Frequently Asked Questions Information bookletImportant information for pensioners of Istituto Nazionale della Previdenza Sociale (Inps) who receive their pension payments abroad.

Why am I receiving this letter? Inps has chosen Citi to perform pension payment services beginning on 1 February 2012. As a global banking leader in providing financial services and the execution of pension payments, Citi will help to ensure punctual, efficient and a secure service. Until the end of January 2012 your pension will continue to be paid by the current provider, according to the procedures used until now.
What do I need to do now?In order to assist us correctly executing the payment of your pensions, we ask you to complete the Life Certificate Form (yellow form) attached to this letter. Filling out this form is fast and easy, just follow the directions below:
1. Please fill out the form in BLOCK CAPITALS using black ink.
2. Then you must sign the form, having the signature authenticated by:
an official of the Italian Consulate (or Italian Embassy) in your country; or
a public official recognised by the legislation of your country of residence, who has the qualifications indicated in the table further in this document.
3. Please also supply ONE of the following supporting documents (which will not be returned):
a photocopy of a valid photo-identity document (passport, identity card or driving licence); or
a copy of the first page of a recent bank statement for your current account; or
a copy of a bill with your name on it (e.g. gas, telephone, electricity).
The Life Certificate Form (yellow form), together with the copy of the requested documents, must be sent no later than 2 April 2012, using the pre-addressed envelope attached to this letter (alternatively, you may send the documents to the following address: PO BOX 69957 London E14 1NJ United Kingdom).
Should you require assistance to fill out the form, you may also contact a Patronato recognised by the Italian Government.
Why do I have to complete and sign the Life Certificate Form (yellow form)?Citi must verify the life existence of all

May I share or photocopy the Life Certificate Form (yellow form)? No. This Life Certificate Form (yellow form) is unique for each pensioner and is intended to be used exclusively by the same pensioner. It cannot be shared or photocopied. In the event that you need a new Life Certificate Form (yellow form), please contact the Citi Inps Pensioner Support Team.
Where must I send my completed Life Certificate Form (yellow form)?For your convenience, we have included a pre-addressed envelope to facilitate delivery. Alternatively, you may send the completed Life Certificate Form (yellow form), together with the requested documents to the following address: PO BOX 69957 London E14 1NJ United Kingdom.
When is the deadline for sending the completed Life Certificate Form (yellow form)?For the purpose of timely payment execution by Citi, the Life Certificate Form (yellow form), duly completed, signed, accompanied by the requested documentation, must be sent, no later than 2 April 2012.
What will happen if the Life Certificate Form (yellow form) is not sent by 2 April 2012?Failure to return the Life Certificate Form (yellow form), duly completed, signed and accompanied by the requested documentation, could result in suspension of your pension payments starting on 1 May 2012.
Who can authenticate my Life Certificate Form (yellow form)?The Life Certificate Form (yellow form), may be authenticated by an official from the Italian Consulate/Italian Embassy, or alternatively by a public official recognised by the legislation of your country of residence.
The table below lists the public officials recognised in your country or geographic area, qualified to authenticate the Life Certificate Form:

Under exceptional circumstances - if the pensioner is hospitalised in a public hospital, imprisoned, or a resident of a public rest home, unable to move independently - the authentication may be performed by an official of the hospital or institution where the person is living, such as, for example, the Prison Warden, the Director of the Rest Home, to the extent as they are public officials recognised by the laws of the country of residence.

Can I still receive payment to a bank account held jointly with another person?If you receive your pension in an account held jointly with one or more persons, you will have to fill out, sign and have the co-holders of the account sign the Joint Account Declaration as well, which must be returned to Citi no later than 15 December 2011.
The form and instructions for filling out the Joint Account Declaration are attached to this notice. The Joint Account Declaration must be completed and signed by the person or persons who hold the bank account jointly with you. The declaration, once completed and signed by you and any co-holders, must be returned to Citi using the attached pre-addressed envelope for your convenience or sent to the following address: PO BOX 69957 London E14 1NJ United Kingdom. Failure to submit a declaration of the holding of a joint account for your current account may impede future pension payments to that current account. If you receive your pension payments in more than one current account, Please contact the Citi Inps Pensioner Support using one of the methods indicated in this booklet.
Do I need to go to a Citi branch office for any reason? No, there is no need to go to a Citi branch office. Citi will begin to take an active role in paying pensions only from 1 February 2012. In any case, the Citi Inps Pensioner Support Team is already active and available to answer all of your questions, including those pertaining to the Life Certificate Form (yellow form), without the need to visit a Citi branch office. All the telephone numbers are listed further in this document.
If I have a question regarding the current payment of my pension, who should I contact?If you have questions on the current payment of your pension, please contact the bank that is currently handling your pension payment service. Citi will be able to assist you and answer your questions regarding payments that will be made from 1 February 2012 onwards.
