These tables demonstrate the increased activity since the start of the champion project. 2014 includes only 6 months of activity. 2013 activity included a couple of large events outside supermarkets in RBKC with Diabetes UK which increased numbers considerably. The total number of adults within Hammersmith and Fulham, Kensington and Chelsea and Westminster contacted by champions between July 2011 and November 2014 has been 24,881.The total number of champion events

within Hammersmith and Fulham, Kensington and Chelsea and Westminster between July 2011 and November 2014 has been 752.

Larger events such as health fairs and community meeting are the most frequent engagement events and unsurprisingly have reached the largest number of people. “Other conversation” which includes one to one and smaller groups of people have engaged 10% of people.

This Map illustrates the Index of Multiple Deprivation (IMD), 2010 at small area level for Hammersmith & Fulham, Royal Borough of Kensington & Chelsea and Westminster. A high number darker orange indicates a high level of deprivation. Diabetes champion activity from 2011 to November 2014 within the Triborough area has been overlaid on the map. It shows that champions are doing events predominantly in areas of deprivation, where diabetes prevalence is higher.

Diabetes mentorsThe total number of mentees who have met with a mentor within Hammersmith & Fulham, Kensington and Chelsea and Westminster between April 2012 and September 2014 is 346.

Fourteen mentors have worked with these mentees, speaking a range of languages including Arabic, Somali, Sylheti, Cantonese, Portuguese and French.

Three accredited mentor training courses have been run with a total of 35 mentors successfully completing the course. On average about 50% of those attending have gone on to work as mentors. They work as volunteers or within local community organisations such as Rain Trust, French African Welfare Association and Midaye in K&C and the Abbey Centre and Chinese Healthy Living Centre in Westminster.

The project started in Hammersmith and Fulham and we currently have mentors working in six GP Practices in Hammersmith and Fulham. Since 2013 we have had mentors in the Barlby Practice in Kensington and Chelsea and in Westminster a mentor works in the Victoria Medical Centre. For a year a Chinese mentor worked at the Soho Centre for Health and the Chinese National Healthy Living Centre before retiring.

In K&C and Westminster recently qualified mentors will soon be working in newly engaged GP Practices in Notting Hill and Harrow Road.

Mentors have also started working with the community diabetes teams in K&C and Westminster.

We are currently undertaking a service evaluation to describe a range of outcomes and mentee characteristics in more detail which should be completed in early 2015.
