CENTRAL FILL MODELSHospitals are evaluating the insourcing of sterile products through two
primary approaches. One approach for insourcing is a central operation
independent of existing hospitals (such as a distribution center or home
infusion), or the creation of a new central facility. This may be more capital
intensive but it provides the opportunity for superior workflow design and
processes. The expansion of compounding operations within a large hospital
is another approach. This expansion model will be less capital intensive. It may
focus on patient-specific doses or PN preparation and is often considered
a first step to establishing a stand-alone facility.
SINGLE SITE MODELIf you create an insourcing pharmacy in a single site model, there is
opportunity to improve your operations and realize savings as well.
The investment will be less, but just as a large IDN, you’ll have more
flexibility and control with your in-house sterile IV compounding.

• Data is collected through observation of practices and product utilization. Interviews with pharmacy staff will include 2–3 patient care areas and will require one day on-site.

Introduction
OVERVIEW
The TPN (Total Parenteral Nutrition) Order Analysis Tool is a tool for clinicians to identify potential opportunities to incorporate Baxter’s CLINIMIX and CLINIMIX E into a patients total parenteral nutrition program.
The tool also provides a printable PDF of the analysis results:
CLINIMIX and CLINIMIX E Injections are contraindicated in patients having intracranial or intraspinal hemorrhage, in patients who are severely dehydrated, in patients hypersensitive to one or more amino acids and in patients with severe liver disease or hepatic coma. Solutions containing corn-derived dextrose may be contraindicated in patients with known allergy to corn or corn products. The TPN Order Analysis Tool is not intended to provide or replace good clinical practice regarding the proper treatment of individual patients. It is intended as a tool for clinicians to identify potential opportunities to incorporate manufacturer prepared remix parenteral nutrition into a patients total parenteral nutrition program. It is solely within the clinician's professional judgment as to what protocol should be prescribed for a patient. Baxter encourages you to read and follow all prescribing information for CLINIMIX Injections. It is essential that a carefully prepared protocol based on current medical practice be followed, preferably by an experienced nutrition support team.

1. From Outlook, open the TPN Order Analysis email.
2. Click the TPN Order Analysis Tool link. (https://webapphosting.inbaxter.com/tpnoa)
3. In the login box, type your Passport ID (6+1) and your network password. Click the LOGIN button.
4. The tool requires that you read a pop-up statement regarding the tool’s intent. Check the box in the lower, left corner to designate you read the statement and then click the CONTINUE button.

Create New Analysis
An analysis enables the Sales Rep to enter current patient solution details into the tool to see if CLINIMIX injection can be substituted.
1. From the home screen, click the CREATE NEW ANALYSIS button. The Search field that appears enables you to find the hospital or organization for which you are conducting the analysis. You can search by hospital’s zip code, name, state, or account number. Since the tool knows you are logged in, the search is directed towards hospitals/organizations in your area. 2. Type in the search criteria and click the SEARCH button.
3. From the search results, click the SELECT link from the appropriate option.

The Hospital Analysis Information page displays with the contact’s information fields auto-populated. 4. If needed and known, type information into the blank Contact fields.
5. Type a name for this analysis in the Analysis Name field.
6. If needed, type notes pertinent to this analysis in the Analysis Notes field.
The name you choose will be the name used in the Project column of the overall analysis.

1. Select the appropriate format to enter the total daily Dextrose and Amino Acid amounts for each case.
2. For assistance on choosing, click the question mark icon beside each choice to review the format options along with completed examples.
For the purpose of this guide, we’ll choose the Final Concentration case entry - the other options require similar steps.

ENTERING THE NUMBERS
Before you enter your numbers, you have a choice on how to enter them. You can type the entries on the screen shown below, or download a spreadsheet, type your entries there, and then upload those numbers into the tool. Important Note: For the results to be calculated and printed onto a report, you need to enter numbers for at least 25 patients. If you cannot complete all 25 at once, you can click the EDIT link from the home screen and complete additional entries. Choose how you want to enter the patient’s numbers: 1. Type the patient’s numbers into the blank entry fields.
2. Click the ADD button.
3. Repeat Steps 1 and 2 until you have entered at least 25 patients.
Or…
1. Click the DOWNLOAD link to put all your entries into a spreadsheet.
The entry fields are intelligent enough to know if your entry is outside of allowable parameters and will generate an error message if outside.
The columns without entry fields are auto-calculated based on your entries.
Continue to the next section entitled Analysis Results.

The Initial Concentration spreadsheet opens.
3. In the yellow bar, click the ENABLE CONTENT button.
The spreadsheet may take approximately 15 seconds to open.

4. Type the patient’s numbers into the blank entry fields.
When done with all entries, save the spreadsheet to your laptop to upload in the next step.
5. Back in the tool, click the CHOOSE FILE button.
6. Locate your previously saved spreadsheet.
7. Click the spreadsheet file and then click the OPEN button.
The entry fields are intelligent enough to know if your entry is outside of allowable parameters and will generate an error message if outside. A range of allowable entries are shown in yellow text above each column.
The columns containing pre-filled zeroes will auto-calculate based on your entries.
It is recommended to change the spreadsheet’s name to something related to this particular analysis.

The upload begins processing.
Once done, you should see the phrase Uploaded Successfully. Also notice that the screen’s title changes from New Analysis to Manage Analysis.
