MR. MUSLEH’s Learning from Incidents

Welcome to issue No. 15 of Mr. Musleh’s News, the communication which provides a ‘one stop shop’ for you to receive learning's from incidents in PDO and the oil and gas industry.

This month Mr. Musleh will summarise the learn-ing's from incidents of the lost time injuries (LTIs) and High Potential (HiPos) that have occurred dur-ing quarter four, 2017.

Fire in the tank

While welding inside a brine tank, a small amount of circulation water with oil residue entered the tank. When the spark ignited the fluid the welder immediately evacuated and alerted the crew members.

Sitting floorman

During laying down Drill collar, the Floor man unlatched the elevator, causing it to swing towards the draw works side knocking him off balance onto the rig floor landing in a sitting position.

JAWS

Whilst breaking out using the Manual tong, the long jaw hinge pin and jaw parted. land-ing approximately 1 m away from the Driller’s console.

Nodded off

A driver was on a flatbed trailer when he lost consciousness falling to the ground frac-turing his left forearm.

Wrong platform

While using a ladder backwards instead of a work platform the welder has fallen.

Cloud goes bang

Ignition occurred through an electrostatic discharge the resulting flash fire injured a nearby worker.

Wall collapses

A Civil crew engaged in digging the trench deeper when a part of the trench wall col-lapsed and fell on the ground worker.

Mind your Step

While the scaffolder was guiding the scaf-fold he placed his right foot on a step lad-der. The ladder slipped and he lost his bal-ance and fell fracturing his upper left arm.

Cave in

During welding of a 6’’ pipe line a welder suffered fracture to his pelvis. Working inside a trench when the soil stored on the side collapsed on to the pipe crushing him against the side wall.

Improper Lifting

While connecting a web sling to an excava-tor bucket, the helper’s finger was trapped at bucket pivot pin fracturing his little fin-ger. View

Shortcut goes wrong

A Field Operator stepped on the edge of the new skid foundation losing his balance and twisted his left leg.

Three rig floor crew members were about to latch hydraulic casing power tong to the casing when one trapped his hand between the tong gate handle and casing joint. View

Hammer splinters

Whilst one of the crew members was ham-mering a spanner to remove bolts, a small splinter from the hammer cut through his cotton glove and hit his right index finger.

Night driving rollover

While driving back on a right of way at night the driver hit a sand pile then rolled over. The driver managed to come out without any injury and then called the rig through his mobile.

Tanker lost control of his vehicle with a heavy load tipping over onto the right shoulder down a gradient. View

Shocker

During a Rig move while a crane was ma-noeuvring an electrical arc between the centre line of the OHPL’s and the sheave pin on the crane boom.

During a recent Rig Move, several kicker board lines were removed to allow a clear path for the rig mast without shutting off the power to the lines.
