ECCS Impact State Overview:• Overview: ECCS Impact builds upon NJ’s existing progress in the
development of an integrated, comprehensive early childhood system of care for families from pregnancy to age five.
• 5 Placed Based Communities (PBC’s) (Camden, Cumberland, Essex, Middlesex and Passaic counties) along with the State Impact Grantee (NJ DCF) participate in the ECCS Impact Collaborative Innovation and Improvement Network (CoIIN) – a collective impact approach to enhance NJ’s comprehensive, coordinated preventive health system.
• Primary Aim of ECCS CoIIN:
• Participating communities will show a 25% relative increase in children birth to age 3 that are achieving age appropriate developmental health/skills in all 5 developmental domains within the community
• Strengthen state and local leadership in CQI expertise to inform change ideas to enhance existing EC system building on prior work through NJ-ECCS/Help Me Grow to ensure an integrated system of care to improve infant/ child outcomes, eliminate disparities, support parents and families, and strengthen communities.

ECCS Impact State Overview:ECCS Impact Strategies to achieve ECCS Aim:
• Universal promotion of child’s developmental heath through use of the CDC’s “Learn The Signs. Act Early.” developmental health materials (milestone moments booklet, brochure, tracker app, etc;)
• Implementation of HMG NJ Child Developmental Passports as a parent education and empowerment tool – so that parents are aware of their child’s developmental health, milestones, immunizations and developmental screenings conducted. Central Intake info. to promote early linkages to services.
• Implementation of Universal Developmental Screening through Brooke’s Publishing Ages and Stages Questionnaire Family Access Portal to expand the reach of access to FREE developmental screening (ASQ-3 and ASQ -SE access to families with children birth -5yrs. (Expands Central Intake’s ability to track a targeted health outcome -developmental health and social emotional health).
• Develop and Implement a Child Profile into Population Based Database (SPECT) – to track presenting (child outcomes) needs of child, referral, and linkage, follow-up and ongoing developmental screening results and follow-up. 8

• February 21st, 2018 concerning strategies to leverage Title V work to support early childhood developmental screening.
• April 19th, 2018 Identify strategies to support the use/acceptance of developmental screening (ASQ-3 and ASQ SE-2) with diverse populations
• NJ Learning Session/Harvest Party - February 28th, 2018 (State Team and Placed Based Community (PBC)) – Meeting, ongoing learning, planning
and support.
• NJ Learning Session: Grow NJ Kids presentation, Family Access Portal Update and CQI Mini Learning Session
• Harvest Party: Reflecting on work thus far and celebrating each states accomplishments in the CoIIN.
• Help Me Grow National Conference: April 4th – 6th
• Learning Session 4 & Annual Impact Grantee Meeting: July 17th-19th

ECCS Impact Updates:PDSA updates – Recap Focus of PBC by county:
• Cumberland: Developmental Health Promotion and monitoring using the LTSAE resources and child development passport through engagement of Parents in the FQHC with a pediatrician, EI (children who are ineligible for EI) and Library (Toddler Time).
• Essex: Continuing universal developmental screening promotion using the Brookes Publishing Family Access Portal. Reaching out to local health departments to find out the protocol concerning serving children with high lead levels and how the team can collaborate around linking families to screening and other services. Conducted a pilot Books Balls & Blocks events.
• Middlesex: Continuing outreach and developmental health promotion at FQHC. Initiate new protocol for Social Determinants of Health and Family Engagement surveys at FQHC. Update Family Toolkits with universal resources like nutrition and following up with families around feedback of the toolkits.
• Passaic: Collaborated with Family Success Center and Child Care Center to increase awareness of the importance of childhood developmental milestones with parents. Family toolkit for developmental milestones is being distributed to families after a short presentation. Had Books, Balls and Blocks activities on March 24th
• Camden will highlight their work today via presentation to the ICHC.
