This newsletter shall contain flashes of HSE achievements, award winners, messages and any other related issues. The staff and workers are encouraged to send in their contribution regularly so that we can publish selected items in the quarterly issues. The newsletter shall also contain pictorial presentation, client comments, audit results, etc. The main objective of this newsletter is to serve as a communication catalyst in our improvement plan for HSE efforts. We have faced them all – vanquishing all doubts, carrying out in every project andpresenting with an even stronger front, enabling us to meet the challenging needs of time. Our continuing willingness to learn is our strength.

Be Safe Always!

As part of HSE initiative for competency based training, the ‘3-tiers’ training and incentive programmewas introduced in May 2013. DM Supervisors are mandated to complete the training programme & appraisal which consists of Level 1 – Core Competency, Level 2 – Risk Management & HSE Workshop, Level 3 – Performance Appraisal for Zero Harm. Winners are categorized into Good, Excellent and Safety Champion. Awards were given to Sarowar, Ahmaddol, Nandha, Lewis, Jaya and Ramli during the Mass Tool Box Meeting on 24th January 2014.

HOORAY! – Kudos to all Dyna-Mac employees and contractors for the successful safe load-out of QUAD 204 TURRET Project. The team efforts paid off with MORE THAN 3 MILLION MAN HOURS WITHOUT LOST TIME INJURY.

The project team effectively complied with legal and management system requirements to ensure that no one is hurt. The HSE programmes and activities implemented in the project includes: HSE audits, daily inspections, supervisors’ reviews, management walkabout, training and ‘observation & interventions’. To continually increase morale, incentive awards were given to those deserving site personnel.

Featured PersonalityAppreciation of Workers By Matin

Do it right the first time! This is the goal of the two riggers / signalmen, Natarajan and Kamal, to ensure nobody will get hurt and the job will be carried out seamlessly. Natarajan and Kamal are known by their colleagues as easy to work with, and always give points in terms of safety. As riggers / signalmen, they maintain a good working environment by keeping everybody aware of the lifting activities and to ensure that every lifting procedures are followed. And true enough, they were recognized for their exceptional demonstration of safety on the first safety management walk through conducted by Shell, SBM and Dyna-Mac.

SBM has given Natarajan and Kamal a token of appreciation and encouraged them to maintain the practice and share their skills with all their colleagues.

They are a role model for other personnel and an example of how safe you may become if you follow the safe work procedures and with a good attitude, we can all achieve zero accident, zero incident!

Moving forward with the different practices that were derived or requested by the client’s project HSE team, together with our very own DM HSE department specific to the project, there is a need to standardize the practices across the board. This helps to ensure a smooth transition between the current to the next project and benefiting all parties of the Health, Safety & Environmental aspects to ultimately achieve the HSE goal. In line with SS 506 Part 1:2009; “For the management of change, an organization shall identify the OHS hazards and risks associated with changes in the organization, the OHS management system, or its activities, prior to the introduction of such changes.”A Management of Change (MOC) register has been developed to monitor and track key changes throughout the project, highlighting the control measures adopted and action status, which will be shared between the different project teams and a tool to share during lessons learnt at the end of the respective projects.

The year 2013 was a good year for Dyna-Mac with the successful delivery of all projects. All the safety milestones achieved were celebrated with awards given to “Best Safe Worker”, “Best Workgroup”, “Best Safe Supervisor”, “Best Zone Leader”, “Best Safety Personnel”. QA/QC awards were also presented to dedicated employees in recognition of their efforts.An array of celebration can be observed in the pictures as a testimony of accomplishment and success towards safety. The support given by the management in each project is monumental to motivate the workforce. Mark it, success never stops here, all milestones set a guide to envision path through accolades.

Improvement Plans:By Lynn

HSE Statistics 2013 reveal that Accident Frequency Rate (AFR) is 0.27 – best achieved AFR since 2008. The achievement is only possible through the dedication of our workforce to ensure a safe working environment. In addition due to the lost time incurred in the 1st

quarter of 2013, a HSE improvement plan has been initiated. The plan consists of the following elements. • Heightened WSH Committee Meeting Bi-monthly meetings were initiated in the first half of 2013 in an attempt to address key factors on control of work issues. Upon improvements made, the meetings has been re-scheduled to monthly with key management and worker representatives including sub-contractors and clients.• Risk Assessment CommunicationRisk Assessment to be communicated daily by the immediate supervisor/ HOD on the specific task during Toolbox meeting.Risk Assessment communications were evaluated byDM and Client HSE personnel to check on its effectiveness. Refresher training shall be provided

for supervisors to enhance their communication skills.• Core Competency TrainingAll New DM & Contractor HOD & Supervisors shall attend core competency trainings as a standard.• HSE WorkshopAll New DM & Contractor HOD & Supervisors shall attend the HSE Workshop, conducted by SBM & DM HSE Team• HSE Checklist by Line SupervisorLine Supervisors shall conduct daily HSE inspections using specific checklists on worksite conditions, position for tasks and similar.The checklists are reviewed by the individual trade HOD and trends were reported accordingly.• HOC CardEnhanced use of HOC cards to inculcate and exercise intervention; Stop Walking Past Violations.HOC Card Trends are captured in the weekly HSE report for projects.• Critical Equipment ScheduleInspection Schedule for Critical Equipment has been formulated comprising those equipment required Mandatory inspection and In-house Inspection by competent personnel.• Alternate day (1-3-5) HSE ReviewsAppointed supervisory staff shall attend to address on daily HSE findings with a stipulated date (corrective and preventive actions)Daily HSE inspections are sent towards top management & department heads.

Vanan : Challenges faced/Lessons learnt in Quad 204?Peter : Very high and stringent requirements, amidst the level of experience due to Quad being our very first Turret Project for Quad 204. There were many regulations pertaining to European directives where we have to perform or meet not only local standards but also internationally as well.From this experience, lessons that were learnt, should be taken into account of the initial set up of the project team. Pre-refit meetings before strike steel is very critical to fully understand the requirements through adequate gap analyses, to better highlight our concerns and mitigate or negate any potential impacts that may affect the project technically as well as commercially.

Continual Improvement:Keeping the system updatedBy Peter Sujin

Revised HOC Card:Hazard observation and communication (HOC) card is an important tool in the identification of unsafe acts and conditions and to take proactive measures for up keeping the safety and wellness of the work force. To reinforce the system in place and to improve further, DM has revised the HOC card with some enhancement.

Salient features of the Revised HOC Card:• Description is written in Multi-language.• Descriptions are enlightened with detailed classifications.• Follow up verification traces out the root cause of the issue and as such effective remedial actions can be imposed to prevent similar recurrences.• It can be used as an educational tool especially for a newbie and can be influential in streamlining different disciplines and respective contractors who have varying safety standards and backgrounds.

Way forward, DM should keep on encouraging and educating workers and supervisors to be more proactive and instill safety ownerships among them in order to further promote safety culture in our workplace. In that, the frequent use of HOC cards is strongly encouraged.

the 3rd of December 2013 in our premises. DM

has achieved a commendable exercise and once

again our ERT personnel have proven their

competency and level of professionalism in

handling emergency situations.

WSH UpdatesWSH (Work at Heights) Regulations 2013(Excerpt from Workplace Safety and Health Council)

The Workplace Safety and Health (Work at Heights) Regulations 2013 (WAH Regulations) have been gazetted and published on 15 April 2013. It will come into operation on 1 May 2013.About the new RegulationsThe key provisions in the new Regulations are:a. Fall Prevention Plan (FPP) – A specific provision mandates the establishment and implementation of a FPP in accordance with any Approved Code of Practice issued by the WSH Council. This provision will only be effective on 1 May 2014 to allow industry time to build up their capability;b. Permit-to-work System (PTW) – A specific part pertaining to the control of work at height where a person could fall from a height of more than 3 meters. This will also be effective on 1 May 2014 to allow industry time to have people trained and qualified to implement the PTW system;c. Training of workers, supervisors and other WAH personnel – Training requirements for work at height personnel are emphasized to ensure effective conduct and oversight of work performed at heights;d. Work on Roof and Fragile surface – Specific provisions on safe working on roof as well as working near fragile surface are included to require persons working there to be more vigilant; ande. Industrial Rope Access System – Due to its high-risk nature, a specific part has been included to address this area of concern.The scope of coverage of the proposed Regulations will be in two phases, as follow:1 May 2013• Comes into force and applicable only to factories.1 May 2014• Coverage will be expanded to all workplaces• Establishment and implementation of a FPP for workplaces specified in the Schedule• Implementation of a PTW system for work at height where a person could fall from a height of more than 3 meters.

MOM Alert!Workplace Safety and Health (WSH) has announcedan amendment on the existing i-report for LTI whichstates that all accident case regardless whetherthese were consecutive days, employers have toreport all accidents which render their employeesunfit for work for more than three days, after thechanges to the WSH incident reportingrequirements effective from 6 January 2014. Theseaccidents must be reported to the MOM within 10calendar days from the fourth day of medical leave.
