IL-8). In previous studies, this genetic variation has been linked to several inflammatory diseases and cancer, and this genetic variant was significantly more prevalent in patients with AMD. The results remained conclusive after normalization for age, sex, weight, and smoking, all of which can specify a higher risk of AMD.

The authors indicate that, in a larger study, their findings could help create a test to screen for AMD genetically, as well as potential directions for development of drugs to control it. In an editorial accompanying the article, opthamology specialists from the National Institutes of Heath and the University of Michigan cautiously commend the results. They note that many further studies are necessary before any direct causal relationship can be confirmed. However, they claim that the explanation is plausible, because the development of AMD has been linked to cooperative effects from genetic and environmental factors. That is, natural changes that come with aging, such as the accumulation of oxidative stress, seem to initially give rise to the disease, which becomes worse with additional immune system activity.

• Keep NPI information up to date. Physicians are required to report changes to their NPI information within 30 days of the change.

Also, visiting the NPI Web site (https://nppes.cms.hhs.gov/) or calling the NPI enumerator at 1-800-465-3203 can determine whether a physician’s NPI information is correct. Calling the Medicare carrier can confirm whether the carrier has the correct information in its system.

After months of veto threats and heated negotiations, President Bush signed into law the FY 2008 consolidated appropriations bill (HR 2764), which included an important victory for optometry and America’s families.

This legislation – signed into law on December 26 -- provides Federal recognition and funding for the AOA’s sight-saving and lifesaving InfantSEE® initiative. A direct appropriation to the AOA for InfantSEE®, totaling more than $430,000, was backed by Senator Robert Byrd (D-WV) who has been learning about the impact InfantSEE® is having in his home state and across the country from ODs in West Virginia.

In addition to the months of hard work by West Virginia ODs, the AOA Washington Office organized well-timed briefings for senators, congressmen and their aides, and other activities to build further support in Congress. Of course, every AOA member, particularly the 7,800 doctors participating in InfantSEE® who have provided as many as 150,000 no cost vision assessments to America’s babies, helped optometry win new friends and admirers on Capitol Hill.

The good news about InfantSEE® is being heard. America’s leaders in Washington, DC now have a better understanding that early detection is needed to reduce the impact of amblyopia and other conditions that may lead to impairments, loss of sight and can even affect spatial and cognitive development. In fact, newly complied data by the AOA shows that the overall need for concern has increased from one in 14 in 2005 to one in nine in 2007, clearly revealing a growing need for early vision examinations in infants.

The funding will support InfantSEE’s mission of encouraging infant eye and vision assessments and offering early detection of potential eye and vision problems at no cost, regardless of income or ability to pay.

In April, optometrists and optometry students from around the country will gather in Washington, D.C., at the 2008 AOA Congressional Advocacy Conference. The goal is to raise awareness of the profession on Capitol Hill and to highlight priority issues. The 2008 Congressional Advocacy Conference will be optometry’s largest and most important advocacy gathering all year, providing the profession with an ideal opportunity to focus squarely on critical legislative issues including: ending the exclusion of optometry from the National Health Service Corps, ensuring children’s readiness to learn vision through comprehensive eye exams and a revision of the Medicaid physician definition to include ODs. As 2008 unfolds, lawmakers will likely begin to consider proposals to reshape and even overhaul the American health care system. With so much at stake, it’s expected that groups with an anti-optometry agenda -- including organized medicine, insurance and managed care companies and Internet contact lens sellers -- will continue to be active on Capitol Hill. Attending this conference from Arizona are Drs. Annette Hanian, Marla Husz, Don Jarnagin, Rand Siekert . Aleta Gong and Executive Director Jennifer Parker.

2008 AOA Congressional Advocacy Conference “Advocacy for Optometry’s Future” Taking Shape

The world lost a great guy on March 25th, 2008. Dr. Donald Shapiro passed away peace-fully at home, surrounded by his family. Don was born in Wampum, PA. on September 13, 1919. Don graduated from the Pennsylvania State College of Optometry in 1940 where he was the senior class president. After college, he enlisted in the US Army and partici-pated in the USO Army war show, which traveled throughout the U.S., as a coordinator. He eventually was sent to New Guinea in the Pacific Theatre of World War II where he was a 1st lieutenant. After the war, he opened his own optometry office in Ellwood City, PA. Don met his future wife, Ellie, in 1951, in Youngstown, Ohio. They lived in Ellwood City

until they moved to Arizona in 1960. Don was a practicing optometrist for more than 50 years. Don bought an optometric practice from a retiring doctor on McDowell Road and the office is still thriving today, run by his son, Gary. Don was loved by his many patients and employees and enjoyed his chosen profession well into his late 70s. His favorite avocation was golf, which he enjoyed at the old Century Country Club, the Arizona Biltmore, and most recently Desert Highlands Golf Club. He played golf for over forty years with his many friends and family. In many instances, he was called, “Coach,” because he could analyze everyone’s swing and give them tips to improve. Don was involved in the Lions Club International, B’nai Brith, and Congregation Beth Israel. Don is survived by his loving wife, Ellie, son Gary and his wife Laurie, and daughters Karen and her husband Ron Nachtwey, and Sue and her husband Robert Lyle. His four grandchildren, David, Dorie, Blake, and Samantha will miss him greatly.

Carlsson Pediatric & Family Eye Center celebrated its one year anniversary back in December of 2007! Made it through the first year!
