Worldwide enrichment of healthcare

professionals in 2014

In the competitive CME arena, providers must rise to the challenge of offering the most updated scientific content in the most engaging formats. EXCEMED continuously looks inward and outward for ways to optimize its relationships with healthcare professionals. EXCEMED took bold steps forward in its 2014 digital portfolio, developing three new websites in chronic disease management for general practitioners, internists and specialists working in thyroid dysfunction, hypertension and diabetes. These dynamic websites - which boast a growing array of slide presentations, scientific highlights from events, expert perspectives, video lectures and courses - are being used by healthcare practitioners around the globe. Several online courses were launched by EXCEMED in novel areas of multiple sclerosis and in metastatic colorectal cancer. Online case studies were developed in cardiometabolic medicine to guide professionals in monitoring blood pressure and in managing patients with heart failure.

Exciting online surprises await our followers in 2015 – developments which will fine-tune the ease with which healthcare professionals partake in our CME offerings. Live event highlightOver 260 neurologists gathered to discuss role of genetics and environment in MSEXCEMED drew leading international experts in multiple sclerosis to explore the roles of genetics and environment in MS pathology at the conference, Multiple sclerosis: Improving patient outcomes through scientific and clinical advances. The diagnosis and treatment of MS have dramatically improved over recent decades due to scientific advancements, yet MS persists as one of the most disabling neurological disorders, having a significant impact on young adults – the age group most affected by the disease. This live event has evolved into a key date for neurologists from all over the world who specialize in MS.

Over 260 neurologists involved in the management of MS participated

in the 2014 CME-accredited annual Conference in MS. Organized

by EXCEMED, the conference took place on 9-10 May, in Dubai, UAE.

Presentations, workshops and roundtable sessions focused on important aspects of MS relating to epidemiology, genetics and patient-centered clinical management:• Latest advances in MS pathogenesis• Genetic susceptibility and environmental factors

Multiple sclerosis therapy: mechanisms of actionProf David Bates, University of Newcastle upon Tyne, UK

Therapeutic options for MS have seen tremendous expansion in recent years, offering physicians and patients an array of options and the challenging responsibility of choosing the optimal therapy for each patient. Extensive clinical evaluation of patient, clinical phenotype and disease progression are fundamental for selecting the best therapeutic option. This online course offers the chance to fully understand the modes of action of current MS drugs and emerging DMTs. Accredited by the European Accreditation Council for Continuing Medical Education (EACCME) for 1 European CME credit (ECMEC).

Inflammation, synaptic transmission and neurodegeneration in multiple sclerosisProf Diego Centonze, University of Tor Vergata, Rome, Italy

Although MS has been regarded as a disorder of CnS white-matter, early alterations of the neuronal compartment are partially independent of demyelination. Soluble inflammatory cytokines and glutamate have been proposed as major determinants of neurodegeneration. The relationship between these two major determinants has been largely elusive. Data suggest that CnS-confined inflammation in MS is associated with the release of soluble molecules which can alter excitatory synaptic transmission and stimulate secondary neurodegenerative gray matter pathology. This online course explores all these novel aspects of MS. Accredited by the European Accreditation Council for Continuing Medical Education (EACCME) for 1 European CME credit (ECMEC).

Live event highlightItalian expert honored for excellence in medical oncology education One of Europe’s leading medical oncologists was honored by EXCEMED for exemplifying educational excellence in his field. Doctor Fortunato Ciardiello, Professor of Medical Oncology at the Seconda Universita degli Studi di Napoli, Naples, Italy, received the 2013 SSIF Award for medical education in oncology during EXCEMED’s satellite symposium at the European Society for Medical Oncology (ESMO) 16th World Congress on Gastrointestinal Oncology. “Teaching is a very relevant part of my activity – for me, this award recognizes my scientific background but also my teaching,” says Prof. Ciardiello, who leads the Division of Medical

Oncology and the Laboratory of Experimental Therapeutics at his university. Prof. Ciardiello says three essential elements of medical oncology education are to know your audience and their needs, ensure top-level experts present updated material in an accessible way to bring essential messages to the forefront, and offer a consistent programme where one topic builds on the other to form a learning pathway. Employing suggestions from young oncologists when crafting educational programmes could help fill knowledge gaps in medical oncology, Prof. Ciardiello explains. “Our mission in the profession of medical oncology is to become more homogenous,” he says. “There are countries where specialties are more organized. It is important that we bring this standard up to a European level.” The award, which showcases the value of medical education in our society, goes to an outstanding professional from a different medical field each time it is delivered.

EXCEMED Educational fields 2014

Many treatments options, and different timings and sequences of treatment, are used in the management of synchronous liver metastases in CRC. There is a need to produce guidelines to assist oncologists to manage this disease optimally. Discussions on the optimum treatment of these metastases were a major focus of the EXCEMED consensus meeting. Following the success of the Consensus conference on “Criteria for resectability of liver metastasis in colorectal cancer” held in 2011 in Rome, this second Consensus conference took place in Florence, Italy on October 24-25, 2014. The 2011 event yielded a paper published in The Oncologist (September 2012, shown at right) and the 2014 edition led to a paper published in Cancer Treatment Reviews (June 2015) entitled «Managing synchronous liver metastases from colorectal cancer: A multidisciplinary international consensus.»

Live event highlightResearchers edge closer to European approach for managing PKU Researchers took another step toward standardised European guidelines for managing the metabolic genetic disorder phenylketonuria (PKU) at a conference dedicated to the illness. EXCEMED convened dietitians, paediatricians and other healthcare professionals for the 2014 Annual Multidisciplinary European Phenylketonuria Symposium – PKU: A lifetime of challenges in Zagreb, Croatia from 6 to 7 June 2014. “We are very close to making European recommendations for the management of PKU. This was a big feature at the meeting – all the guidelines were presented, some of them based on evidence, some on opinion,” says Prof Anita MacDonald, Consultant Dietitian at Birmingham Children’s Hospital, Birmingham, UK and co-scientific

coordinator of the annual conference. In the past, much PKU research was uncontrolled or not randomized and it occurred non-collaboratively in small centers, explains Prof MacDonald. “We are left with dilemmas such as people within the same country dealing with PKU differently,” she says. PKU is a rare disorder affecting one in ten thousand people; lack of funding and a history of limited, inconclusive research have meant slow advancements in effective management techniques. “For the past 60 years, diet has been the mainstay of treatment for PKU. Although successful, it is challenging, so we are desperate to find effective and cost effective alternative treatments for our families,” says Prof MacDonald. The meeting evaluated management of PKU, interpreted its pathophysiological insights, and compared ways to counteract neurocognitive repercussions for patients. Approaches for managing maternal, transition age and adulthood PKU were also considered.

Live event highlightContinuing Medical Education tour helped enhance diabetes practice among health professionals in Sub-Saharan Africa EXCEMED launched its second CME Tour of Africa in 2014 to enhance local awareness and capability in diabetes and chronic disease management – conditions which are a growing problem in African alongside changes in diet and lifestyle. “Africa, like many areas of the world, is about to experience a tidal wave in the prevalence of diabetes, to be followed by a wave of cardiovascular-related complications. This will be an enormous burden on the healthcare systems of these countries,” says Dr. Jonathan Levy, medical lead for the CME Tour and researcher at the Oxford Centre of Diabetes, Churchill Hospital, Oxford, UK. The CME Tour engaged over 1,000 general practitioners, medical students and pharmacists in Angola, Ghana, Kenya, Mozambique, Namibia, Tanzania and Uganda. The workshops, in collaboration with African universities, showcased evidence-based data and demonstrated diabetes management approaches.

Cardiovascular risk factors – transforming knowledge into practice

Over 130 healthcare professionals who manage hypertension in China, India, the Philippines, Thailand and Vietnam attended this symposium. Presentations focused on managing CV risk factors to prevent the fatal end of the cardiovascular continuum. Activities were designed to ensure that scientific knowledge could best transform into good clinical practice. This is a key aim of every EXCEMED CME activity.

CVD remains challengingCVD remains the most common cause of mortality and morbidity across Asia-Pacific, and indeed globally, despite increased knowledge and ongoing research. Treatment of major CVD, including ACS or stroke, presents costly challenges for healthcare systems, without considering the most effective approach for CVD: i.e. prevention. Essential hypertension remains a problem that is often complicated by metabolic disease. Clinicians should regularly review care for every hypertensive patient to ensure they receive appropriate pharmacological treatment. Regimens should follow current evidence-based guidelines, focusing on normalisation of BP values.

Live event highlightExperts gathered to demystify human implantation in fertility treatments Human embryo implantation has been called the “black box” of assisted reproductive technology (ART) because, until recently, there has been less emphasis on its biological underpinnings. EXCEMED’s annual conference in reproductive medicine - Implantation: Opening the black box - convened top experts in reproductive medicine to explore approaches and technologies useful for successful human implantation. “This event was truly interesting because it addressed a major hurdle that stands between us and success for our fertility patients – that is, addressing human implantation,” says Dr. Nicholas Macklon of the Department of Obstetrics and Gynecology, University of Southampton, Southampton, UK and co-scientific organiser of the conference. “Up until now we’ve been successful in creating embryos through IVF techniques, but our challenge is getting these embryos to implant,” explains Dr. Macklon.

“Implantation of the embryo into the endometrium has been a kind of mystery in the past,” says Dr. Peter Zsolt Nagy of Reproductive Biology Associates, Atlanta, Georgia, USA, Chair of ALPHA, Scientists in Reproductive Medicine and a co-scientific organiser of the conference. “We were happy when it worked and the patient became pregnant, but when it didn’t work, many times we were unable to identify what the reasons were,” he says. “At this meeting we had the most outstanding researchers and scientists giving their views and presentations of their research to explain this black box which is not really a black box anymore,” says Dr. Nagy. The CME-accredited conference targeted clinicians and biologists working in ART who wished to update their knowledge of advanced techniques and innovations. Attendees learned biological aspects of human embryo implantation, enhanced their understanding of advanced techniques, and applied evidence-based recommendations to achieve the best outcomes from ART.

EXCEMED assesses the learning effectiveness of its programmes through a series of surveys at each live event as part of its commitment to ensuring an optimal learning experience for healthcare providers.

Learning effectiveness in 2014

Participants are asked to complete questionnaires related to the learning objectives of the course before, during and after a live event. Comparing the number of correctly answered questions at each stage allows EXCEMED to track the evolution of healthcare providers’ knowledge, competence and change in practice.
