Hasson, Margaret Barry, Allison Green and their staff cared for children from Frederick County. They provided the children with screenings, radiographs, prophylaxis, topical flouride and sealants. Thanks to DeDe Pew, community liaison for Frederick County Schools, for arranging the transportation for the kids. Unfortunately, due to the ice storm, the 35 children who were scheduled to attend from Carroll County were unable to. However, Dr. Hasson and his team will treat these children as promised, as well as provide follow-up care for all of the children throughout the year, pro bono.

In another part of Frederick County, the Dental Office of Drs. Levy, Somerville, Patel and Bhushan were also pleased to take part in their 10th annual GKAS Day. All of the children received free exams, x-rays, cleanings, flouride, oral hygiene instructions and goodie bags filled with both oral hygiene supplies and prizes. This time of year it is especially hard to devote time to exercise. It’s dark in the morning and cold, often wet (snowy!), and the evening is also dark, and has the usual social commitments to compete with exercise schedules. But it is still important to get out there!

Paul Williams, PhD, Life Sciences Division, Lawrence Berkeley National Laboratory found in the June 2013 Med & Sci in Sports & Exerc that moderate walking and vigorous (running) exercise were both significantly associated with lower cataract risk and their effects similar. “Cataract risk appears to decrease linearly with increasing exercise energy expenditure...” This study was done over 6.2 years with 32,610 runners and 14,917 walkers.

So even our eyes don’t deteriorate when we exercise.Endorphins are released to make you feel good; you’ve established a sense of

accomplishment the first thing in the day; you’ve made more mitochondria to produce more energy burning capability and more fat burning facility (especially if you drink that coffee first!).

$5000 Prize Offered in the MSDA C&E Foundation’s First Fundraiser

The MSDA Charitable & Educational Foundation (MSDAF) has just begun its first large scale fundraiser. As the MSDAF has been gearing up and reaching out to the community, they have already helped thousands of people by granting six Mission of Mercy projects with supplies, equipment, materials and funding, as well as awarding five dental student scholarships. To keep this charitable outreach of the Association alive and growing the MSDAF has been seeking funding from various sources, including a fundraiser whose grand prize is $5000. Two runner-up prizes valued at $500 will also be offered.

To keep the odds of winning high, only 1000 tickets will be sold. If all of these tickets are sold, the fundraiser may raise up to $50,000 for the Foundation and its outreach programs. This funding would go a very long way in supporting the MSDAF and all of its charitable programs.

I cried when I heard about Philip Seymour Hoffman. The news scared me: He got sober when he was 22 and didn’t drink or use drugs for the next 23 years. During that time, he won an Academy Award, was nominated for three more and was widely cited as the most talented actor of his generation. He also became a father to three children. Then, one day in 2012, he began popping prescription pain pills. And now he’s dead.

The root causes of addiction, like those of many multifactorial diseases, are frustratingly elusive, a nebulous mixture of genetics, exposure and environment. While addiction runs in families, there are plenty of addicts who come from families with no history of the disease. Availability plays a role, too, but having access to crack doesn’t make someone a crack addict. The science about recovery is also hazy: Alcoholics Anonymous, the most widely used form of treatment in the country, has no set structure or methodology, which makes it tough to evaluate its effectiveness. (There’s also the fact that its core principle – that members never publicly acknowledge their presence in the program – makes broad longitudinal studies difficult, to say the least.) In-patient treatment centers, like the one Hoffman checked himself into last May, have been accused of obfuscating their success rates.

If anything, the science on relapses is even more slippery. (We do know that relapse rates for drug and alcohol addiction are comparable to people’s inability to control other chronic illnesses, such as type 2 diabetes, asthma and hypertension.) The challenges are as basic as agreeing on a definition for long-term sobriety. In a graphic titled “Extended Abstinence is Predictive of Sustained Recovery,” the National Institute of Drug Abuse says, “After 5 years – if you are sober, you will probably stay that way.” I unconsciously added a “forever” to the end of that

sentence, but the study that chart is based on ran for eight years, a bar Hoffman cleared easily.

My first attempt at recovery came in 1991, when I was 19 years old. Almost exactly two years later, I decided to have a drink. Two years after that, I was addicted to heroin. There’s a lot we don’t know about alcoholism and drug addiction, but one thing is clear: Regardless of how much time you have clean, relapsing is always as easy as moving your hand to your mouth.

In August 2011, after a dozen years in New York, I moved back to Boston, the city in which I was raised and attended college. I was 39 years old and married; my wife and I had a 1 1/2-year-old boy and another child on the way. I’d written three books, won some awards and was about to start teaching at MIT. If I’d been asked to provide a CliffsNotes version of my life, those are the details I would have included.

Being back in Boston was a visceral reminder that there’s an important part of my past that isn’t on the bio page of my website: From 1995 to 1997, the last time I’d lived in the area, I’d been an IV drug addict. Living here again made me acknowledge that past every day: The drive to my son’s preschool took me within blocks of the apartment that I’d lived in during those years; my route from his school to my office went past the free acupuncture clinic where I’d sought relief from withdrawal pangs. One afternoon, I looked up and realized I was in front of the emergency room I’d been taken to after overdosing on a batch of dope laced with PCP. I did a double take and looked to my wife, but, of course, that wasn’t a memory we shared. We met in 2004, when I’d been sober for more than six years.

There is a lot of science that demonstrates the positive effects being happy has on people, including better health and a longer life. According to Webster, happy can be defined as “being pleased or glad about a particular experience or event.” So, how does being happy apply to the dental practice? Is creating happiness important and can it become a system within the practice that is implemented on a daily basis? Happily, the answer is yes!

What is Happiness?As doctors, your role is not just to cure, but also to care. When people know

you care, whether they are patients or team members, they tend to be happier. Logically, making patients happy should be a deliberate part of delivering effective treatment. So, let’s consider what makes a happy patient. Being happy is much more dynamic than being satisfied. Dr. Martin Seligman, Director of the Positive Psychology Center at the University of Pennsylvania and founder of Positive Psychology, a branch of psychology which focuses on the empirical study of such things as positive emotions, strengths-based character and healthy institutions, defines authentic happiness as a positive emotion resulting from engagement and meaning. In dentistry, happy patients are quality patients, as demonstrated by their behavior. A quality patient commits to consistent, continuing care visits, accepts your treatment plan, agrees to your financial arrangements, keeps appointments, re-appoints for her next appointment, refers family and friends and values your practice’s philosophy of preventive dentistry.

Creating a Happy EnvironmentOwning and operating a small business is an exciting opportunity, professionally

and personally. The best part is being able to develop other people to their potential, to uncover and enhance their strengths and make their limitations irrelevant. As a leader of a dental practice, the dentist sets the example for conduct and behavior, including happiness expectations for employees and patients.

In other words, they cast the vision and determine the practice philosophy, which, subsequently, determines the practice systems, including both the hard-side and soft-side systems. The hard-side systems are the structure, processes, benchmarks and feedback tools. The soft-side systems focus on creating a positive environment and the expectation for happiness, which is equally, if not more, important.

For example, dental practices understand the importance of having a daily huddle, but tend to focus on what happened yesterday, what’s on the schedule today and what is coming down the road tomorrow. These are all hard-side systems. They often miss the other component, the soft-side, which is about setting the right tone for the day. At Straine, we have a huddle every morning and begin with a gratitude statement. There is science that shows gratitude exercises have the ability to change people’s happiness set-point. Based on my practice philosophy, the first thing that I want to do is plant positive seeds to sprout and bloom for the rest of the day so my team will embrace opportunities to share their happiness with others.

Creating a Happy TeamUnfortunately, having a positive work environment does not automatically result

in a happy team. A happy team feels they have control over the things that happen to them and they can solve the problems they encounter on a daily basis because there are clear policies, a proven management system and understanding roles and responsibilities. A happy team is created through encouragement, education and positive reinforcement. The most powerful types of reinforcement are attention, approval and appreciation and need to be given abundantly every hour of every day. But it must be authentic, which means the reinforcement is specific to the team member’s behavior and an important component of that person’s job responsibilities. For example, instead of being general and saying to your financial coordinator, “You’re really great at handling patients,” you would be more specific with, “I really

Surgeon General Updates Tobacco, Oral Health Linksby Craig Palmer, ADA News staff

The U.S. Surgeon General’s 50th anniversary review of tobacco science since Dr. Luther Terry’s landmark report on smoking and health updates evidence on the implications for oral health from tobacco use and the “expanding use of multiple products or the replacement of conventional combustible cigarettes with other nicotine delivery systems.”

The 998-page report significantly expands the list of illnesses “causally associated” with active smoking to include orofacial clefts in infants, the acting Surgeon General, Boris D. Lushniak, M.D., said in a preface. “The evidence is sufficient to infer a causal relationship between maternal smoking in early pregnancy and orofacial clefts,” the report said.

The Association congratulated acting Surgeon General Lushniak “for issuing a report that further examines the relationship between tobacco use and an array of health problems, including oral cancer, gum disease and dental caries.”

The dental disease chapter in the 2014 report offers these conclusions and implications based on recent research reported in the scientific literature:

• The evidence is suggestive but not sufficient to infer a causal relationship between active cigarette smoking and dental caries.

• The evidence is suggestive but not sufficient to infer a causal relationship between exposure to tobacco smoke and dental caries in children.

• In developed nations, smoking is strongly associated with sociodemographic characteristics and a wide range of health behaviors that also are strongly associated with elevated risk for caries. Given the public health importance of dental caries, further research on smoking is needed with careful attention to confounding.

• The evidence is suggestive but not sufficient to infer a causal relationship between cigarette smoking and failure of dental implants.

• The existing evidence suggests that smoking may compromise the prognosis for osseointegrated dental implants. Thus, an intervention to discontinue tobacco use should be part of the treatment plan for persons who are considering a dental implant.

The report also examines evidence on the health consequences of nicotine exposure.“The possibility of increasing chronic nicotine exposure in the population from

various nicotine-containing products for the long-term merits further research,” the report said. “Cancer, cardiovascular and neurocognitive outcomes are of concern. The evidence is already sufficient to provide appropriately cautious messages to pregnant women and women of reproductive age as well as adolescents about the use of nicotine-containing products such as smokeless tobacco and electronic cigarettes, and newer forms of nicotine-containing products, as alternatives to smoking.”

The Surgeon General’s tobacco science update concludes with “calls for dramatic action” to reduce tobacco use. “The current rate of progress in tobacco control is not fast enough,” the report concludes. “More needs to be done.”

The commissioner of the U.S. Food and Drug Administration, which regulates tobacco products under the Family Smoking Prevention and Control Act, said the FDA “will continue to propose and implement tobacco products regulations to protect

the nation’s health.” F

How Advances in Tissue Engineering and Diagnostics Affect Treatment Planning and Treatment OutcomesThis presentation will focus on how advances in diagnostics and tissue engineering impact our ability to more predictably and more safely treat our patients. With the advent of cone beam radiography, and the use of 3-D imaging software we are able to treatment plan both the simple and complex cases and provide the patient with a much better understanding of what we are able to accomplish. Many of the challenges we have faced in terms of hard and soft tissue grafting have been answered with advances in tissue engineering. We will explore the current state of the art of tissue in engineering and how these technological breakthroughs afford opportunities to treat challenging cases.

Objectives:1. Participants in this program will be able to appreciate the difference between 2-D and 3-D

Maryland Study Looks at Correlation Between Dental Insurance and Utilization

Providing people with dental insurance does not necessarily mean they will use it and seek dental care, according to a new study from the University of Maryland School of Dentistry, published in the American Journal of Public Health.

The research suggests that outreach and education are needed to ensure that people value their dental health and use their coverage to seek appropriate dental care. The researchers hope that policymakers will use the findings in designing future programs and initiatives, according to first author Dr. Richard J. Manski, Ph.D., professor and chief of dental public health at the University of Maryland School of Dentistry.

“You can’t just hand people coverage and say, ‘There, that’s better,’” says Dr. Manski. “You need to offer some inducements, some promotional campaign to change people’s attitudes and beliefs. We hope this starts the process of a new way of thinking about the problem.”

The researchers examined data from the Health and Retirement Study of 2008, looking at older Americans who had dental coverage and those who didn’t, and examining who was using dental care. They also looked at personal characteristics such as race, gender, marital status, age, health status and more.

The scientists found that providing dental coverage to uninsured older Americans who do not tend to use dental care will not necessarily mean that, once insured, those people will seek dental care.

Rather, if policymakers want people to use dental coverage and seek care, they have to go a step further than just providing insurance.

While many of the factors that keep people from seeking care—such as age and gender – can’t be changed, other factors could be influenced by outreach. These factors include knowledge, beliefs, attitudes, tastes, health status and income, according to the study.

One truism of addiction science is that long-term abuse rewires your brain and changes its chemistry, which is why triggers (or associated stimuli, in scientific parlance) are major risk factors for relapse. But these changes can be reversed over time. Walking past the apartment where my dealer used to live didn’t make me want to score; it made me feel as if I was in a phantasmagoria of two crosshatched worlds – but I was the only person who could see both realities. None of my colleagues at MIT, no one in the science writing community I lean on for professional advice and support, none of the people who’ve worked on my books has ever seen me slip into that other world.

That was a disquieting realization. I worry about the day that I stop acknowledging both of those realities. Most adults with jobs and mortgages and spouses and kids can have a glass of wine after work. For me, a glass of wine is a gateway to my past – and that past provides a pretty robust pool of evidence that there’s not much separation between my having a drink and my ending up alone in an apartment with a needle in my arm. F

Treating Periodontal Disease as Infectious Diseases: Anti-Infective Periodontal Therapy (4 CE CREDITS)

Periodontal diseases are chronic infectious diseases. About 1/2 of the population has periodontitis ranging from slowly progressing with little destruction to rapid loss of periodontal support. Traditional treatment, both surgical and non-surgical, can only predictably control periodontitis in less susceptible people. The most susceptible individuals often continue to lose support with either of these treatment modalities.

Properly applying the principals of infectious disease control as developed for other infectious diseases can stop periodontal destruction, and may reduce or arrest systemic disease effects caused by periodontitis. Periodontal pathogenic microorganisms must be identified, monitored and effectively treated before, during and just as important, after any type of active periodontal treatment to insure disease control.

This course presents effective methods for infection monitoring and treatment of pathogenic periodontal infections by dental professionals, as well as strategies to reliably control supra- and sub gingival biofilms via patient home care procedures. Participants will be able to: use methods for effective identification of high risk infections; treat these infections; train patients in effective biofilm removal, design an effective hygiene program for disinfection and select antibiotics for therapy and monitor for recurrence of infections.

Periodontology: Concepts and Review (3 CE CREDITS)

Recent discoveries have established a strong link between periodontal disease and overall health. Poor oral hygiene and associated periodontitis can lead to heart disease, diabetes, respiratory diseases and perinatal complications. Conversely, smoking, diabetes, genetic factors and pathogenic bacteria increase risk for periodontal disease. Periodontitis is a set of inflammatory diseases affecting the periodontium, which is comprised of the gingiva, alveolar bone, cementum and periodontal ligament. Periodontitis is caused by microorganisms that adhere to and grow on the tooth’s surfaces, along with an overly aggressive immune response against these microorganisms. Locally this can lead to progressive loss of the alveolar bone around the teeth, and if left untreated, can lead to the loosening and subsequent loss of teeth. Systemically, the exaggerated inflammatory response can cause pathology throughout the body. Paradoxically, periodontitis is both under diagnosed and under treated. In the early stages, periodontitis has very few symptoms; in many individuals the disease has progressed significantly before they seek treatment. Given the importance of periodontal disease in causing local as well as systemic inflammation and disease, improving the diagnosis and treatment of periodontitis is a significant opportunity to improve the patient’s overall health. This talk will cover the basics in the treatment and prevention of periodontitis.

appreciate that you resolved the treatment plan with Mrs. Smith as effectively as you did. I appreciate the way you handled her objections.”

It is critically important that the doctor is in tune with the team and is proactively encouraging and reinforcing positive behavior. Why? Because a lot depends upon it, including the team’s own personal and professional growth, their openness to redirection and positive criticism and their impact on others, including patients. Ultimately, patient happiness, treatment acceptance, retention and referrals are all influenced by the dental team. Human beings are very porous. When the team is not happy, they transfer their emotional state onto the patient. There’s a new discovery called mirror neurons that show we imitate the behaviors of the people we’re interacting with and mimic the emotion that accompanies those behaviors. So, in other words, if the team is not happy, patients will pick up on the negativity and, more than likely, will not end up as happy, quality patients. Conversely, if your team is truly happy, their behavior will reflect that mental state and patients will, again, begin to mimic them.

Creating Happy PatientsBut just like having a happy environment does not necessarily make a team happy,

having a happy team does not guarantee happy patients. There’s a bit more to it than

that. Start by identifying if there is a problem with patient happiness. There are two ways to find out. First, measure and monitor key practice vital signs like patient retention, treatment acceptance and referrals. If they are not healthy, your patients are not happy. The second way is to simply ask

a patient if there’s a problem if their behavior or body language seems negative. Researchers interviewed patients at the Mayo Clinic in Scottsdale, Arizona and

Rochester, Minnesota and identified seven ideal physician behaviors.1 Patients want their doctors to be confident, empathetic, humane, personal, forthright, respectful and thorough. Patients also want their health problems to be properly diagnosed and completely treated. It’s about respecting the patient, listening to the patient’s concerns and addressing or providing solutions to those concerns. This means that as a healthcare provider, they want and expect us to let them know the current health of their mouth, all issues and concerns and treatment solutions. So, we need to confidently, empathetically and respectfully communicate the results of the patient’s examination and treatment recommendations without filtering it with our own perception of the patient’s willingness or ability to accept the dentistry or invest in their oral health.

If this is done consistently and with compassion, it will be difficult for patients to be unhappy with the dentist or team. Now, they may be unhappy with the extent of care they need, the limitations of their insurance benefits, the time needed to achieve oral health and the cost of care. But if happiness is a system and a goal in your practice, your team is more likely to proactively provide solutions to these patient concerns. They will work with patients to maximize their benefits, patiently explaining how insurance really works. They will try to consolidate appointments to minimize the patient’s time investment. And they will proactive offer financing solutions, like CareCredit’s healthcare credit card, so the patient is happy about how the cost of care fits into their monthly budget.

Creating a happy environment, happy team and happy patients must be done systematically and deliberately. The good news is that when you create and maintain happiness in your practice, you’ll end up happy, too. Authentic happiness is contagious. So, every day take advantage of the opportunity to get everyone on your team and every patient in the chair – infected! F

Poor oral hygiene and associated periodontitis can lead to heart disease, diabetes, respiratory diseases and perinatal complications. Conversely, smoking, diabetes, genetic factors and pathogenic bacteria increase risk for periodontal disease.

Periodontitis is a set of inflammatory diseases affecting the periodontium, which is comprised of the gingiva, alveolar bone, cementum and periodontal ligament. Periodontitis is caused by microorganisms that adhere to and grow on the tooth’s surfaces, along with an overly aggressive immune response against these microorganisms. Locally this can lead to progressive loss of the alveolar bone around the teeth, and if left untreated, can lead to the loosening and subsequent loss of teeth. Systemically, the exaggerated inflammatory response can cause pathology throughout the body.

Paradoxically, periodontitis is both under diagnosed and under treated. In the early stages, periodontitis has very few symptoms; in many individuals the disease has progressed significantly before they seek treatment. Given the importance of periodontal disease in causing local as well as systemic inflammation and disease, improving the diagnosis and treatment of periodontitis is a significant opportunity to improve the patient’s overall health. This procedure is designed specifically for patients who have severe bone loss to the upper and lower jaw, advanced gum disease, multiple missing teeth, jaw relationship discrepancy or difficult cosmetics. Lasting and predictable results can be achieved.

Edentulous patients typically had to choose between a loosely fitting denture and the numerous surgeries and implants associated with traditional implant supported prostheses. Because traditional dentures have no implant support at all, they demonstrate poor stability and retention, and require multiple realignments. Traditional implant supported prosthesis can require six, eight, or even more implants per jaw, and often involve bone grafting.

In contrast, placement of multiple dental implants at diverse angles will take maximum advantage of the patient’s available bone while gaining more favorable distribution within the dental arch. This allows patients to avoid additional bone grafting procedures with the associated additional cost, longer treatment times and the need for removable prosthesis. This technique can use as few as four implants per arch and can allow the placement of an immediate full fixed-detachable screw retained prosthesis.

health could alter these factors and make people who get coverage more likely to use it. Improving the economy and the unemployment rate could also affect the problem for the better.

The number of providers available in the market could also affect the likelihood that patients will use their dental coverage, supporting the development of programs encouraging people to enter the field of dentistry.

The data also indicate that getting people to use dental coverage to seek care is not a short-term process, Dr. Manski said.

“We need to set long-term goals for such things and understand that dental coverage and use is a long-term issue, so that we don’t get frustrated that rates of use aren’t going up right away,” Dr. Manski said.

Oral health is a critical part of a person’s overall health, and the study has implications for other types of health insurance as well, Dr. Manski said.

“Dentistry and dental coverage is a perfect experimental model for health care,” he said. “There are lessons to be learned for overall health coverage and use as well.”

Marko Vujicic, Ph.D., managing vice president of the ADA Health Policy Resources Center, which has studied older Americans’ utilization of dental services, praised the study but said it’s important to note that it focuses on people 50 and older. Many studies, including those published by HPRC, have shown the 50 plus population is the group where dental insurance has a much lesser impact on dental care use, mostly because of a shift in the source of financing, Dr. Vujicic said.

“The evidence suggests that baby boomers place a high value on dental care, have invested in their oral health and are more willing to pay out of pocket for their dental care when they lose coverage,” Dr. Vujicic said.

“The generation behind them, however, is totally different. As our HPRC studies and others have also shown, the access to care issue, the decline in dental care use, the increased financial barriers to care, are all rising among working age adults, especially those under 35. From a policy perspective, all of this evidence emphasizes that expanding dental insurance among baby boomers might not be the most critical issue in improving access to care.” F
