Deductions that may be authorized include: (1) Paying premiums on any policy or certificate of group life insurance or group disability insurance issued by an admitted insurer. (2) Paying premiums for a prepaid group medical or hospital service plan. (3) Paying premiums for a vision care program or dental plan, approved by the board, for the benefit of the retired member or his or her dependents. (4) Paying premiums on national service life insurance or United States government converted insurance. (5) Payment for the purchase of shares in or the payment of money to any regularly chartered credit union. (6) Payment to a charitable organization or a federally chartered veterans’ organization that is approved by the board. (7) Payments to a recognized retiree organization. (8) Payment for the purchase of United States savings bonds. (9) The payment of personal income taxes to the government of the United States or the State of California. (10) Payment for any retiree benefit programs available through the recognized retiree organization. The Board has previously authorized deductions for (1) Paying premiums for a prepaid group medical insurance offered through the County of Fresno; (2) Payments to a recognized retiree organization, specifically the Retired Employees of Fresno County (REFCO); and (3) The payment of personal income taxes to the government of the United States or the State of California (which was extended to include all 50 of the states that collect income taxes). Although statutes allow for the collection of a reasonable fee for the making of the deductions and payments, to date, the Board has not imposed a fee for any voluntary deductions. BackgroundandDiscussionThe California Government Code limits the voluntary deductions that can be made from benefits paidto a retired member of a retirement system authorized under the County Employees Retirement Lawof 1937 (CERL). In recent years, the State Association of County Retirement Systems (SACRS) has developed a visioncare package and is working to develop a dental care plan. Participation in the program does requireadoption of payment of the premium through a deduction from the retiree’s monthly retirementbenefit. In addition, REFCO has contracted with Pacific Group Agencies, Inc. to provide optionalbenefits for its members. The optional benefits include insurance for dental, vision, travel, car, homeowner’s, identity theft, pet care, personal accident & secure travel, hearing aid, and legal shield.Premiums are currently deducted directly from the participant’s bank account. REFCO believes thatparticipation would increase if payroll deduction were authorized.

The County of Fresno provides access to retiree health insurance, including vision and dental, toformer County employees. However, FCERA’s population includes former members of severalspecial district employers as well. And not all former County employees are enrolled in the County’shealth insurance program. The availability of a low-cost vision program would allow those memberswho do not participate in the County’s health insurance program to have access to much neededvision care. In addition, the program offered through SACRS includes enhanced coverage for thoseencountering issues resulting from diabetes or who have hearing issues. I have attached informationon Vision Service Plan (VSP) to this discussion item.

SACRS offers a vision care package to eligible retired members of 1937 Act systems. Many FCERAretirees have expressed interest in the program. For a small monthly premium (negotiated bySACRS), the participant can receive both routine and follow-up medical eye care, including diabeticeye care, retinal screening, and even hearing aid discounts. In order for retired members toparticipate in the program, the retirement system must authorize the use of payroll deduction forthe premium which is paid by the benefit recipient. Currently, two or three 1937 Act systemsparticipate in the program. Once set up, the program is at little to no cost to the retirement system.The retirement system is responsible for the costs to set up the program, custodial bank costs, andissuing the premium check to VSP, all other costs are paid by VSP and the participant.

Kelly Prinz, Retirement Benefits Manager, discussed the SACRS vision care benefit with arepresentative of REFCO to confirm that the program would not conflict with any offerings availablethrough their program. They expressed gratitude that FCERA Administration and the Board wereconsidering this idea. In addition, they have a slate of optional services available to members ofREFCO through Pacific Group Agencies, Inc. (PGA) that they would like the Board to considerincluding as an approved payroll deduction. FCERA staff met with REFCO and representatives of PGAto discuss their program and FCERA’s programming requirements. We have had retirees expressinterest in the program, as well. I have attached information about these services as part of thisdiscussion item.

In addition to these two established programs, SACRS is getting ready to offer a dental programthrough CIGNA Healthcare. A description of the program is not currently available. As with the VSPprogram, the retirement system is responsible for the costs to set up the program, custodial bankcosts, and issuing the premium check, all other costs are paid by the vendor and the participant.

Kelly Prinz, Thanaphat (Pat) Srisukwatana, Systems and Procedures Analyst, Kim Zepeda, Systems andProcedures Analyst, and Becky Van Wyk, Assistant Retirement Administrator, met withrepresentatives of SACRS and VSP to discuss the vision care program and the steps required to bringit to FCERA. FCERA staff also met with representatives of REFCO and PGA regarding their program. Ifthe Board approves the deductions, FCERA staff will work with the vendors (VSP, CIGNA and PGA)and Northern Trust to set up deduction codes and processes to submit the deduction information forprocessing. We anticipate that we will be ready to process the first deduction file for REFCO inAugust or September, 2015 and for VSP in January, 2016. An implementation date is not yetavailable for CIGNA.

Security of Member DataSACRS has contracted with VSP to provide the vision care services. VSP submitted a confidentialindependent auditor’s report completed by KPMG for the period of January 1, 2014 to December 31,2014. FCERA Administration review of the confidential report determined that the key criticalcontrols, including proper physical security protocol to their facility, daily back-up of main-frame,network monitoring tools, password policy, firewalls, encrypted tunnel via VPN to their network, anda disaster recovery plan that includes a warm site for critical business system, were found acceptableby KPMG.

REFCO has contracted with PGA to provide the optional benefits to REFCO members. PGA states thatphysical files are kept locked in locked offices. Electronic files are stored on an in-house secureserver. Access to the electronic files requires two passwords. Access to either the physical files orthe server is limited to those with “a need to access customer files” have been granted access tothem. A back-up of the electronic files is maintained on a secure drive in a safe located offsite of thephysical location of the server.

Security information on the proposed SACRS sponsored dental plan through CIGNA Healthcare is notcurrently available. FCERA will carefully review security protocols before initiating a deduction withCIGNA Healthcare.

The confidential data file exchange that will occur between FCERA, the vendors (VSP, PGA, andCIGNA Healthcare) and FCERA’s disbursement bank, includes encrypting the files with one of thehighest encryption levels and transmitting the files via secured connections and servers.

FCERA is comfortable with the level of security that the vendors provide for their informationsystems and we believe that they maintain industry standards for monitoring, back-up, and security.

In addition, the vendors and/or their third party print/mail shop will sign confidentiality agreementsconfirming their agreement to keep the mailing information confidential and not sell it to or share itwith any third party, whether affiliated or not.

Recovery upon death of participantFCERA staff will immediately notify the vendors upon the death of a member participating in eitherprogram to ensure timely elimination of the deduction. Currently, FCERA requests the return of the“net check” from the bank or estate of the deceased recipient. Sometimes, this means that FCERA isout of pocket insurance premiums, REFCO membership dues and taxes until returned by therespective agency. Recovering insurance premiums is not always possible, resulting in losses toFCERA.

If FCERA were to request the return of the gross check (less taxes) from the estate, the vendorswould be responsible for forwarding recovered premiums to the participant’s estate. This wouldresult in less processing and costs to FCERA.

Cost to set up and monitorFCERA will incur some costs to set up these authorized deductions. In addition to the time spent inresearching the opportunity and meeting with representatives of the programs, staff time will beused to set up the file transfer that will initiate the deduction. This is a one-time cost that will recuranytime FCERA changes disbursement banks. We anticipate 20 hours of staff time.

Additionally, staff time will be used to create an initial and annual mailing data base of all retiredmembers that will be used to notify members of these opportunities. Each program has its own“opt in” parameters, specifically at retirement and then at least annually thereafter. The cost of themailers will be borne by the vendors. We anticipate 2 hours per year of staff time.

FCERA will incur staff time to forward and approve a “change file” (list of deduction adds anddeletes) to the disbursement bank each month. A separate file will be created for each vendor. Thisis the same process used to modify the insurance deduction for members that participate in theCounty’s health insurance program. In addition, FCERA staff will copy and forward the deductionlisting to each vendor. We anticipate 1 hour per month of staff time.

FCERA may incur a cost to set up a deduction and issue and mail a check to each vendor. There willbe no additional charges with the current bank, but future contracts may result in additional costs.The bank will charge less than $2 per check issued each month to the three vendors, or less than $6.

FCERA will incur staff time to include the optional benefit information in the retirement packet andmention it during counseling sessions, retirement workshops, and retirement seminars and updatelinks on FCERA’s website as needed. We anticipate this will have minimal impact on the process, lessthan 5 minutes per member. Benefit information will be provided by the vendors to be included withthe retirement packet.

Participant NotificationEach vendor will provide information to the participants via a separate mailing. We believe thatthere will be some confusion specifically due to the vision and dental care offered by both SACRS andREFCO. It will be incumbent upon the participants to carefully read the promotional information andspeak with representatives of the programs to determine which program is best suited to theirsituation. FCERA staff will not discuss the program, premiums, or offerings with members.

FCERA will obtain written confidentiality agreements from each vendor. FCERA will then provide afull listing of benefit recipients to each vendor or their third party print/mail firm so that informationcan be provided to all FCERA benefit recipients notifying them of the benefit. FCERA will have theopportunity to review and approve the announcements prior to distribution.

VSP and PGA will provide information packets to FCERA for inclusion in retirement packages. FCERAand vendors will work with Northern Trust, FCERA’s disbursement bank, to set up and test thededuction files that will update participant payment records for the premium. Deductions cannot bestarted without the written authorization of the participant. Thus, FCERA staff will need to confirmall adds to the deduction files. Similar procedures will be established with CIGNA Healthcare once itis ready to implement.

FiscalandFinancialIm pactsUsing an average hourly salary and benefits amount for full-time staff of $40.42 (FY2015-16 salaryrequest $2,522,120) divided by budgeted full time hours (30 positions times 2,080 hours), weanticipate the per vendor initial set up to cost $808 ($2,424 one-time costs for three vendors) andon-going costs of $566 per year plus $24 per year ($1,770 per year for the three vendors) to issue thechecks.

Statutes do allow the collection of a reasonable charge for deductions to participants’ retirementbenefits. FCERA has not imposed such a fee in the past and does not recommend doing so at thispoint.

If the insured dies other than by accident within the first

two years, the beneficiary will receive a limited graded

W HER EA S , Government Code section 31452.5 authorizes the Retirement Board to give

effect to a revocable written authorization signed by a retired member or beneficiary of a

retired member entitled to a retirement allowance or benefit to deduct a specified amount

from the retirement allowance or benefit payable for a variety of allowed purposes; and

W HER EA S , Government Code section 31452.5(a)(3) authorizes a deduction for

premiums for a vision care program or dental plan, approved by the board, for the benefit of

the retired member or his or her dependents; and

W HER EA S , the State Association of California Retirement Systems (SACRS), of which the

Fresno County Employees’ Retirement Association (FCERA) is a member, has contracted with

Vision Service Plan (VSP) to offer a vision care plan to retired members and their beneficiaries;

W HER EA S , the State Association of California Retirement Systems (SACRS) has

contracted with CIGNA Healthcare (CIGNA) to offer a dental plan to retired members and their

members and their beneficiaries of the Fresno County Employees’ Retirement Association;

Diabetes is a national epidemic. One in 13 of al! Americans have diabetes, one in four have diabetes and

don't know it yet, and one in five are on their way to getting it.

V5P's Diabetic Eyecare P{us Program provides coverage of eyecare services specifically for ►nembers

with diabetic eye disease. The program expanded coverage to include services that target glaucoma and

age-related macular degeneration (AMD) --chronic eye conditions members with diabetes are at elevated

risk of developing. Members with diabetes can also receive preventive retinal screenings if they show no

signs of diabetic eye disease.

Eligible members can receive both routine and .fallow-up medical eyecare from their VSP Preferred

Provider—the provider who already knows their eyes best.
