1. Welcome to the Gallup Student Poll. 2. Gallup will measure the hope, engagement, and wellbeing of fifth through twelfth grade students via a Web based survey administered in America’s schools. 3. Your individual responses will be completely confidential. Under no circumstances will your responses be revealed to anyone. 4. Please do not talk to anyone during the survey. (provide students with as much privacy for the student during the poll as possible. Students should remain quiet during the survey and should avoid talking about the items with other students who have not yet taken the survey.) 5. If you have a question, please raise your hand and I will help you. 6. If you do not understand a question, answer it to the best of your ability. (you may define a word for the student, but you should not attempt to interpret what the item means for the student. If a student is unable to read the items, you may read the item to the student) 7. If you would like to discuss this survey or how you feel about some of the items in the survey, please talk to a parent, teacher, or school guidance counselor. 8. Thank you for participating.
