What is Hospice at Home
• A team of nurses who provide specialised care for those who are at the end of life (18yrs old and upwards)
• Enable real choice for patients and families• Work in partnership with GP’s/Community
Nurses/charities• Service that is available 24/7 - 365 days year
Who is in the team
• Range of staff, trained and untrained• At any one time there is a trained and untrained
person on duty to provide care• Alongside them is a co ordinator who works to
carry out assessments and manage the day to day work – this person enables a responsive service
Who do they care for
Provide support to those people who are:-•actively dying•being discharged from hospital/hospice for end of life care•in crisis where a care package is being adjusted or set up•in care/disability homes
We prevent hospital admissions
What do they do
Work in partnership with community team•provide personal care•provide emotional support•administer medication•support a family when patient has just died•sit with patients at night

•Total referrals 359
•121 (34%) patients from Fareham and Gosport

a week, timing visits with such care
managing his situation wonderfully
well to keep him peaceful”.
On the day mum died, the team were
with me within minutes and they told
me not to panic and helped stay calm.
They were amazing!
The nursing care was excellent but most reassuring and helpful was having mobile number to call at
night. Response to this was excellent.
With your help you enabled the four of us to take care of dad at home which was what he really
wanted.

from E8. I was starting to feel that we would miss
our window of opportunity with this lady. If it wasn’t
for your team we wouldn’t have managed it so

I am very impressed with the Hospice at Home Team. There has been a rapid and reliable response with good communication which has
provided invaluable support to the patient, carers and families. I am certain that we have avoided unnecessary hospital admissions by
having such a wonderful resource.
What do we need ….
Funding:-
Ultimately ………•we enable choice for people at end of life •save the NHS money by:-
– supporting care until care packages are in place– preventing unwanted hospital admissions– delivering care overnight
