SECTION 1: OVERVIEW AND BACKGROUND

The Northwest PA Healthcare industry partnership serves 8 counties in NW Pennsylvaniaincluding Erie, Crawford, Warren, Venango, Clarion, Forest, Mercer and Lawrence. Thesecounties encompass two workforce investment areas (WIAs): Northwest PA WIA and WestCentral WIA.

This Request for Proposal provides interested consultants and training providers withinformation to enable them to submit proposals for developing a process/program designed toteach the Industry Partnership members and their employees Chronic Care Management.

The Healthcare Industry Partnership members would like to host a consortium trainingprogram that will provide information on coaching, specifically as a leadership tool. Thisshould include the process for effective coaching, building trust with employees, developing anenvironment for open communication and accountability, reinforcing positive behaviors, andhelping leaders recognize opportunities for coaching. The partnership is looking for a trainingthat will support leadership at all levels. The partnership employers encompass the continuumof care and the training should be available for all health care occupations. 

SECTION 2: REQUIREMENTS

Prerequisites for Trainer/Consultant 

Throughout this Request for Proposals, ‘trainer’ means educator, trainer, consultant,consultant(s), consulting firm, and/or consulting team comprised of representatives of multiplefirms.

Trainer must be well versed in the analysis, research, development and implementation ofleadership training programs. Trainer must have past experience in providing leadershiprelated training that includes coaching. Trainer must include prior successful relatedexperience in each of the elements that are part of the purpose and scope of work for thistraining program.

The selected trainer will be required to assume responsibility for all coordination and

implementation of the training program, including the site/location, materials,scheduling and delivery. The Industry Partnership will market the program to members andthe IP’s facilitator will coordinate reservations. Further, the trainer will direct contract with theRegional Center for Workforce Excellence (RCWE) for the total cost of the training delivery,which will be paid upon completion of the program.

The Industry Partnership is interested in offering this training to member employees. Thisgroup is also very interested in moving towards self sufficiency and would also like the option

for a Train the Trainer Workshop. Interested vendors should also include the cost andproposed delivery model for a Train the Trainer Workshop should one be available.

This can be included in the traditional training proposal or as a separate proposal.

SECTION 3: CONTRACT INFORMATION

GENERAL INFORMATION:

Proposers may be in the private, for-profit sector or not-for-profit entities with significantcapacity, demonstrable experience and outcomes to develop and deliver the specified training.

INCURRING COSTS

The NWPA HIP and its Fiscal Agent are not responsible for any pre-proposal costs incurredprior to the selection of service providers.

DISCLOSURE OF PROPOSAL CONTENTS

Proposals will be held in confidence and will not be revealed or discussed with competitors. Allmaterials submitted with the Proposal and the Proposal itself become the property of theNWPA HIP and will not be returned. The NWPA HIP reserves the right to use any or all ideaspresented in any proposal. Selection or rejection of the proposal does not affect this right.

PERIOD OF PERFORMANCE:

The contracts will begin on or after January 1, 2011 and be in effect no longer than June 30,2011. However, based on performance and availability of funding, contracts may be eligible foroptional yearly extensions for a second and third year.

REPORTING AND PROGRAM OVERSIGHT:

Successful contractors will be responsible for monthly data and documentation that tracksutilization and customer satisfaction among participating HIP members.  The documentation ofefforts and outcomes is critically important to the NWPA HIP and must be submitted monthly toStrategy Solutions, Inc., the facilitator of the NWPA HIP , on the required dates. If the trainingonly occurs one time the same data must be documented and submitted upon completion ofthe training program.

The training provider will be responsible for submitting all receipts related to the notedreimbursable expenses that are outlined in the final contract.

Program and performance oversight and monitoring will be the responsibility of the NWPA HIP.

PROPOSAL VERIFICATION: 

Proposals will be reviewed to verify that they are complete and consistent with the guidelinesset forth in this RFP. No incomplete, faxed, e-mailed or late proposals will be accepted.

SECTION 4: PROPOSAL SUBMITTAL

Each proposal must include the following using the order below:

1. Proposal Cover Sheet

2. Program Component Narrative: Please describe the following:

a. Provide a summary overview of the contents of your proposed approach

b. Describe in detail the course format and outline - your technical plan foraccomplishing the scope of work, including the minimum and maximumnumber of participants and the costs associated with each. Outline the ideallength of the course and format, in order to meet the overall objectives

c. Prior experience in chronic care management training or with new practicemodels or competency based training

d. Bios of key personnel to be involved with the training

e. Methods of program evaluation that will be implemented.

The Proposal Submission must meet the following specifications:

a) Formatted using a 1” margin and no smaller than a 12-pt. font.

b) Include a table of contents.

c) No more than ten (10) typed single-sided, single spaced pages.

d) Pages should be numbered and include a header/footer identifying the proposer’s nameand program component(s).

e) One original hard copy and one electronic copy (flash drive or CD) must be submitted

by mail or delivered to appropriate address below.

f) The hard copy should not be placed in a binder, stapled or bound in any way. Pleaseuse paper or binder clips only.

g) All proposal applications must be received by December 31, 2010. Late proposals will

not be accepted.

Sample course outlines, materials, or evaluation tools may be submitted as a separateappendix and will not be counted towards the 10 pages. The cost and course information for atrain the trainer model can be included along with the basic training proposal or may be

submitted as a separate proposal. Separate proposals should follow the same specifiedguidelines.
